MRI contrast is usually a gadolinium-based medicine injected into a vein when it can help a radiologist see particular organs, blood vessels, or tissues more clearly. It is not needed for every MRI. The decision depends on the diagnostic question and on factors such as kidney function, prior reactions, and possible pregnancy.
How does MRI contrast work?
MRI scanners use magnetic fields and radio waves to produce signals mainly from hydrogen protons in the body’s fat and water. A gadolinium-based contrast agent (GBCA) changes the local magnetic environment, which changes the contrast in the resulting images. It can make some structures or tissue differences easier to assess; it does not simply make every abnormality light up or diagnose a condition on its own. GBCAs are given through an intravenous line. The radiologist chooses the imaging protocol, including whether to use contrast, to help answer the specific clinical question.
Why do I need contrast for my MRI?
Contrast is considered when the additional visualization may provide information needed for the examination. The FDA gives cancer, infections, and bleeding as examples of problems evaluated using GBCAs. Whether contrast is useful depends on the body area and the question being investigated; many MRI scans are performed without it. The FDA’s patient information on GBCAs describes their use to improve visualization of internal organs, blood vessels, and tissues.

A radiologist weighs the expected diagnostic benefit against relevant patient factors and alternatives, including whether a noncontrast MRI or another imaging approach can answer the question. Contrast is one part of an examination, not a stand-alone diagnosis.
Is MRI contrast the same as CT contrast?
No. MRI GBCAs and CT iodinated contrast are different classes of agents used with different imaging methods. GBCAs do not contain iodine, and the two types of contrast are not interchangeable. An iodine-contrast allergy does not mean you have had a reaction to a GBCA, but tell the care team about it. A previous reaction to gadolinium contrast is directly relevant to planning an MRI.

Allergic reactions to gadolinium agents are rare and are generally less common than reactions to iodine-based contrast, according to RadiologyInfo’s contrast-material safety guidance. Tell the imaging staff about any prior contrast reaction and follow their instructions if symptoms occur during or after the scan.
Is gadolinium safe if I have kidney disease?
Most administered gadolinium is eliminated through the kidneys. Severe kidney dysfunction or acute kidney injury can delay its removal and is associated with nephrogenic systemic fibrosis (NSF), a rare but serious condition. This does not mean every person with kidney disease must avoid contrast: the radiologist considers the clinical need, kidney status, available alternatives, and the specific agent. Current professional guidance is available in the ACR Manual on Contrast Media, which includes 2025 chapter updates on NSF.

Tell the imaging facility if you have kidney disease, have had a kidney transplant, or may have an acute kidney problem. Depending on your circumstances and local protocol, the facility may ask for a kidney-function blood test before the scan. Screening and agent selection are individualized rather than based on a blanket rule for all gadolinium agents.
Can I get MRI contrast while pregnant?
Routine GBCA use is generally avoided during pregnancy because the potential risk to the fetus is uncertain. In an unusual case where contrast could provide critical information that cannot be obtained otherwise, the radiologist and care team may weigh that benefit against the uncertainty. The ACR Manual on Contrast Media and its 2022 pregnancy screening statement discuss this approach.

Tell the care team if you are pregnant or might be pregnant before contrast is administered. Imaging facilities use screening procedures to identify possible pregnancy.

Most of a GBCA dose is cleared through the kidneys, but trace amounts can remain in the body long-term. The FDA says retention has not been directly linked to adverse health effects in people with normal kidney function. That finding does not mean retention is nonexistent or that every question about it has been settled. The FDA advises clinicians to consider retention characteristics, particularly for patients likely to receive repeated doses over their lifetime.

There is no basis in the cited FDA guidance for using consumer “detox” products to remove retained gadolinium. If you expect to have repeated contrast-enhanced MRIs, ask your radiologist how contrast affects the specific examination and whether an alternative can provide the needed information.
What should I tell the imaging team before the scan?
Before the examination, tell the imaging facility about:
- A previous reaction to gadolinium-based MRI contrast.
- Kidney disease, a kidney transplant, or a recent kidney-function concern.
- Pregnancy or the possibility that you may be pregnant.
- Any iodine-based contrast allergy, so the team has a complete history even though iodine contrast and GBCAs are different agents.
The facility may ask screening questions or request a kidney-function test depending on your history and its protocol.
