Dog dementia, or canine cognitive dysfunction (CCD), has no single cure. Treatment focuses on managing signs and supporting quality of life, often by combining veterinary care with a suitable diet and predictable, enriching routines. Medication is not interchangeable with supplements or toys, and a change in behavior alone cannot confirm CCD: pain, sensory loss, and other illnesses can look similar. A veterinarian can help identify the cause and build a plan for your dog.

What treatment can—and cannot—do

Canine cognitive dysfunction is a progressive syndrome that may affect a dog’s orientation, social interaction, sleep, house training, learning and memory, activity, and anxiety. Cornell University College of Veterinary Medicine says, “There is no one treatment or cure for CDS, and research is ongoing in this area.” The practical aim is to address relevant symptoms and make everyday life safer and more comfortable, not to reverse the condition.

If you are wondering, “Why is my senior dog pacing at night?” or “Why is my old dog having accidents in the house?”, those signs deserve attention but do not prove dementia. Pain, declining vision or hearing, organ disease, and neurologic problems can cause or contribute to similar changes. Some may be treatable, so start with an assessment rather than assuming the behavior is age-related.

Medication: what to ask about selegiline

In its 2023 Senior Care Guidelines for Dogs and Cats, the American Animal Hospital Association (AAHA) identifies selegiline as the only drug labeled for canine cognitive dysfunction. AAHA says it can be effective in up to 70% of dogs. “Up to” is an upper estimate in the guideline, not a guarantee that a particular dog will improve or an estimate of how quickly it will happen.

AAHA cautions that selegiline can have potentially toxic interactions with certain antidepressants, narcotics, phenylpropanolamine, and monoamine oxidase inhibitors. Give the veterinarian a complete list of prescribed medicines, over-the-counter products, and supplements before treatment. Do not start, stop, combine, or change medicines without veterinary direction. The veterinarian should select and monitor any medication plan.

Cornell’s owner-facing guidance says some owners notice improvement within three weeks, while others may see changes after six weeks. These observations are not a promised timeline. Ask the prescribing veterinarian when to check in and what specific changes to track. Anxiety or pain may need separate attention; a medicine considered for a particular symptom is not automatically a treatment for the underlying cognitive syndrome.

Diet and supplements: potential support, limited certainty

Cognitive-support diets

AAHA discusses diets rich in antioxidants and medium-chain triglycerides. It reports the most positive effect when diet is paired with environmental enrichment, while also cautioning that the quality and rigor of research on diets and nutraceuticals are limited.

Cornell names prescription diets including Hill’s b/d, Purina ProPlan NeuroCare, and Royal Canin Veterinary Canine Mature Consult. These are examples, not endorsements or proof that the products have equal evidence. Availability and formulations can change; confirm current options and suitability with your veterinarian, particularly if your dog has other health conditions.

Supplements

Cornell mentions Senilife, Novifit (SAMe), and Aktivait, but advises consulting a veterinarian before adding a supplement. Cornell also notes that there are few studies of supplements in senior dogs. Do not assume a product is proven effective or harmless because it is sold for older dogs. Bring the package or ingredient list to the appointment so the veterinarian can review it alongside your dog’s medications and health conditions.

Home changes that support comfort and engagement

Environmental changes are practical management, not a cure. A dog who is confused may find familiar surroundings and a predictable day easier to navigate. Keep beds and food and water bowls in familiar places, and avoid rearranging furniture unnecessarily. Regular interaction and walks, varied or interactive toys, simple food puzzles, and snuffle mats may provide suitable engagement.

A food puzzle or snuffle mat is an optional enrichment tool, not a dementia treatment. Choose one that is easy for your dog to use and supervise as needed. Adapt walks and play to mobility, pain, stamina, vision, and other health conditions; a frail or uncomfortable dog needs an individualized plan rather than more activity by default. AAHA also describes cognitive games, new toys, tasks, and agility as enrichment approaches, but activities should fit the individual dog.

Diagnosis and tracking: make the veterinary visit more useful

Diagnosis depends on the pattern of signs and on considering other causes. Cornell describes a workup that may include a physical examination, blood and urine tests, and sometimes MRI to look for conditions with similar signs. The 2025 Canine Cognitive Dysfunction Syndrome Working Group guideline organizes changes into DISHAA domains: disorientation, social interaction, sleep disruption, house soiling, learning and memory, activity, and anxiety. Its proposed Level 1 criteria include progressive signs, examinations and laboratory work to investigate alternative causes, and persistence of signs after relevant coexisting problems are managed. Proposed Level 2 criteria add MRI evidence and normal cerebrospinal fluid (CSF) cell counts. These are guideline criteria for veterinary assessment, not a checklist for diagnosing your own dog.

AAHA recommends directed questions and notes that client-completed questionnaires can support diagnosis and monitoring. Before the appointment, keep a dated log of what changed, when it happens, how often, and what else was different that day. For example, record nighttime pacing, accidents, sleep changes, appetite, mobility, and responses to familiar people or routines. A veterinarian can help decide which observations or questionnaire scores are useful to follow over time.

What clinical-practice research says about uncertainty

A 2025 survey of U.S. veterinarians received 318 responses. Among respondents, 97.2% had diagnosed canine cognitive dysfunction during their career, while about 80% rarely or never referred potential cases to a specialist. About 30% considered selegiline the most effective option among those asked about. These are survey findings about clinical experience and opinions, not a controlled treatment trial or an estimate of a dog’s chance of improvement. They should not be compared directly with AAHA’s “up to 70%” efficacy figure, which is a separate guideline estimate.

Questions to take to your veterinarian

  • Could pain, vision or hearing loss, another illness, or a neurologic problem explain or contribute to these changes?
  • What examination or tests make sense for my dog, and what would they help rule out?
  • If CCD is suspected, is selegiline appropriate given my dog’s other medicines and supplements?
  • Would a particular diet be suitable, and how should we assess whether it is helping?
  • Which home activities fit my dog’s mobility and comfort?
  • What signs should I record, and when should we review the plan?
