The American Academy of Pediatrics (AAP) and Johns Hopkins Medicine offer practical carrying guidance, not a controlled, current head-to-head comparison. Neither bag type should be described as universally safer or as a guaranteed way to prevent pain.
When a backpack is the better default
A backpack is a sensible starting choice when the student carries books, a laptop, or other supplies over a school day. Look for a lightweight bag proportional to the child’s height, with two broad, adjustable shoulder straps and a padded back. Compartments can help organize supplies, and the AAP suggests considering a waist or chest belt.
Use both straps evenly and keep the backpack close to the body. Put heavier items nearest the back, carry only what is needed, and clear out unnecessary contents regularly. These steps follow AAP and Johns Hopkins advice; they do not guarantee that a particular bag will feel comfortable for every student.

When a messenger bag can work
A messenger bag may suit a student who prefers its style or access to contents. Johns Hopkins advises wearing it across the body, not hanging it from one shoulder. Keep the load as light as practical by removing items the student does not need.
The reviewed sources do not show that messenger bags are inherently harmful, nor do they establish that wearing one crossbody matches the load distribution of a two-strap backpack. If the student has pain, numbness, or tingling, Johns Hopkins advises seeking medical advice rather than treating a change of bag as a diagnosis or cure.

How heavy should a school bag be?
The AAP’s HealthyChildren.org guidance, on a page reviewed in 2026, recommends that a child’s backpack weigh no more than 15% of the child’s body weight. Treat this as the AAP’s recommendation, not a universally settled cutoff: recommendations differ, and the material reviewed here does not establish one threshold as proven for every student or bag type.
Whatever limit a family follows, first reduce avoidable weight: pack only what is needed and place heavy items close to the body. If the load remains too heavy, the AAP says a rolling backpack may be considered if the school allows it. Stairs, crowded corridors, and school rules can make a wheeled bag impractical.

What injury figures do—and do not—tell you
The US Consumer Product Safety Commission estimated an annual average of 1,200 backpack-related emergency-room injuries among children under 19 from 2019 through 2021. That figure concerns backpack-related injuries generally; it does not compare backpacks with messenger bags or show which type is safer.
An AAP Grand Rounds commentary published in 2008 summarized a small experiment involving ten young teens and backpack load and wearing conditions. It reported higher perceived pain at loads of 30% of body weight than at 10% or 20%, and higher pain with one strap than with two in the tested conditions. The small sample and backpack-only setup make it unsuitable as a direct messenger-bag comparison or a universal estimate of effect.

In an August 24, 2026, American Academy of Orthopaedic Surgeons release, pediatric orthopaedic and spine surgeon Sumeet Garg, MD, FAAOS, said: “A backpack may seem harmless, but when it is too heavy, poorly fitted or carried incorrectly day after day, it can put unnecessary strain on a child’s back, neck and shoulders,”

Quick school-bag checklist
- Choose a lightweight bag that fits the student’s height.
- For a backpack, check for two broad adjustable shoulder straps and a padded back; have the student wear both straps.
- For a messenger bag, have the student wear the strap across the body.
- Pack only necessary supplies and put heavy items nearest the back in a backpack.
- Check the school’s rules and the student’s route before choosing a rolling bag.
- If pain, numbness, or tingling occurs, seek medical advice.
