There are two different questions: whether a vagus nerve stimulation (VNS) device is delivering its programmed stimulation, and whether VNS is improving the condition it is meant to treat. You usually cannot confirm either from a single sensation. Track the symptoms that matter for your condition and ask your treating team to check the device and its settings.
Feeling stimulation does not tell you whether VNS is helping
Many people do not feel stimulation from an implanted VNS device. Some notice a hoarse voice when stimulation activates, but that sensation does not prove that treatment is working clinically. Not feeling anything does not, by itself, prove that the device has stopped working either. The Epilepsy Foundation explains what people may notice and how VNS is used for epilepsy.
Do not use a magnet or another self-test to judge effectiveness. The magnet may provide extra stimulation in certain situations; it is not a diagnostic test. Follow the instructions for your specific device and your care team’s advice.
How to judge whether treatment is helping
Measure changes in the condition VNS is intended to treat, rather than looking for a particular sensation from the device. Record observations over time and review them with the clinician managing your care.

If VNS is being used for epilepsy
Keep a record of seizure frequency, severity, and recovery, along with any relevant changes in your usual pattern. The Epilepsy Foundation describes possible reductions in seizure frequency and severity and shorter recovery as treatment outcomes; regular visits let the care team monitor and adjust the device.
The American Academy of Neurology’s 2013 guideline summarizes older Class III studies, not a prediction for an individual: more than 50% seizure reduction was reported in 55% of 470 children with partial or generalized epilepsy across 13 studies, and in 55% of 113 patients with Lennox-Gastaut syndrome across four studies. Two Class III studies also found an approximately 7% increase in the proportion achieving at least 50% seizure-frequency reduction between one and five years after implantation. If VNS is being used for depression
Review symptoms and everyday functioning over time with your clinician. The American Association of Neurological Surgeons’ patient page reports that studies leading to FDA approval for treatment-resistant depression did not show benefit in the first two or three months; it says that after one year, 20–30% reported significant improvement. The page’s publication date is not established here, and those study results are not an individual forecast: some patients improved later, while others did not improve or worsened. Response may take time and can build over months or years, but there is no fixed timeline or guaranteed response for an individual. Keep taking prescribed medication and following your care plan unless your clinician changes it; for epilepsy, VNS is described as an add-on treatment.

How the care team checks device function
Device function is a technical question for the treating team, separate from whether your symptoms improve. The team can monitor the implanted generator and review or adjust its programmed settings. Mayo Clinic notes that activation takes place during a medical visit a few weeks after implantation and that follow-up helps confirm the generator is working and has not moved. Mayo Clinic describes VNS activation and follow-up.
There is no universal at-home function check established for every VNS model. If you are concerned about operation or possible movement, contact the team that manages your device rather than trying to test, reprogram, or repair it yourself.

Make sure you are comparing the same kind of VNS
VNS can mean an implanted system or transcutaneous VNS (tVNS), which delivers stimulation through the skin using a portable device. These are different approaches. Evidence and approved uses depend on the device and the condition being treated, so do not assume findings for an implanted system apply to tVNS, or vice versa. NIMH says findings for depression are mixed and research into tVNS for depression, PTSD, and other mental disorders continues. NIMH summarizes brain-stimulation therapies, including VNS.

When to contact your treating team
- Your seizures, depression symptoms, or day-to-day functioning have not improved, or have worsened.
- You are concerned that the implanted generator has moved or that the device is not operating as expected.
- You need to know whether the system is delivering its programmed stimulation or whether a setting should be reviewed.
Bring your symptom or seizure record to the appointment if you keep one. A paper journal can help organize observations for follow-up, but it cannot verify device function or prove treatment efficacy. Do not change device settings, stop medication, or delay contacting your clinician in order to run a self-test.
