Then use the provider’s verified recovery process and involve the people authorized to manage the incident. Recovery depends on the specific wallet, its signer setup and any recovery mechanism configured in advance; there is no universal way to restore a lost key or reverse an on-chain transaction.
What should you do first?
- Identify the exact wallet and account. Record the provider, chain, wallet address, account type, signer set and approval threshold. List which devices, credentials, key shards and recovery materials remain available. Establish whether this is a custodial account, a provider-managed non-custodial wallet, a self-custodied smart-contract account or another arrangement.
- Determine whether this is lost access or a suspected compromise. If a device, key or account may have been stolen or exposed, treat the situation as a security incident under your institution’s process. Do not sign an unverified transaction or move assets to an address supplied through an unverified channel.
- Contact the provider through a verified support or incident channel. Keep a record of affected addresses, timestamps, relevant approvals and recovery steps already attempted. For Coinbase Token Manager, the support guidance asks organizations for the organization name, inaccessible admin wallet address, account type and steps tried. Do not send a seed phrase, private key or recovery phrase to support.
- Bring the right internal teams into the response. Include the people responsible for security, treasury, legal, finance and compliance so they can authorize actions, assess operational and client impact, and determine whether notifications are required in the relevant jurisdiction.
- Follow the documented recovery path for that product and configuration. Do not assume another provider can recover a key or that support can reverse a blockchain transaction. Coinbase says its support cannot recover a private key or seed phrase or override on-chain ownership without a valid signature.
Recovery steps vary by provider, product and setup. Provider procedures can change, so verify the current instructions with the provider before acting. The examples below describe specific products; they are not general rules for all institutional wallets.
How do Coinbase Prime Onchain Wallet recovery paths differ?
Coinbase describes Prime Onchain Wallet as non-custodial. Its documented recovery paths depend on which signers still have access to their devices or key shards and whether the portfolio’s recovery passphrase is available.
One signer loses a device or app
If another signer retains a key shard, an existing signer can reprovision the affected user’s access using the documented process.

All signers lose access to their devices
The documented route uses the portfolio’s 12-word recovery passphrase, an administrator to initiate recovery, a signer to complete it and portfolio consensus approval. Coinbase says the passphrase encrypts the recovery backup; it is not the wallet’s private key. The flow requires an administrator to authenticate with a YubiKey. Confirm the hardware requirements and compatible model with Coinbase for your organization before obtaining a device.
The recovery passphrase is lost, but a signer still has a key shard
Coinbase documents a process to replace the recovery backup and invalidate the previous passphrase. Follow the provider’s instructions for the actual portfolio rather than attempting an improvised backup change.

All signer shards and the recovery passphrase are unavailable
Coinbase says there is no recovery option for this situation, and access may be permanently lost. Do not assume support can create a replacement signature or reconstruct the missing materials.
What if the inaccessible admin wallet controls a smart-contract account?
For Coinbase Token Manager self-custodied smart-contract accounts, the owner or admin wallet controls account-level actions, including configuration changes, escrow withdrawals and transferring ownership. Coinbase says it cannot recover the key, sign on the organization’s behalf or replace the on-chain owner without a valid signature from the current owner. If the key is permanently inaccessible and there is no configured recovery route, available options may be very limited.

The impact on stakeholder tokens can depend on the account type and network support. Some vested and unlocked tokens may remain claimable, while unvested tokens or future vesting may no longer be administratively adjustable. Have legal and finance assess the specific account and plan how to communicate with affected stakeholders.
Does wallet recovery mean the assets can be recovered?
No. Restoring a key or signer restores the ability to authorize transactions. A separate recovery architecture may instead move assets to a pre-approved destination without restoring the lost key, and it only helps if it was configured and approved in advance.

For example, Circuit Security describes an Automatic Asset Extraction product that uses pre-signed transactions to sweep assets to a backup vault when keys are lost, systems fail or a wallet is compromised. This is a vendor-described feature, not a general emergency remedy. Ripple describes an open-source recovery CLI for its wallet product and says that path does not depend on Ripple or a single third party; confirm its current implementation and your institution’s setup before relying on it.
How should an institution prepare for wallet recovery?
- Spread administrative responsibility. Avoid making one person the sole route to wallet administration. Coinbase recommends multisig or an institutional custody solution for admin access. For Prime Onchain Wallet, Coinbase recommends considering at least three signers if possible; that is product guidance, not a universal institutional standard.
- Maintain a wallet and recovery inventory. Document wallet addresses, chains, account types, signers, roles, approval thresholds, recovery materials and escalation contacts. Store sensitive recovery material under your approved security policy, with access separated from day-to-day signing where appropriate.
- Set policies for people and keys. Define how key backups, signer recovery, employee role changes, offboarding, ownership transfers and signer rotations are handled.
- Check access periodically. Coinbase recommends regular access checks. Confirm that more than one authorized person can complete the required signing path, without disclosing secret material as part of the check.
- Rehearse approved procedures. Record recovery exercises and their results. A drill can reveal unclear responsibilities or unavailable dependencies before an incident; it does not prove that a provider or recovery service will perform as expected.
- Review dependencies. Assess provider availability, quorum requirements, independent backups, audit trails and whether recovery can work if the primary custody provider or platform is unavailable.
What should you compare when evaluating custody and recovery options?

What if the institution cannot restore access?
Preserve the incident record and continue working through the provider’s verified channel, but do not treat support contact as a substitute for a valid signing or recovery mechanism. If the account is governed by a smart-contract admin key, have legal and finance assess which account actions remain possible and the consequences for stakeholders. Any notification duties depend on the relevant jurisdiction and circumstances; the provider guidance described here does not establish a universal legal deadline or reporting rule.
