A vertical mouse changes forearm posture, but it is not automatically more comfortable or better for gaming.
What changes when you use a vertical mouse?
A vertical mouse holds the hand in a more upright position than a conventional flat mouse, which can reduce forearm pronation—the inward rotation used to place the palm downward. That posture change does not by itself prove better comfort, lower injury risk, or improved aim.
In a 2015 comparison involving 12 participants and standardized pointing tasks, the tested fully vertical mouse reduced forearm pronation but produced the poorest pointing performance among the designs tested. The study was not about gaming, so it cannot show whether a vertical mouse helps or hurts performance in a particular game. The authors concluded that “Increasing mouse height and angling the mouse topcase can improve wrist posture without negatively affecting performance.” That conclusion applies to the tested designs and tasks, not to every vertical mouse or gaming situation. Set up your workstation for a neutral wrist
Before changing devices, arrange your desk so you can use the mouse without reaching and keep the wrist straight rather than bent sideways or up and down. NIOSH recommends a neutral, straight wrist posture when using an external mouse. It also advises avoiding unpadded, sharp work-surface edges pressing against the forearms or wrists. OSHA similarly advises: “Position the pointer/mouse to allow you to maintain a straight, neutral wrist posture.” Its guidance lists vertical mice as one available input-device type; it does not recommend a single design for everyone. OSHA’s pointer and mouse guidance emphasizes choosing a device that suits the user’s ability, task requirements, physical limitations, and preference.
Choose a mouse for your hand and your games
Compare candidate mice against the work and play you actually do. A vertical ergonomic mouse is worth considering if its shape fits your hand and lets you maintain a comfortable, neutral posture. Check whether the buttons, wheel, and grip are practical for the games you play, rather than assuming an ergonomic design will deliver better control.

- Hand fit: Can you hold the mouse without stretching to reach buttons or gripping tightly?
- Posture: Does the shape let your wrist stay straight, and does it reduce the forearm rotation you are trying to avoid?
- Task control: Can you point accurately in everyday use and perform the movements your games require?
- Desk placement: Can you keep the mouse close enough to avoid reaching?
- Preference: Does the device feel comfortable to you during actual use?
The evidence does not establish that a vertical mouse prevents injury, treats pain, or improves gaming performance. If you have persistent pain or other symptoms, a mouse change alone is not a diagnosis or treatment.
Make the switch as a personal trial
Moving from a standard mouse to a vertical one can feel difficult at first, but the available guidance does not establish a universal number of days or hours for adapting. Avoid treating a fixed adjustment schedule as a proven rule. Instead, assess the mouse in ordinary pointing tasks and in the games you play, noting whether your wrist posture, comfort, and control are acceptable to you. When a fully vertical mouse is not the right fit
If a fully upright design makes accurate pointing or game control harder, that trade-off matters; reducing pronation does not guarantee that the device suits your task. The 2015 study suggests that increased mouse height and an angled top case may offer a posture change without the performance cost seen with its tested fully vertical mouse, but it does not establish which product is right for an individual. Use fit, posture, task control, and your own comfort to decide.
