A controlled study found that intravenous THC increased paranoia, anxiety and worry, negative thoughts about the self, and unusual perceptual experiences in a selected group of participants. The study most directly matching this description was led by researchers at Oxford and published in 2014—not a newly published study.
What the brain study found
In a randomized, placebo-controlled experiment, Freeman and colleagues studied 121 people who already reported paranoid ideation. Participants received intravenous THC, a placebo, or THC after a cognitive-awareness condition. Researchers assessed their responses using a real social situation, an immersive virtual-reality task, self-report measures, and interviews.
Compared with placebo, THC significantly increased paranoia, negative affect—including anxiety, worry, depression, and negative thoughts about oneself—and anomalous experiences. It also reduced working-memory capacity. In the researchers’ analysis, negative affect and anomalous experiences helped account for the rise in paranoia; changes in working memory did not. The awareness condition had little impact. The study was published in Schizophrenia Bulletin in 2015.
How anxiety and altered perceptions may feed paranoia
The results suggest that THC-related paranoia may involve more than a single feeling. Anxiety and worry, negative self-directed thoughts, and unsettling perceptual experiences rose alongside paranoia in the experiment. The researchers’ mediation analysis found that negative affect and anomalous experiences accounted for the paranoia increase, while working-memory changes did not.

University of Oxford’s 2014 summary quoted study lead Professor Daniel Freeman describing a possible pathway: “Paranoia is likely to occur when we are worried, think negatively about ourselves, and experience unsettling changes in our perceptions.” That is his interpretation of the study’s findings, not a universal rule for diagnosing or explaining every person’s experience. Oxford’s summary also quotes Freeman saying the study showed cannabis can cause short-term paranoia in some people.
What the study’s numbers do—and do not—mean
Oxford reported that paranoid thoughts occurred in 50% of participants who received THC and 30% of those who received placebo. Those percentages describe the groups in this particular experiment. They are not estimates of how many cannabis users generally experience paranoia.

The researchers deliberately recruited a selected sample: participants were 21–50, had used cannabis at least once, and already reported paranoid ideation. Oxford says screening excluded people with a history of mental illness and certain relevant health conditions. The participants therefore do not represent all cannabis users.
Why intravenous THC limits what you can infer
The study used injected THC, rather than smoked or edible cannabis, to reduce differences in participants’ blood THC exposure. Oxford described the administered dose as equivalent to a strong joint and said its effects lasted about 90 minutes. Those details apply to the experiment; they do not establish how long anxiety or paranoia lasts after cannabis use by another route, at another dose, or in a different person.

The experiment supports a conclusion about an acute effect under controlled conditions: THC can increase paranoia and related negative feelings in some people. It does not show that every cannabis product or dose will cause anxiety, predict any individual’s reaction, or explain the long-term effects of cannabis use.

The U.S. Centers for Disease Control and Prevention says cannabis can cause disorientation and sometimes unpleasant feelings of anxiety and paranoia. The CDC also describes associations between cannabis use and psychosis and other mental-health outcomes; it reports a stronger association with schizophrenia for earlier and more frequent use. These longer-term associations are distinct from the acute effects tested in the Oxford-led experiment, and they do not mean that one episode of anxiety or paranoia establishes that someone has a mental illness.
