Measure code review quality with a small, team-level set of signals about feedback usefulness, quality follow-through, escaped defects and rework, workflow, and developer learning. Treat pull request counts, comment counts, and review speed as activity or context—not as quality targets. No universal, validated score or threshold for “good” code review is established by the available evidence.
Why pull request volume is the wrong quality target
Pull request (PR) volume tells you how many changes moved through a workflow; it does not tell you whether reviewers understood the changes, caught meaningful risks, improved maintainability, or helped the author. Comment counts have the same problem: a review can contain many trivial or duplicate notes, while a concise review can identify a consequential issue.
These activity measures can still help explain workload or a change in process. They become misleading when used to rank reviewers or reward higher output. Raw comparisons also ignore differences in reviewer assignments, code ownership, availability, and the complexity or risk of the changes being reviewed.
DORA’s 2025 measurement guidance distinguishes quantity, time-based, and frequency measures, and warns that logs-based measures depend on toolchain observability and interpretation. A framework can help examine complex behavior, but it cannot fully represent it. DORA’s guidance on choosing measurement frameworks is a useful basis for treating metrics as lenses rather than verdicts.

What code review quality should measure
Start by naming the outcome you want reviews to support. Code review may help prevent defects, improve design and maintainability, spread knowledge, and move work through the team. Those goals require different evidence: repository logs can show workflow events, while sampled reviews and developer feedback can reveal whether the interaction was useful.
A qualitative study of 88 Mozilla core developers found that perceived review quality was associated with thorough feedback, reviewer familiarity with the code, and perceived code quality. The study also identified context such as time pressure, organizational culture, personal priorities, and context switching. It is an exploratory study of that population, not a universal formula. A practical team-level measurement set
Use a compact dashboard grouped by purpose. Before comparing periods or teams, define what starts and ends a review, which changes are excluded, and how you handle reopened reviews, drafts, and automated changes. Keep each metric tied to a question the team can act on.

Sample usefulness, not comment volume
Periodically sample completed reviews and ask both authors and reviewers whether feedback was clear, actionable, relevant to the change, and accompanied by enough context. Use a short rubric and calibrate how reviewers apply it together. If you summarize responses numerically, show the sample size and preserve comments or examples that explain the result; do not present the rating as an objective quality score.

Record substantive findings as follow-through
For sampled reviews, record whether a substantive concern was raised and whether it led to a change, clarification, or documented decision. Categorize concerns—for example, correctness, security, maintainability, or design—and separate them from style-only and duplicate comments. The purpose is to understand what review work contributed, not to set a target number of findings: a high count may reflect risky changes or repeated problems rather than better reviewing.
Track defects and rework as lagging signals
Track post-merge defects, rollbacks, or rework related to changed code using consistent attribution windows and severity categories. For meaningful cases, ask whether the issue was detectable during review and whether review was the relevant control. A replication and Bayesian-network study using Qt and Google Chrome data found that review-measure relationships with post-release defects were unstable; models without review predictors performed as well or better, and review measures did not directly affect defects in the combined model. Prior defects, module size, and authorship had stronger relationships in that study. Read the study.

Consequently, an escaped defect is a reason to examine the system, not evidence that an individual reviewer failed. Likewise, a low defect count does not prove reviews were effective: change mix and other detection mechanisms matter.
Use flow measures to find bottlenecks
Useful workflow signals can include time to first substantive review, total time waiting for review, active review duration when it can be measured reliably, and distribution of review load. Define event boundaries precisely: for example, a first automated response is not the same event as a first substantive human review. Use the results to investigate queues, availability, or handoffs—not to reward the shortest review time.

Ask whether reviews support learning
Ask authors and reviewers whether a review clarified design decisions or helped share context. Look for recurring concerns or knowledge bottlenecks over time. These outcomes are difficult to infer from repository logs alone. Google’s modern code review case study combined interviews with a survey and logs for 9 million reviewed changes; its qualitative components included 12 interviews and 44 survey respondents. Those figures describe one company-specific study, not a universal sample. Read the case study.

- Keep the dashboard at team level. Avoid public individual leaderboards and quotas for PRs, approvals, comments, lines reviewed, or review speed. Assignment patterns and change risk can make raw individual comparisons unfair.
- Pair signals with the question they answer. A faster first substantive response may help workflow, but only if review remains useful. A defect trend needs context about change mix and where issues were detected.
- Set definitions and a baseline before changing policy. Compare like periods and types of work, annotate tool or policy changes, and investigate outliers rather than reacting to one aggregate.
- Test for gaming and harm. Ask whether the number could improve while code understanding, risk detection, maintainability, or flow got worse. If so, do not use it as a quality target.
- Keep qualitative evidence visible. A chart should prompt an investigation or a process change; it should not replace the accounts of the people doing and receiving reviews.
What changes when teams use AI-assisted coding?
Revisit output measures when AI changes how much code can be generated. DORA’s AI and SDLC guidance says generated-code volume can rise without demonstrating productivity or quality; it recommends holistic measures aligned with organizational goals and points teams toward reviewable small batches and downstream signals such as rework and incidents. Keep batch size and those downstream signals in view rather than treating more code or more PRs as proof of better performance. Read DORA’s AI and SDLC guidance.
Why anonymizing reviews is not a universal fix
Measurement and review design interact with social context, but one process change does not fit every team. In a field experiment at one company involving 5,217 code reviews and 300 professional software engineers, Google researchers withheld author identities; reviewers could frequently guess identities, and the authors reported tradeoffs involving power dynamics and high-bandwidth conversations. This finding is evidence to consider social context, not a recommendation to anonymize reviews universally. Read the field experiment.
Is there a single code review quality score?
The sources here do not establish a universally validated composite score or a numerical threshold for good review quality. A single score would also obscure tradeoffs: flow, defect prevention, maintainability, knowledge sharing, and developer experience are related but not interchangeable. Use a small set of measures tied to explicit goals, report their limits, and use sampled evidence to understand what the numbers cannot show.
