There is no good basis for saying caregiving generally shortens caregivers’ lives. The often-cited 1999 study found higher adjusted mortality only among older spousal caregivers who reported mental or emotional strain—not among caregivers as a whole. A later national analysis found no caregiver subgroup with higher mortality, including those reporting strain. Both studies were observational, so neither establishes that caregiving itself caused or prevented deaths.
What did the “63% caregiver mortality” study actually find?
The 1999 Caregiver Health Effects Study followed adults aged 66–96 in four U.S. communities. It included 392 caregivers and 427 noncaregivers, with data collected from 1993 to 1998 and average follow-up of about 4.5 years. The researchers distinguished caregivers who reported strain from those who did not, rather than treating all caregiving as one exposure. Clinician.com’s clinical abstract describes the study and its result.
Among caregivers who said providing care caused mental or emotional strain, adjusted relative risk of mortality was 1.63 compared with noncaregiving controls (95% confidence interval 1.00–2.65). That is the source of the “63%” headline: it is a relative-risk estimate for this specific subgroup and comparison, not a 63-percentage-point increase in an individual’s chance of dying. The confidence interval begins at 1.00, the null value, and extends to 2.65, so the estimate is uncertain.
The result does not establish that caregiving caused deaths. The study was observational, and its participants were older adults caring for disabled spouses in four communities. It cannot automatically be generalized to younger caregivers, people caring for relatives other than spouses, or all family caregivers.

Did the study find increased mortality among all caregivers?
No. In the later synthesis of the original findings, caregivers who did not report strain had an adjusted relative risk of 1.08; the review does not report an elevated adjusted rate for the other comparison group. The original result is therefore better understood as a finding about strain among a narrowly defined group, not proof that caregiving status alone raises mortality. Magellan Longevity’s 2026 review summarizes these comparisons.
How does later evidence change the picture?
A 2013 analysis of the national REGARDS cohort used propensity matching to compare 3,503 family caregivers with matched noncaregivers. During the reported follow-up, 264 caregivers died (7.5%), compared with 315 matched noncaregivers (9.0%); the hazard ratio was 0.823 (95% CI 0.699–0.969). The analysis did not identify a subgroup with higher mortality, including caregivers who reported strain, according to the 2026 review.

Those results do not show that caregiving protects health. Propensity matching balances groups on measured characteristics, but it cannot eliminate differences that were not measured. People who take on caregiving may differ from those who do not in ways that affect both caregiving and health. The two studies also examined different populations and used different designs:

A 2015 reappraisal of population-based evidence argued that public accounts often overstate a general mortality risk. That review-level interpretation adds context, but it does not settle why findings differ across populations and methods.

Why focus on strain rather than a blanket mortality claim?
Caregiver strain is not interchangeable with caregiving itself. The 1999 finding singled out caregivers reporting mental or emotional strain, while later evidence did not find higher mortality in strained caregivers. These results leave uncertainty about mortality, but they do not make strain irrelevant to health or well-being.
A 2026 review summarizes support-intervention findings that address outcomes other than caregiver mortality. A meta-analysis of 84 articles reported standardized effect sizes of 0.58 for depression, 0.55 for stress, and 0.18 for physical health; these are effect-size measures, not percentages. In the REACH II trial, involving 642 caregivers, clinical depression prevalence at six months was 12.6% in the intervention group versus 22.7% in the comparison group. In a separate NYU caregiver intervention trial, the nursing-home placement rate was reduced by 28.3%, with a model-predicted median delay of 557 days. That is a trial result, not a promised delay for any family.

Evidence for respite care is less conclusive. A 2014 Cochrane review of four trials with 753 participants, as summarized in the 2026 review, detected no significant effect on caregiver outcomes and rated the evidence very low quality. A failure to detect an effect in sparse, low-quality evidence is not proof that respite has no benefit.

What can caregivers take from these studies?
- Do not interpret the 63% figure as applying to every caregiver: it concerns strained older spousal caregivers in one observational study.
- Mortality findings do not measure the full effects of caregiving on stress, depression, physical health, or quality of life.
- Support interventions have shown benefits for some outcomes in specific trials, but results do not guarantee that a particular program will work for every person.
- If caregiving strain is affecting your health or ability to provide care, discuss practical support and health concerns with a clinician or local caregiver-support service.
