Choose sleep earbuds by how little they protrude, how they feel against your usual pillow, and whether you need audio, noise reduction, or both. A low-profile sleep design is a sensible starting point, but no model is comfortable for every ear or sleep position. If in-ear devices hurt, consider sleep earplugs, headband-style headphones, or a bedside speaker.
What matters most for side sleepers
When you lie on your side, your outer ear is pressed between your head and pillow. An earbud that feels comfortable while sitting upright may create pressure once you lie down. Cleveland Clinic notes that earbuds can cause overnight pain or irritation, particularly when the fit is poor. Give priority to a small housing that sits close to the ear rather than a regular earbud that sticks out.
If you can, check the fit with your ordinary pillow during the retailer’s return period. This is a practical way to assess pressure, not a clinical test. Stop wearing a device if it causes pain, persistent soreness, or irritation.
Choose between audio, noise reduction, or both
First identify what you want the device to do. Sleep earbuds play audio that can mask some background sound; sleep earplugs reduce sound without playback. Neither approach guarantees that you will block snoring, traffic, or other nighttime noise. Check playback, volume, and awareness
Prefer controls that make it easy to keep playback quiet and set it to stop. Cleveland Clinic recommends low volume, limiting listening time, cleaning devices regularly, and avoiding noise-canceling modes when you need awareness of your surroundings. A wake-up alarm played through an earbud may not be heard reliably; use a separate alarm and consider household alerts or other urgent sounds.

The World Health Organization’s 2026 safe-listening guidance says hearing risk depends on volume, duration, and frequency of exposure. Its general adult examples are 80 dB for up to 40 hours per week and 90 dB for up to four hours per week, and it recommends keeping personal-device volume at no more than 60% of maximum. These are general listening recommendations—not an overnight-use guarantee or an assessment specific to sleep earbuds. The WHO-ITU standard published in 2019 describes an adult listening mode of 80 dB for 40 hours per week and recommends features such as exposure tracking and volume limits; it does not show that a particular sleep-earbud model implements them.
Understand the risks of overnight in-ear use
Cleveland Clinic identifies potential concerns including hearing damage from loud, extended playback; earwax buildup; moisture-related infection risk; and pressure or irritation, especially for side sleepers. Keep earbuds clean, and do not wear them when your ears are wet or uncomfortable. Stop use and seek clinical advice if you have pain, hearing changes, or signs of infection.
