After stopping a GLP-1-based weight-loss medicine, many people regain some of the weight they lost, and some improvements in cardiometabolic health may fade. Clinical trials show that this can happen over the following year, but their averages do not predict what will happen to any one person. If you are considering stopping—or have missed several doses—make a plan with your prescriber, particularly if you also take the medicine for type 2 diabetes.
What withdrawal studies found
The available evidence includes two different studies: an exploratory follow-up of semaglutide trial participants and a randomized withdrawal trial of tirzepatide. Both found weight regain after participants switched off treatment, but their designs and reported measurements differ.

The numbers are group averages, not a schedule or forecast for an individual. The 14.0% in SURMOUNT-4 describes change during the randomized withdrawal period; it is not 14% of the weight participants had originally lost. The STEP 1 extension’s 11.6 figure is a percentage-point regain in body weight, not a guarantee that each participant—or each reader—will regain the same fraction.
Can other health benefits change?
Weight is not the only outcome to consider. In the STEP 1 extension, improvements in cardiometabolic measures moved toward baseline for most variables measured by one year after semaglutide and lifestyle intervention stopped. That does not mean every measure changed, or that every person experienced the same course. The two withdrawal studies cited here enrolled participants without diabetes, so they cannot establish what will happen to blood glucose when someone with diabetes stops treatment.

Is there an immediate withdrawal syndrome?
The cited studies report longer-term changes in weight and health measures; they do not provide a complete account of short-term symptoms after the final dose or establish one universal withdrawal syndrome. They also do not show that everyone regains all the lost weight, specify how quickly an individual might regain weight, or prove that a particular diet, exercise plan, supplement, or consumer product prevents regain.
How to decide whether to stop
Stopping is a treatment decision to make with the prescriber, not a dose change to improvise. The American Diabetes Association’s 2025 standards recommend continuing obesity medication after treatment goals are reached to maintain health benefits, noting that discontinuation often brings weight recurrence and worsening or reemergence of cardiometabolic risks or obesity-related conditions. The guidance calls for an individualized decision that considers effectiveness, benefits, and tolerability; possible approaches include a suitable maintenance dose, a different treatment, or stopping with close monitoring. Before changing treatment, ask your prescriber:
- What should be monitored after a change, and how often?
- If the medicine also treats diabetes or another condition, what is the plan for managing that condition?
- Would a maintenance dose or another treatment be appropriate?
- What should you do if treatment is interrupted or you want to restart?
Do not stop, taper, restart, or change a dose on your own. A weight-loss trial in people without diabetes cannot estimate an individual’s glucose response. If you take semaglutide for type 2 diabetes, arrange a diabetes treatment and monitoring plan with your prescriber.

Instructions depend on the specific medicine and formulation. The U.S. Wegovy prescribing information says that after two or more consecutive missed doses, reinitiating dosage escalation at a lower dosage may reduce gastrointestinal adverse reactions. This is a missed-dose instruction for Wegovy, not a general stopping or restarting plan and not a rule for other GLP-1 medicines.
