For an accurate cholesterol test, follow the instructions from the clinician or laboratory that ordered it. Many routine lipid panels can be done without fasting; fasting is used when a person’s history or earlier results make it useful. If your order says to fast, confirm the exact instructions rather than assuming that every test requires the same preparation.

What is a cholesterol test?

A cholesterol test, often called a lipid panel, is a blood test that measures fats in the blood. The standard panel usually reports total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides. Your clinician orders the test and uses the results alongside your health history to assess cardiovascular risk.

The American Heart Association notes that a clinician may order a lipid panel and that the test can be fasting or nonfasting depending on the situation. An advanced lipid panel may include measures such as apolipoprotein B (apoB) or cholesterol-particle counts; it is different from the standard panel and is not necessary for everyone.

Do you need to fast before a cholesterol test?

Not necessarily. The American Heart Association says testing may be fasting or nonfasting, and the 2026 ACC/AHA dyslipidemia guideline recommends either approach for baseline lipid levels and risk assessment in adults and children. Use the instructions for your specific order: if your clinician or lab has requested fasting, do not switch to a nonfasting test on your own.

Approach When it may be used Practical point
Nonfasting lipid profile Suitable for many baseline assessments. The clinician may choose it based on your situation. You may not need to schedule the test around a fasting period, but follow the order’s instructions.
Fasting lipid profile May be requested when triglyceride-related conditions are known or suspected, after a nonfasting triglyceride result of 400 mg/dL or higher, or in some cases involving relevant family history. Fast only for the interval and under the conditions specified by your clinician or laboratory.

These are not competing rules: nonfasting testing is appropriate in many situations, while fasting can provide information a clinician needs in particular cases. The 2026 guideline identifies higher triglyceride results and certain clinical or family-history contexts as reasons to consider fasting. Your care team determines which approach fits your order.

How long should you fast if instructed?

Official patient guidance gives slightly different ranges for cases where fasting is requested. CDC and the National Heart, Lung, and Blood Institute (NHLBI) describe 8 to 12 hours; NIH’s patient health page says 9 to 12 hours. Ask the ordering clinician or laboratory which interval applies to your test.

Do not assume that fasting means the same thing for every detail. The sources do not set one universal instruction for water, coffee, medicines, alcohol, exercise, or changes to your usual diet. Confirm those points with the clinician or laboratory, and do not stop or alter prescribed medication unless they tell you to.

How to prepare for your appointment

  1. Check the order. Confirm whether the test is fasting or nonfasting and when and where it will be collected.
  2. Ask about anything unclear. If fasting is required, verify the start time, duration, and what you may have during that period, as well as what to do about medication and your usual routine.
  3. Follow the instructions you receive. If you cannot follow them or are unsure whether you did, contact the ordering clinician or laboratory before the draw rather than guessing.
  4. Review the result with your healthcare professional. Ask how the numbers fit with your history and whether any follow-up is needed.

How should you understand the results?

A lipid panel is one part of an overall health assessment, not a standalone diagnosis or treatment plan. CDC says clinicians consider cholesterol numbers along with factors such as age, family history, sex, lifestyle, and health factors including smoking. Discuss your results with your healthcare professional, who can interpret them in your individual context and advise whether follow-up is appropriate.

The scale of the issue helps explain why testing matters, but it does not set an individual treatment threshold: CDC reported that 86.4 million U.S. adults aged 20 or older had high or borderline-high total cholesterol (at least 200 mg/dL) during 2017–2020. That is a population statistic, not a rule for diagnosing or treating any one person.
