Electric toothbrushes have a modest average advantage over manual brushes in reducing measured plaque and gingivitis, according to a 2014 Cochrane review. That does not mean every electric model cleans better, or that the measured difference is proven to prevent more cavities or other long-term dental problems. The American Dental Association (ADA) says either type can work effectively when used properly.

What the evidence says about cleaning

The main broad comparison is a Cochrane review by Yaacob and colleagues, published in 2014. It included 56 studies published from 1964 to 2011, with 5,068 participants randomized; 51 trials involving 4,624 participants contributed data to the meta-analysis. Most participants were adults. The review’s evidence search was current through January 23, 2014, so these findings describe that evidence base rather than a newly conducted comparison.

Across studies, powered brushes reduced plaque and gingivitis scores by the following averages compared with manual brushes:

These percentages are average reductions in measured indices—not the share of people cured, nor a direct estimate of cavities prevented. The review rated the evidence for plaque and gingivitis as moderate quality, found substantial variation between trials, and said the clinical importance of the score differences for long-term dental health remained unclear. It also reported few side-effect data; reported effects were localized and temporary.

Which powered-brush designs were studied?

More of the review’s evidence concerned rotation-oscillation brushes, which showed statistically significant reductions in plaque and gingivitis at both measured time points. That finding should not be extended automatically to every powered-brush motion, current design, or individual model.

One study is not the whole comparison

The ADA’s summary of one four-week study describes participants who began as manual-brush users with gingivitis. The oscillating-rotating groups had greater reductions in plaque and gingivitis than the manual group. In that study, 20% of people in the manual group transitioned from gingivitis to healthy gums, compared with 46.7% in sensitive mode and 60% in daily-clean mode. Those results illustrate one study and do not replace the broader evidence; the ADA notes that its article does not necessarily indicate ADA endorsement.

Does that mean an electric toothbrush is better for you?

Not necessarily. The review compares group averages, and the difference’s long-term health importance is uncertain. The ADA’s practical guidance is that either manual or powered toothbrushes can be used effectively. A brush you can use comfortably, consistently, and across all tooth surfaces may be a better fit than choosing by category alone.

  • Consider ease of use: A powered brush may be easier for some people with limited dexterity, disabilities, or braces. Comfort and control matter.
  • Consider technique and consistency: Choose the type that helps you brush thoroughly for the full recommended time without pressing too hard.
  • Consider upkeep: Powered brushes can cost more. Account for compatible replacement heads, charging or batteries, and whether replacement heads are easy to obtain. Current prices vary and are not established here.
  • Consider convenience features: A built-in timer may help you brush for two minutes, but it is a task aid, not evidence that a model produces better clinical results.
  • Check product evidence: The ADA Seal of Acceptance means a product’s safety and efficacy evidence has been reviewed under the program’s requirements. Check the ADA’s current listings before relying on a specific product’s status.

How to brush effectively with either type

The ADA recommends brushing twice a day for two minutes with fluoride toothpaste. Its toothbrush guidance calls for soft bristles, gentle pressure, and attention to the inner, outer, and chewing surfaces. For a manual brush, place bristles at about a 45-degree angle to the gumline and move gently in short strokes. With a powered brush, follow the manufacturer’s instructions rather than using manual-brushing strokes by default.

Replace a toothbrush or powered-brush head every three to four months, or sooner if the bristles are visibly matted or frayed. Individual needs can differ; ask a dentist for advice if you have an oral-health condition or other special circumstances.

What this comparison cannot establish

The Cochrane review’s search ended in January 2014. The sources cited here do not establish whether a newer comprehensive systematic review has replaced or materially changed its pooled comparison. The figures above should therefore be read with their evidence date in mind, not as proof about every brush currently sold.
