If the picture appears briefly and later disappears or your old gamerpic returns, that does not necessarily mean the upload failed. It may have been removed during moderation. Avoid content that violates the Xbox Community Standards, including hateful imagery, harassment, graphic violence, sexual content, or attempts to impersonate another person.
Why the new gamerpic may not appear immediately
Profile-picture synchronization can take up to 48 hours, depending on system load, according to Microsoft’s Xbox app guidance. Check the profile again later before repeating the upload. Re-uploading will not bypass moderation or necessarily make synchronization faster.
Fixes for common problems
The custom-upload option is missing

Allow up to 48 hours for the change to synchronize across Xbox services, as Microsoft’s guidance says synchronization may take that long depending on system load. Close and reopen the relevant app, then check the profile again.
Technical API note
Xbox’s developer documentation describes the underlying upload operation as PUT /users/me/gamerpic.
