For most people considering total knee replacement for osteoarthritis, current evidence does not show that robotic assistance reliably leads to less pain, better function, fewer complications, or fewer revisions than conventional surgery. Robotic systems can improve the accuracy of some alignment measurements, but that advantage has not consistently translated into outcomes patients notice. The right choice depends on your surgeon’s recommendation and experience, the system’s requirements, your circumstances, cost, and local availability.

What is different about the two procedures?

In conventional total knee replacement, the surgeon prepares the bone using cutting guides and standard instruments. In robotic-assisted surgery, planning and navigation or robotic tools help guide parts of the operation. The exact process varies by system: some platforms use a preoperative CT scan to create a three-dimensional plan.

In either procedure, the surgeon performs the operation and places the prosthetic components. The robot does not operate on its own or replace the surgeon’s judgment.

What does the evidence say about results?

Alignment measurements

Robotic assistance improves some measures of component or limb alignment in randomized-trial reviews. A 2023 systematic review of 12 randomized controlled trials involving 2,200 patients found fewer mechanical alignment outliers with robotic assistance (risk ratio 0.43, 95% confidence interval 0.27–0.67) and a smaller average deviation from neutral mechanical alignment (mean difference −0.94 degrees, 95% confidence interval −1.1 to −0.73). These are radiographic measures; the review cautioned that the alignment difference may not be clinically meaningful. 2023 systematic review

A separate 2024 meta-analysis of seven randomized trials and 1,942 knees also found better postoperative anatomical and mechanical alignment with robotic assistance, while reporting similar clinical and functional outcomes and complication rates. Its authors called for more trials using current systems. 2024 meta-analysis

Pain, function, and movement

More precise alignment has not consistently meant a noticeable improvement in recovery. In the 2023 review, patient-reported outcomes showed little to no difference (WOMAC mean difference −0.35, 95% confidence interval −0.78 to 0.07), as did range of motion (mean difference −0.73 degrees, 95% confidence interval −7.5 to 6.0). The American Academy of Orthopaedic Surgeons (AAOS) guideline similarly states: “Evidence suggests no significant difference in function, outcomes, or complications in the short term between robotic assisted and conventional total knee arthroplasty.” AAOS knee osteoarthritis guideline

Complications and revision

The available evidence does not establish that robotic assistance generally reduces complications or the chance of needing revision surgery. The 2023 review detected no difference in revision rates or major adverse effects, but noted that evidence about these relatively uncommon outcomes was insufficient to draw firm conclusions.

A registry analysis reported by AAOS Now followed 9,220 patients aged 65 or older who had cementless primary total knee replacement for osteoarthritis. At two years, all-cause revision was 1.16% in robotic-assisted cases and 1.3% in conventional cases; the difference was not statistically significant. Those figures apply to that specific older, cementless cohort and follow-up period, not to every patient or system. AAOS Now registry report

A 2025 review of implant survivorship, based on literature searched through December 19, 2024, concluded that conventional surgery was non-inferior to robotic-assisted surgery for short- and long-term implant survival, complications, and postoperative pain. It reported subtle improvements in some functional measures with robotics. This synthesis does not prove that every platform or patient will have the same result. 2025 survivorship review

What could affect your choice?

Your surgeon’s experience and plan

Ask which robotic system the surgeon would use, how often they use it, and why they believe it suits your anatomy and surgical plan. The practical comparison is not simply “robot versus no robot”: it also involves the surgeon’s familiarity with the specific approach and what they recommend for you.

Imaging, pins, and time

Requirements vary by platform and facility. Some systems use preoperative imaging, which may involve radiation; navigation pins may carry a risk of fracture where they are placed. Planning or operating time may also differ. Ask which steps apply to your proposed procedure and what their significance is in your case. Cost and availability

Robotic assistance may add cost, and it is not available at every facility. Ask whether it changes your out-of-pocket expense or the facility charge. In its guideline announcement, AAOS quoted Dr. Srivastava saying, “Therefore, if the current evidence demonstrates no difference in outcomes, the extra cost might not be necessary.” That comment is conditional on the evidence and your circumstances; it is not a conclusion that robotic assistance is never worth considering. Your anatomy, health, and goals

Your anatomy, overall health, priorities, and the surgeon’s recommended plan all matter. Ask how these factors affect the choice rather than assuming that one method is best for everyone. Questions to ask at your appointment

  • Would you use robotic assistance for my surgery? What are the pros and cons in my case?
  • Which system would you use, and how often do you perform knee replacements with it?
  • Does that system require a CT scan or other imaging, or use navigation pins? What are the associated risks for me?
  • What is the expected out-of-pocket or facility-cost difference?
  • How do my anatomy, health, and recovery goals affect your recommendation?
  • How many knee replacements do you and this facility perform, and will I need a walker, crutches, or cane afterward?

How to make the decision

  1. Start with the outcome you care about. Ask how each option is expected to affect pain, function, recovery, complications, and revision—not just alignment measurements.
  2. Understand what the proposed system entails. Confirm its imaging, pin, planning, and navigation requirements with your surgeon.
  3. Compare experience and practical costs. Discuss the surgeon’s familiarity with the technique, any added charge, and whether the option is available locally.
  4. Choose a plan tailored to you. Weigh those details with the surgeon’s assessment of your anatomy, health, and goals.
