If a water test finds PFAS while you’re pregnant, save the full report, contact the water authority responsible for your supply, and share the result with your prenatal clinician. Ask whether to use an alternate water source while you get local advice. A positive water test measures PFAS in the water; it does not diagnose exposure-related illness or show that a pregnancy has been harmed.

This guidance is for people in the United States, where federal agencies direct public-water customers and private-well owners to different sources of advice. The right response depends on the water source, which PFAS were tested, the reported concentrations, the testing method, and local recommendations.

What should you do first?

  1. Keep the complete test report. Save the sampling date, laboratory name, test method, PFAS compounds measured, concentrations, reporting limits, and any sample-handling notes. These details help the water authority and your clinician interpret what the result does—and does not—cover.
  2. Contact the authority for your water source. If you use public water, call the utility. Ask which PFAS were measured, what levels were reported, whether the results are current, what the utility is doing, and what it recommends for pregnant customers. If you use a private well, contact your state environmental agency or health department for local advice.
  3. Ask about an alternate water source. While you are getting advice, ask whether to use another source for water that could be swallowed. EPA’s private-well guidance includes drinking, cooking, food preparation, brushing teeth, and preparing infant formula among the activities to consider. The appropriate advice depends on your result and location; a positive finding does not automatically mean every household needs the same precautions.
  4. Tell your prenatal clinician. Share the report and describe, as best you can, how long you have used the water. Ask whether anything about your routine prenatal follow-up should change.
  5. Discuss treatment before choosing a filter. Ask the utility or state agency whether a treatment option makes sense for your water and result, and verify any filter’s specific PFAS claim and certification before buying.

How to get advice for your water supply

If you use public water

Start with your water utility. EPA’s PFAS guidance for water customers recommends asking about monitoring, measured levels, and the utility’s response. Request information specific to your service area and test result rather than relying on a general statement about PFAS or a result from another location.

If you use a private well

Contact your state environmental agency or health department about testing, interpretation, and any local recommendations. EPA says it does not regulate private wells or provide recommended standards for them. It directs well owners to their state for advice and to state-certified laboratories using EPA-developed methods. State rules and assistance programs can differ.

If your result came from a consumer home test kit, ask the state agency whether confirmatory testing is appropriate. EPA’s guidance points well owners to state-certified laboratories using EPA-developed methods; a home kit should not be treated as a substitute for that testing.

What a PFAS result means for pregnancy

ATSDR says that community exposure to high levels of certain PFAS may be associated with pregnancy-induced hypertension or preeclampsia and small decreases in birth weight. Those are population-level associations, not a prediction for an individual pregnancy. A water result alone cannot show that PFAS caused a complication or establish what will happen to you or your baby.

ATSDR recommends discussing water exposure with a healthcare provider. Its clinician guidance says usual prenatal care, including close attention to blood pressure, is appropriate. ATSDR’s public guidance notes that routine prenatal care can detect these conditions and prevent health consequences; that statement describes the role of care, not a guarantee about an individual outcome. Continue your prenatal care and let your clinician decide whether your circumstances call for any change.

Would a blood test help?

A PFAS blood test measures certain PFAS in the blood, but it does not diagnose a health problem, predict a future problem, or provide treatment information. ATSDR has not developed health-based screening recommendations based on PFAS blood levels. If you are considering testing, discuss its limits and possible usefulness with your clinician; it is not a universal next step after a positive water test.

How to evaluate a water filter

EPA identifies granular activated carbon, reverse osmosis, and ion exchange as treatment technologies that can reduce PFAS. That does not establish that every device using one of these technologies—or every filter advertised for PFAS—will work for your water.

  • Verify the exact model. EPA advises looking for PFAS-reduction certification to NSF/ANSI 53 or NSF/ANSI 58 and checking the model in an accredited certification body’s product directory. A label or listing that mentions a standard is not, by itself, proof that the exact model is certified for every PFAS.
  • Read the performance details. Check which compounds the certification or performance data cover and whether the product’s claim matches your needs. EPA cautions that current certifications may not establish reduction to EPA drinking-water standard levels.
  • Account for upkeep. Compare the format—such as pitcher, faucet-mounted, or under-sink reverse osmosis—along with installation needs, replacement intervals, and maintenance. Replace parts as instructed by the manufacturer; performance depends on proper upkeep.

Ask your utility or state agency whether a filter is suitable for the specific result and whether another temporary or long-term water option is recommended.

Why there is no single safe-water number for every situation

The guidance described here does not provide a universal pregnancy-specific risk estimate or a cutoff that can be applied to an unspecified test report and location. The water source, PFAS measured, concentration, exposure history, and state or local recommendations matter. Ask the relevant authority to interpret your result rather than applying a number from a different compound, water system, or jurisdiction.
