Start by verifying the notice through contact information you find independently, then match your next steps to the information exposed. A breach notice does not by itself mean your identity has been stolen. Check relevant accounts and medical records for unfamiliar activity, and dispute any errors directly with the provider and insurer.
Read and verify the breach notice
Identify the organization, the incident date or period, the types of information involved, and any steps it recommends. Note whether it offers monitoring and, if so, what it covers and for how long. HHS says that, to the extent possible, an individual notice should explain what happened and what information was involved, suggest protective steps, describe the organization’s response, and provide contact information (HHS breach-reporting guidance).
- Verify contact details using the organization’s official website or a phone number on a statement you already have. Do not trust an unsolicited caller just because they know details about your health.
- Keep the notice, reference number, and related correspondence. Record the dates and details of calls or messages.
Choose protections based on what was exposed
Different information calls for different checks. Use the notice to focus your effort rather than assuming every health breach includes financial credentials or requires a paid service.

| Social Security number | Review your credit reports and consider a free credit freeze or fraud alert. A freeze can make it harder for someone to open new credit in your name; you may need to lift it when applying for credit. Use the official channels of each nationwide credit bureau. | Passwords or login details | Change the affected password and any password you reused on other services. If you cannot access the account, contact the service through a verified channel. |
| Payment or insurance information | Check account activity, insurer communications, and explanation-of-benefits statements. Contact the relevant financial institution or insurer’s fraud department about suspicious activity. |
| Clinical information without financial credentials | Prioritize checking patient records, claims, and bills for care or charges you do not recognize; credit monitoring alone will not identify incorrect medical records. |
If the organization offers monitoring, review the notice for the service’s coverage and duration. The FTC advises using an offered service in the circumstances addressed by its guide, but monitoring is not a guarantee against misuse and is not a substitute for checking your records.

Check for medical identity theft
Exposure is not proof that someone has used your information. Review available patient portals for your providers, labs, clinics, hospitals, and health plan. If records are not online, ask the provider how to obtain them. Look for unfamiliar treatment, procedures, diagnoses, prescriptions, or charges.
Unexpected medical bills and an insurer’s denial because its records show exhausted benefits or a condition you do not have can be warning signs, according to the FTC’s medical identity theft recovery guidance.

- TEST THE CARD: Test the card is working at your local supermarket. At the self-service checkout machines, combine the card and a contactless card on the payment reader. Payment with the contactless card will be blocked and an error message should occur on the reader.
If you find an incorrect record or bill
- Write to the provider and identify the specific entry or charge and why you believe it is wrong. Ask the provider to correct the record and, where relevant, share the correction with other providers.
- Notify your health insurer’s fraud department and ask how to dispute the claim or charge.
- Keep copies of the notice, records, bills, and letters. Maintain a dated log of conversations, including who you spoke with and what they said.
Respond to confirmed identity theft and credit-file problems
If you find evidence of identity theft—not just a breach notification—use IdentityTheft.gov’s recovery steps to report it and build a recovery plan. If fraudulent accounts or credit entries appear, document them and dispute them with the relevant business and credit bureau.
The FTC describes a one-year fraud alert as an option that can be initiated through one nationwide credit bureau; a credit freeze requires contacting each of the three nationwide bureaus. Both are free. A fraud alert asks creditors to take steps to verify your identity, while a freeze restricts access to your credit report. Choose based on the information exposed and how you expect to use credit.

Contact HHS OCR if you suspect a privacy-rule violation
If you believe a HIPAA-covered entity or business associate violated HIPAA’s Privacy, Security, or Breach Notification Rules, you may file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights (OCR) through its Health Information Privacy & Security complaint portal. Health-related data is not automatically covered by HIPAA: covered entities include health plans, clearinghouses, and certain health care providers, so the organization’s role matters.
Substance-use-disorder records covered by 42 CFR Part 2 have a distinct confidentiality framework. See HHS’s Part 2 guidance for information about that framework and its complaint provisions.

Know which breach-reporting deadlines apply to the organization
HHS’s breach-reporting page, reviewed February 13, 2026, says covered entities must report qualifying breaches of unsecured protected health information to the Secretary without unreasonable delay. For an incident affecting 500 or more people, the deadline is no later than 60 calendar days after discovery. For an incident affecting fewer than 500 people, reporting is due within 60 days after the end of the calendar year in which it was discovered. These are organizational reporting deadlines, not deadlines for you to take protective action.
