If a vertical mouse makes your wrist or forearm uncomfortable, first check its fit, your grip, and where it sits. Keep it close to the keyboard, set pointer speed so you can guide it with a light touch and a straight wrist, and break up long stretches of mouse use. A vertical shape is not comfortable for everyone, so consider another device if adjustments do not help.
1. Check that the mouse fits and your grip is relaxed
A vertical mouse is designed to hold the hand in a more handshake-like position, but its shape alone does not guarantee comfortable use. The device should fit your hand, support its natural curve, and let your hand, wrist, and forearm stay in a comfortable, straight line. The Canadian Centre for Occupational Health and Safety (CCOHS) notes that there is no one-size-fits-all mouse: people may need to try different shapes and types.
Notice whether you are squeezing the mouse, stretching your fingers to reach buttons, or pressing buttons that take noticeable force. A device that is too large or too small, or awkwardly placed buttons, can make relaxed control harder. Try holding it with a light grip and moving it without bending your wrist sharply.

2. Bring the mouse within easy reach
Place the mouse on the same working surface as the keyboard and close enough that you do not need to reach forward or outward. Keep your upper arm near your body and your wrist straight while using the device. A mouse positioned too far away can require sustained arm elevation; a hard or sharp desk edge can also put pressure on the forearm or wrist.
Check whether the mouse has enough room to move without forcing you to pull your elbow away from your side. If the desk edge presses into your arm, change your position or the contact point rather than leaning against the edge.

3. Adjust pointer speed and use a light touch
Set pointer sensitivity so you can move the cursor across the screen while keeping your wrist comfortable and relatively straight. If sensitivity is too low, you may need large wrist movements; if it is too high, fine control can feel difficult and encourage tense or forceful corrections. Change the setting in small increments and try ordinary tasks to see whether control feels easier.

4. Break up repetitive mouse use
Even a well-positioned mouse can mean long stretches of repetitive or static work. Where practical, use keyboard shortcuts, alternate tasks, or vary how you work so mouse movements are not uninterrupted for extended periods. Take short pauses that let you move and recover.
OSHA gives five minutes per hour as an example of frequent short breaks for computer users. That is general work-process guidance, not an individualized medical prescription or a guarantee that symptoms will improve. See OSHA’s Work Process and Recognition guidance.
5. Try another pointing device if the vertical mouse still feels wrong
There is no single mouse design that works best for everyone. If you have adjusted fit, placement, and pointer control but remain uncomfortable, try a different shape or pointing device and judge it by how it feels during your actual work. Consider these practical differences:

- Another vertical mouse: Different sizes, contours, and button layouts may fit your hand differently while retaining the handshake-like design.
- Conventional mouse: A familiar shape may be easier to control for some people. Check that it still allows a relaxed grip and comfortable wrist position.
- Trackball: A trackball can keep the arm and hand more stationary because you move the ball rather than moving the whole device. OSHA notes it may be less fatiguing for certain tasks, but that does not establish it as the best option for every user.
When comparing devices, consider whether they fit your hand and operating side, allow a comfortable wrist position, fit your workspace, require movement from your fingers, wrist, or arm, and have buttons you can operate without force. OSHA lists a mouse pad with a wrist or palm rest as one possible aid for neutral posture and reducing contact stress. CCOHS, however, says evidence on wrist rests is inconclusive and warns that incorrect use can add pressure around the carpal tunnel. A rest is not a treatment for discomfort, and pressing your wrist into a hard edge can create additional pressure.
If you try a support, pay attention to where your hand rests while you are actually moving the mouse. Avoid bearing your weight on the wrist while controlling the device. The guidance differs in emphasis: compare OSHA’s pointer/mouse recommendations with CCOHS’s mouse selection guidance.

When discomfort persists
Wrist or forearm discomfort can have more than one possible contributor; these workstation adjustments cannot identify its cause. OSHA lists symptoms that may occur with computer-related musculoskeletal disorders, including numbness or burning in the hand, reduced grip strength, swelling or stiffness, wrist or forearm pain, tingling, cramping, and weakness. Its guidance encourages recognizing and addressing symptoms early, but does not provide a diagnosis or a symptom-specific medical triage rule.
If symptoms persist or concern you, do not assume that changing mice rules out a medical problem. The ergonomic sources cited here offer general workstation guidance, not an individualized diagnosis or treatment plan. OSHA’s symptom information is in Work Process and Recognition.
