How hearing aids and cochlear implants differ

How a hearing aid works
A conventional hearing aid has a microphone, an amplifier and a speaker. The microphone picks up sound, the device increases the signal, and the speaker sends it into the ear. The aid does not replace the inner ear’s role: surviving sensory hair cells still need to help convert those vibrations into neural signals.
That is why more amplification is not always the answer. Hearing-aid benefit depends partly on the type and severity of hearing loss and on how well the inner ear can use the amplified signal. Amplification also has practical limits, and a device may make unwanted background sounds louder along with speech. The FDA outlines hearing-aid benefits and limitations.

How a cochlear implant works
A cochlear implant has external and internal parts. An external microphone and speech processor capture and arrange sounds. A transmitter sends the signal to a receiver under the skin, which passes it to electrodes placed in the cochlea. The electrodes stimulate the auditory nerve, bypassing damaged sensory hair cells. The brain receives a pattern of electrical signals rather than sound delivered through the usual hearing pathway. NIDCD’s overview of cochlear implants describes this distinction from hearing aids.
The internal receiver and electrode system require surgery; the external processor is worn outside the body. After activation, users may need programming adjustments and practice to learn to interpret the new signal. An implant does not restore normal hearing, and its sound can take time to become useful for communication.

Who may benefit from each option
Hearing aids
Hearing aids may help when amplification makes sound more accessible to the inner ear. NIDCD says they are primarily useful when hearing loss results from damage to inner-ear sensory hair cells, because amplification can make vibrations more detectable by surviving cells. The right device and fitting depend on a hearing evaluation that identifies the type and degree of hearing loss.
Over-the-counter hearing aids in the United States
The U.S. OTC category is intended for adults who believe they have mild to moderate hearing loss. OTC devices can be bought without a health-care professional examination. NIDCD advises seeking professional help if an OTC aid does not help or if loud sounds are difficult to hear. Cochlear implants
FDA describes cochlear implants as designed for adults and children with severe to profound hearing loss who get little or no benefit from hearing aids. That is a broad description, not a personal candidacy rule. An implant center assesses factors such as hearing-aid benefit, cochlear and auditory-nerve structure, health, communication goals and the person’s circumstances before advising whether an implant is appropriate.
Children and early evaluation
In the United States, NIDCD reports that eligible children have been FDA-approved for cochlear implantation beginning at 9 months of age since 2020. The age figure describes an approval context, not a universal recommendation or proof that any child is eligible. Early assessment can help families understand options and support access to sound; children with implants may need intensive therapy to develop speech and language skills. What benefit and adjustment can look like
With hearing aids
Hearing aids can improve awareness of sound and communication, but they do not restore normal hearing. Because they amplify sound, background noise may also become more noticeable, and adjustment can take time. If the inner ear cannot make enough use of the amplified signal, simply increasing volume may not provide adequate benefit.
With cochlear implants
Implant outcomes vary widely. FDA says results range from near-normal speech understanding to no hearing benefit. Factors associated with results include how long a person has been deaf, age at onset and implantation, implant use, learning and support, the structure of the cochlea and nerve cells, and implantation variables. These factors do not allow a general article to predict an individual’s outcome. The FDA’s benefits-and-risks overview is a starting point for discussing possible benefits and risks with a clinician and implant team.

How to decide what to explore next
- Arrange a hearing evaluation. An audiologist or other hearing professional can assess hearing and help identify the type and degree of loss.
- Discuss hearing-aid options. Ask whether a professionally fitted aid or, for an adult in the United States who believes they have mild to moderate loss, an OTC device is suitable.
- Ask about an implant evaluation if hearing aids provide little or no benefit. A cochlear-implant team can assess candidacy, discuss surgery and rehabilitation, and explain the factors that may affect outcomes.
- Include communication goals and practical circumstances. The decision involves more than a hearing-test result; health, goals, support and willingness to take part in follow-up and learning are also relevant to an individualized discussion.
