Crashes, No Sound, or Screen Glitches?

On December 3, 2024, the newspaper reported that the problem had been fixed and ChatGPT was responding to prompts containing it. That is a historical explanation and reported fix—not confirmation of behavior across every ChatGPT model or interface today.
What happened when users typed “David Mayer”?
In late November and early December 2024, users described ChatGPT responses that stopped after beginning to produce the name, or returned errors such as “something seems to have gone wrong” and “I’m unable to produce a response.” A contemporaneous OpenAI Developer Community discussion captured users’ questions about the failures. It documents their reports, but does not establish the technical cause.
On December 3, 2024, The Guardian quoted an OpenAI spokesperson saying: “One of our tools mistakenly flagged this name and prevented it from appearing in responses, which it shouldn’t have. We’re working on a fix.” The paper reported that the issue had been fixed and that ChatGPT was responding to queries using the name.
Why did ChatGPT fail on the name?
The best-supported explanation is OpenAI’s statement that a tool mistakenly flagged the name. The company did not identify the tool or explain its code, architecture, or how it interacted with the model. Claims that a particular hard-coded blacklist or intentional censorship mechanism caused the failure are not established by the cited reporting.

The incident also prompted speculation that someone named David Mayer had asked OpenAI to remove the name. The Guardian reported that David Mayer de Rothschild denied making such a request and said he had never had contact with ChatGPT. OpenAI declined to say whether the glitch was connected to a right-to-be-forgotten request. The available reporting therefore does not establish a privacy request as the cause.
What does this incident say about name filters?
A separate December 2024 article about similar name-related failures offered broader context, not a technical postmortem for this incident. Harvard Law professor Jonathan L. Zittrain said OpenAI told him that a very small number of names might be omitted because of privacy requests or to avoid persistent AI hallucinations. He also reported that OpenAI told The New York Times that “David Mayer” had been added mistakenly. That account does not reveal which tool triggered the failure or prove that a privacy request was involved here.

The distinction matters: OpenAI’s reported explanation addresses the Mayer glitch directly, while the broader discussion describes possible reasons for other name omissions. Neither account discloses the underlying system design.

The Guardian reported on December 3, 2024 that the issue had been fixed and ChatGPT was responding to prompts using the name. That report supports what was said at the time; it is not a current test. It does not establish that every model, interface, region, or account behaves identically today.
