A pitcher should stop throwing as soon as a hand blister or developing hot spot becomes painful, uncomfortable, or red. Stop too if the pitcher changes grip or mechanics to protect the area; there is no established blister-specific pitch-count or pain-score threshold that makes throwing through it safe.

Use symptoms—not a pitch count—to decide when to stop

The American Academy of Dermatology (AAD) advises stopping the activity if pain, discomfort, or skin redness appears. The American Orthopaedic Society for Sports Medicine (AOSSM) similarly advises youth baseball players never to throw with pain. There is no validated blister-specific limit based on number of pitches, blister size, or a pain score in the available guidance. A grip or delivery change is a sensible additional stop signal: it suggests the pitcher is compensating for the skin problem, although that criterion is a practical precaution rather than a published medical cutoff.

Why a pitcher’s hand can blister

The review’s abstract reports that, since 2010, six Major League Baseball players accounted for seven disabled-list stints due to blisters, totaling 151 days. It also reports that eight minor-league players spent time on the disabled list due to blisters since 2012. Those are historical figures reported in a 2016 review, not current incidence estimates.

What to do after stopping

  • Reduce friction and keep the blister clean. Cover it loosely with a clean dressing or blister bandage outside play.
  • Usually leave the blister roof intact; do not peel or pop it as a routine measure. AAD and MedlinePlus both advise protecting the area, though they describe exceptions for some large, painful blisters. If drainage is being considered, ask a health professional rather than treating it as the default. MedlinePlus blister guidance
  • AAD says most blisters heal on their own in one to two weeks. That is general skin-care guidance, not a pitcher-specific rest period or return-to-throwing timetable.

When a blister needs medical attention

Contact a health professional if the blister has pus or other concerning drainage, surrounding redness or warmth, significant swelling, marked or worsening pain, or if fever develops. AOSSM advises evaluation for persistent symptoms, pain, swelling, dysfunction, loss of sensation, or bleeding that will not stop. These signs call for assessment rather than a return to throwing.

When can the pitcher throw again?

The cited guidance does not establish a pitcher-specific clearance protocol or required number of rest days. A cautious practical threshold is that the skin tolerates gripping and throwing without pain or a compensatory change in grip or mechanics. Persistent or recurring symptoms warrant assessment by a clinician or sports medicine professional.

Check the league’s rules before taping a pitching hand

MLB’s glossary says a pitcher may not pitch with an attachment—such as a bandage, tape, or bracelet—on either hand or wrist. A dressing can still be useful for protecting the skin off the field, but do not assume it is allowed during pitching.
