Short answer: A vertical mouse changes how you hold your forearm and wrist, but the available evidence does not show that it reliably relieves wrist pain or lowers pressure in the carpal tunnel. It may suit some people better than a conventional mouse; treat it as a comfort option to try, not a proven treatment. For gaming, small lab studies suggest a possible speed-and-precision trade-off, but they did not test actual games or esports.
What a vertical mouse changes—and what that does not prove
A conventional mouse usually turns the forearm so the palm faces down. A vertical mouse places the hand in a more handshake-like grip, reducing forearm pronation—the inward rotation that turns the palm toward the desk. That is a change in posture, not proof of pain relief or lower risk of a wrist condition.
In a 2013 laboratory comparison, de Oliveira Sato and colleagues measured 28° of pronation with their vertical mouse versus 42° with a standard mouse. They also measured extensor carpi activity of 13% versus 16%, respectively. The vertical mouse took 4.2 seconds to reach the target versus 3.4 seconds for the standard mouse in that task. These are results for the devices, participants, and lab task in that study—not predictions for every hand or mouse. Read the study abstract.
Does a vertical mouse relieve wrist pain or help carpal tunnel syndrome?
It has not been established that a vertical mouse reliably relieves wrist pain. In a 2015 study of 21 patients with carpal tunnel syndrome, a vertical mouse significantly reduced ulnar deviation, a sideways bend at the wrist. However, the ergonomic devices tested did not reduce measured carpal tunnel pressure. The authors said their findings did not justify a strong recommendation for or against the devices and concluded, “Selection of ergonomic devices remains dependent on personal preference.” See the study abstract and conclusion.

That distinction matters: a more neutral-looking posture is a biomechanical finding, not a clinical outcome. The study does not establish that a vertical mouse treats carpal tunnel syndrome, prevents it, or will ease an individual’s symptoms. If you have persistent pain, numbness, tingling, weakness, or symptoms that interfere with daily activities, discuss them with a qualified health professional rather than relying on a mouse change as treatment.
Are vertical mice good for gaming?
There is no direct evidence in these studies that compares vertical mice with gaming mice for latency, aim, or match outcomes. The available tests were laboratory pointing tasks, not games. They raise a reasonable caution for players who need rapid, precise target acquisition: some vertical models may feel slower or less accurate, particularly before you have adapted to their grip.

In a 2015 evaluation involving 12 subjects, the fully vertical mouse reduced pronation but had the poorest pointing performance among the designs tested. The authors also reported that increasing mouse height and angling the topcase could improve wrist posture without a measured performance penalty in their task. That finding suggests an angled design may be worth trying if a fully upright grip disrupts control, but it does not establish how either design performs in a particular game. Read the evaluation.
A vertical mouse may be usable for casual gaming, but judge it in the games you actually play. Compare target acquisition and accuracy with your familiar mouse after giving yourself time to adapt. For competitive play, do not assume that an ergonomic shape will match a mouse designed around your preferred grip and aiming technique.

How to decide whether one is worth trying
- Grip angle: A fully vertical grip changes forearm posture more, but the 2015 pointing study found the vertical model performed worst on its task. An angled mouse is another option if you want a less pronounced change.
- Fit and handedness: Check the device’s size and shape against your hand and dominant hand. A mouse that feels awkward or forces a tight grip is unlikely to be a comfortable choice.
- Tracking and control: Look for adjustable sensitivity if you need to tune cursor movement. For games, test settings and accuracy in your own titles rather than assuming a manufacturer’s DPI range predicts performance.
- Weight, buttons, and connections: Consider whether the weight, button layout, wired or wireless options, and receiver or Bluetooth support suit your setup. The studies cited here do not establish gaming latency for vertical mice.
- Real-use trial: If possible, try the mouse for ordinary work and your regular games before committing. Pay attention to comfort, control, and whether adaptation improves the experience; personal preference was central to the clinical study’s conclusion.
Example: Logitech MX Vertical
Logitech’s official listing describes the MX Vertical as a 57° mouse and specifies adjustable tracking from 400 to 4000 DPI, four customizable buttons, a weight of 135 g, and wired USB-C, receiver, and Bluetooth connection options. Logitech describes its fit as suitable for medium-to-large hands.
