Bone-conduction headphones leave the ear canal open, but they still deliver sound to the auditory system. That means they are not automatically hearing-safe: listening risk depends on volume, duration, and how often you are exposed. Available evidence does not show that consumer bone-conduction headphones are categorically safer or more dangerous for hearing than conventional headphones.

Can bone-conduction headphones damage hearing?

They can expose you to sound at levels and for durations that may harm hearing. Bone-conduction devices transmit audio as vibrations through the skull rather than relying only on sound entering the outer ear. An open ear canal does not eliminate sound exposure or prove that a listening level is safe.

The World Health Organization (WHO) says that “The volume of sounds, the duration of listening, and frequency of exposure to loud sounds all have an impact on hearing.” Its safe-listening standard is intended to reduce hearing-loss risk among users of listening products. WHO’s reference modes specify 80 dB for 40 hours per week for adults and 75 dB for 40 hours per week for children. These are exposure reference levels, not a conversion to a device’s volume percentage or a guarantee that a particular setting is safe.

The U.S. National Institute on Deafness and Other Communication Disorders (NIDCD) says exposure at or below 70 dBA is unlikely to cause hearing loss even after long exposure, while long or repeated exposure at or above 85 dBA can cause hearing loss. “Unlikely” is not a guarantee, and the figures should not be treated as a universal safe-volume setting for every headphone or listener.

Are bone-conduction headphones safer than regular headphones?

The available evidence does not establish a consumer-headphone safety ranking. No direct long-term comparison identified here shows that ordinary bone-conduction headphones produce better or worse hearing outcomes than conventional headphones. The designs differ, but those differences alone do not prove a health advantage.

WHO estimates that more than 1 billion people aged 12–35 are at risk of hearing loss from recreational exposure to loud sound. That population estimate is not specific to bone-conduction headphones and should not be read as a product-specific risk figure.

How to reduce listening risk

  • Keep volume moderate. In quiet surroundings, lower it rather than raising it to overpower background noise.
  • Manage listening time as well as loudness; take breaks, particularly during extended listening.
  • Use available phone or device features that limit volume or monitor sound exposure. A volume percentage alone does not establish the sound level reaching your ears.
  • Pay attention to persistent ringing, difficulty hearing, or other ongoing hearing changes, and seek professional evaluation if they occur.

WHO’s safe-listening guidance recommends reducing exposure to loud sound and monitoring listening habits. Persistent tinnitus or hearing difficulty warrants consultation with a health professional.

Can bone-conduction headphones cause headaches or skin irritation?

Individual users may experience discomfort, pressure, or skin symptoms, but reliable rates for these effects from consumer bone-conduction headphones are not established here. A fit that feels uncomfortable is a reason to stop or adjust use rather than assume the symptom is harmless.

The U.S. Food and Drug Administration discusses possible adverse events, including skin irritation and suddenly worsening hearing, in its resource on bone-conduction hearing aids. That guidance concerns hearing aids; it does not establish how often consumer headphones cause these symptoms or prove that the headphones caused them. Ongoing or worsening symptoms merit medical advice.

Can you hear traffic while wearing bone-conduction headphones?

Leaving the ear canal open may let environmental sounds reach you, but it does not guarantee that you will notice or correctly locate them. A study examined localization of critical environmental sounds while distractor sounds played through bone-conduction headphones. It raises a practical concern about attention and sound localization, but does not establish an accident rate or show that every listener will miss traffic.

Stay alert near roads, and do not use headphones when they could interfere with safe movement or driving. Open-ear design is not a substitute for checking your surroundings.

Consumer headphones are not medical hearing devices

Consumer bone-conduction headphones should not be described as hearing aids or as treatment for hearing loss. Clinical guidance addresses a different category: the American Academy of Otolaryngology–Head and Neck Surgery says bone-conduction hearing devices may be appropriate, and in some cases preferred, for conductive and mixed hearing loss, and may be indicated for selected cases of single-sided deafness. It says recommendations should come from a qualified otolaryngology and head-and-neck surgeon.

Likewise, findings about bone-conduction or middle-ear implants apply to medical devices, not ordinary consumer headphones. A systematic review concluded that state-of-the-art implantable devices are a safe treatment option, while noting that adverse-event reporting in the studies was often poor. Those results do not establish the safety of consumer headphones.
