In indirect trial comparisons, semaglutide and tirzepatide produced larger average placebo-subtracted weight reductions than orlistat and naltrexone/bupropion; phentermine/topiramate also showed substantial average efficacy. These figures compare groups across different trials, not guaranteed personal results or a direct head-to-head ranking. The right choice depends on health conditions, safety, treatment logistics, access, and patient preference.
How the medicines compare on average weight loss
The American Diabetes Association’s 2025 standards summarize a network meta-analysis estimating average weight reduction beyond placebo at each study’s endpoint. The analysis combined results from randomized trials of different medicines; the trial populations, durations, and designs differed. The values are therefore useful for broad comparison, not a prediction for an individual or proof that one drug will outperform another in a direct comparison.
| Medicine | Average placebo-subtracted weight reduction | Trials included in the estimate |
|---|---|---|
| Orlistat | 3.1% | 22 |
| Phentermine/topiramate | 8.8% | 5 |
| Naltrexone/bupropion | 4.8% | 2 |
| Liraglutide | 4.5% | 10 |
| Semaglutide | 11.9% | 13 |
| Tirzepatide | 16.2% | 6 |
All percentages and trial counts in the table are estimates reported in the American Diabetes Association’s 2025 summary of the network meta-analysis; “study endpoint” means the end of each included trial’s treatment period, not one shared duration. Read the ADA’s pharmacologic treatment standards.
What “GLP-1” means in this comparison
Semaglutide and liraglutide act on the GLP-1 receptor. Tirzepatide is different: it activates both GIP and GLP-1 receptors, so it is more precise to describe it as a dual GIP/GLP-1 receptor agonist rather than a GLP-1-only medicine. The ADA’s 2025 standards discuss these medicines among options for weight management and, for people with type 2 diabetes and overweight or obesity, favor semaglutide or tirzepatide among treatments with greater weight-loss efficacy. That guidance addresses a particular clinical group; it is not a universal prescription for everyone seeking weight loss. See the ADA’s 2025 standards on obesity and weight management in type 2 diabetes.

Medication is used with lifestyle care
Prescription weight-loss medicines do not replace healthy eating and physical activity. NIDDK says adults taking medication as part of a lifestyle program lose an average of 3% to 12% more of their starting weight after one year than adults using a lifestyle program without medication. This is a broad comparison across prescription medicines, not an estimate for any one drug. NIDDK explains prescription medications for overweight and obesity.
How to weigh efficacy against suitability
The average efficacy estimates answer only one part of the decision. A clinician can help compare the options against the person’s medical history and goals, including:

- Related health conditions: For someone with type 2 diabetes and overweight or obesity, glucose and cardiometabolic benefits may matter alongside weight change.
- Safety and current labeling: Check the current U.S. prescribing information for the specific medicine under consideration. A warning on one drug’s label should not be assumed to apply identically to every comparator.
- Practical fit: Route and frequency of administration, expected treatment duration, follow-up needs, affordability, availability, and personal preference can affect which option is workable.
- Pregnancy plans: Discuss plans and timing with a clinician before starting or continuing treatment; medicine-specific labeling matters.
The available comparison does not establish a complete side-by-side account of every older medicine’s contraindications, interactions, or adverse effects. Those details should be checked in each medicine’s current prescribing information rather than inferred from its weight-loss estimate.
Semaglutide safety details are drug-specific
The FDA’s 2025 WEGOVY prescribing information says acute pancreatitis has been observed in patients treated with GLP-1 receptor agonists and that acute gallbladder disease occurred in clinical trials. For semaglutide used for weight reduction, the label advises stopping treatment at least two months before a planned pregnancy. These are semaglutide-label details, not a full safety comparison and not a substitute for reviewing the label of another medicine. Read the FDA’s 2025 WEGOVY prescribing information.

Scope of the comparison
This is a U.S.-focused comparison based on the cited 2025 ADA standards, FDA semaglutide labeling, and NIDDK guidance accessed in 2026. Approvals, labels, and availability can change, and the network estimates do not establish what a particular person will lose or which medicine is safest for them.
