A repeated-looking charge is not automatically a duplicate: separate clinicians or facilities may bill for distinct services. The goal is to verify what each line represents and whether the amount you owe matches the documentation and insurance processing.
Gather the documents for the visit
Collect the detailed bill from each provider or facility, every EOB for the encounter, relevant medical records, and receipts or proof of payments already made. If you were uninsured or chose not to use insurance, include the written good faith estimate, if you received one.
Keep statements organized by provider and date of service. A hospital stay or procedure can generate separate bills from the facility and clinicians, and an insurer may send separate EOBs for different providers, facilities, or treatment dates. One statement may not represent the entire episode. CMS explains what to gather in its medical-bill error checklist and bill-reading guide.
Read the bill before judging the total
Check the billing date and date of service, the listed items or services, the charges, adjustments, payments already made, and the remaining balance. An adjustment reduces the gross charge, often because the provider accepted a lower amount. Patient payments may include amounts paid at the visit, such as a copayment.

Compare the final amount shown as due rather than comparing only the initial gross charge. If insurance was used, the provider bill and EOB should agree on your responsibility after the claim and adjustments are processed. Do not assume that an unexplained difference is an error until you ask whether an adjustment or claim reprocessing is pending.
Compare each charge with the care and records
Review each line using the same questions. A mismatch is a reason to ask for clarification; a billing-code label alone does not establish that a service was unnecessary or incorrectly billed.

- Provider and date: Who billed the service, and on what date did it occur?
- Description and code: What service or supply does the line describe, and does the code description make sense for the care you received?
- Documentation: Can you find the service in the relevant records? CMS advises that patients should not be billed for services that are not documented in their records. Ask the provider to identify the documentation or explain the discrepancy. Ask about any records-access fee before requesting copies.
- Possible duplicate: Does the same service appear more than once on this statement or on another provider’s bill? Compare the provider, date, service description, and code before treating similar entries as duplicates. Multiple providers can legitimately bill for different work during one encounter.
- Amount due: Do adjustments and prior payments appear to have been applied, and does the remaining patient responsibility match the EOB?
CMS’s checklist for medical-bill errors recommends checking for duplicate services and comparing billing codes with their descriptions. A code that appears inconsistent with what happened warrants a question, but does not by itself prove intentional wrongdoing.
Reconcile the provider bill with your EOB
An EOB is the insurer’s explanation of how it processed a claim; it is not itself a bill. For each provider and date of service, compare the services and patient-responsibility amounts on the EOB with the provider’s bill. If they differ, ask the insurer whether the claim was reprocessed or an adjustment is pending, and ask the provider to explain the balance. Save all EOBs because they may cover different clinicians, facilities, or dates.

Ask the billing office about specific lines
Contact the billing office with the provider name, date, line description or code, and amount in question. Ask for an explanation or a corrected bill. Useful questions include:

- What service or supply does this line represent, and where is it documented?
- Does this code describe the service performed?
- Is this charge separate from another provider’s service, or might the same service have been billed twice?
- How were adjustments, insurance payments, and payments I already made applied?
- Can you provide the explanation in writing and tell me whether collection activity can be paused while the account is reviewed?
- If the amount is correct but unaffordable, what financial assistance, bill reduction, or payment-plan options are available?
Write down the date of the call, the name of the person you spoke with, and any reference number. CMS recommends documenting provider conversations and asking about payment plans, reductions, and financial assistance. Do not assume an ordinary billing question automatically pauses collection activity; ask the provider about its policy and the rules that apply to your situation. The No Surprises Act took effect January 1, 2022, and protects people with most types of health insurance from certain unexpected out-of-network bills. The protections cover defined circumstances, including emergency services, certain non-emergency services connected with care at an in-network hospital, hospital outpatient department, or ambulatory surgical center, and air ambulance services. Whether a protection applies depends on the care and coverage circumstances; an unmet deductible alone does not establish a violation. For an issue you believe is covered, CMS directs insured patients to the health plan’s appeal process or a complaint, depending on the issue. See CMS’s overview of medical bill rights.

For uninsured or self-pay patients: good faith estimates
Providers generally must give a good faith estimate when care is scheduled at least 3 business days in advance or when a patient who does not use insurance asks for one. If the final charge is substantially higher, a federal patient-provider dispute may be available, but only if the eligibility conditions are met. CMS lists conditions including that you did not have or did not use insurance and told the provider in advance, the care occurred on or after January 1, 2022, you have the written estimate, the initial bill is dated within the last 120 calendar days, and a provider or facility charged at least $400 more than its estimate.
Starting the federal dispute process has a $25 non-refundable administrative fee; CMS says it is deducted from the amount owed if the decision favors the patient. While a qualifying dispute is underway, the provider or facility cannot move the bill into collections, threaten to do so, collect existing late fees, or take action against you because you are disputing it through this process. These protections apply to the PPDR process, not to every ordinary billing inquiry or appeal. Confirm current eligibility and process details with CMS’s dispute-a-bill guidance.
Get help if the issue is not resolved
If the provider or insurer does not resolve the question, CMS points consumers to state Consumer Assistance Programs and patient advocates who can help with billing issues. You can also ask the provider about financial assistance, reductions, or a payment plan if you owe the balance but cannot afford it. CMS describes these options in its medical-bill error checklist and provider conversation guide.
Hospital standard charges and nearby facility prices may provide context for comparing prices, but they do not determine whether a particular charge was billed correctly or what an insurer will ultimately allow. Use the bill, records, EOB, and any applicable estimate to resolve the specific account in question.
