In the U.S., a referral and prior authorization are two different insurance checks that can affect specialist care. A referral is a clinician’s direction to a specialist and may be required for the plan to pay for the visit. Prior authorization is the health plan’s advance review of a particular service or treatment. Your plan may require one, both, or neither, so check its current rules before scheduling when circumstances allow.
Referral and prior authorization are not the same
A referral generally comes from a primary care clinician or another clinician authorized by the plan. It directs you to a specialist; depending on your plan, it may also be a condition for the plan to pay for the specialist visit. The Centers for Medicare & Medicaid Services (CMS) explains that a patient may need a referral from a primary care provider before seeing a specialist for the health plan to pay for the visit in its Coverage to Care Roadmap.
Prior authorization—also called preauthorization, prior approval, or precertification—is the health plan’s decision about whether a specified service, treatment, prescription drug, or durable medical equipment meets the plan’s medical-necessity rules. It is a review of a particular item of care, not a referral to a specialist. CMS describes referral and authorization as separate administrative transactions in its referral, certification, and authorization overview.
For example, a plan might require a referral for the specialist consultation, while separately requiring prior authorization for imaging or a procedure the specialist recommends. Whether either step is required depends on your exact plan and benefit.

Three separate checks affect specialist access
Confirming that a clinician is in network does not answer whether you need a referral or whether a proposed service needs prior authorization. These checks address different questions:
- Network: Does this clinician participate in the network for your exact plan and location?
- Referral: Must a clinician send a referral for this specialty or visit, and who is allowed to submit it?
- Prior authorization: Does the plan require advance review for the consultation or a specific service, treatment, drug, or equipment?
Plan design matters. CMS says an HMO generally limits coverage to contracted providers and generally does not cover out-of-network care except in an emergency, when prior authorization for out-of-network care has been approved, or when otherwise required by law. HMOs may require primary-care referrals to see specialists; POS plans may also require referrals. These are general descriptions, not a determination of your benefits. How to check before booking
When your situation allows, verify the requirements with both the health plan and the relevant provider offices. Ask specific questions so you can distinguish a referral from an authorization and confirm that each request has reached the right place.
- Check the specialist’s network status. Ask the plan whether the clinician is currently in network for your exact plan and location. Confirm the clinician’s identity and practice location, since network participation can be specific.
- Ask whether a referral is required. Confirm whether the plan requires one for this specialty or visit, who may issue it, and which office must send it. Ask the referring office whether it has submitted the referral.
- Ask which services need prior authorization. Find out whether authorization applies to the consultation itself or to a proposed procedure, imaging, medication, or other service. Confirm which party—the specialist, referring clinician, or another office—will submit the request.
- Confirm receipt and decision scope. Ask what records or clinical information are needed and how you can verify the plan received them. If approved, ask what service the decision covers and whether it has an expiration date.
- Check the remaining coverage and cost rules. Authorization does not by itself establish that the clinician is in network or that every part of care is covered. Ask about applicable benefits, deductibles, and cost-sharing separately.
- Ask about exceptions if needed. If care is urgent or the requested clinician is out of network, ask the plan what exceptions or review process apply to your circumstances.
What authorization does—and does not—tell you
A prior-authorization decision concerns the specific service or treatment the plan reviewed. Approval should not be treated as a blanket guarantee that all related care will be paid for: network status, benefit terms, and cost-sharing remain separate questions. Conversely, the consequences of proceeding without required approval depend on the plan, benefit, and circumstances. The official sources cited here do not establish one nationwide delay, denial rate, or outcome for patients who miss an authorization step.

What federal process changes mean for patients
CMS’s 2024 Interoperability and Prior Authorization final rule, CMS-0057-F, sets API and process requirements for defined payer groups: Medicare Advantage organizations; state Medicaid and CHIP fee-for-service programs; Medicaid managed care plans; CHIP managed care entities; and Qualified Health Plan issuers on the Federally Facilitated Exchanges. CMS says the rule is intended to improve data exchange and streamline prior authorization. It does not apply to every insurer, plan, or authorization, and it does not abolish prior authorization. CMS states that, by January 1, 2027, impacted payers must make information about non-drug prior-authorization requests and decisions available to patients through the Patient Access API. That is a future implementation deadline, and the requirement covers specified information for impacted payers—not every authorization record or every health plan.
