Misophonia support can include individualized psychological care—most often approaches from the cognitive behavioral therapy (CBT) family—and practical accommodations for managing trigger sounds. No single treatment or universal protocol has been established. Earplugs, headphones, sound changes, and quiet spaces may help with coping, but they are not cures or stand-alone treatments.
What can treatment for misophonia involve?
A clinician may tailor psychological care to the sounds that trigger distress and the way those reactions affect daily life. Treatment goals can include reducing emotional and behavioral reactivity, addressing anxiety or anger, managing distress, and limiting the disruption caused by triggers.
CBT-family approaches
CBT-family approaches are the most extensively described treatment group in the sources available. Depending on the individual formulation, they may include psychoeducation, setting goals, identifying triggers, emotion-regulation and distress-tolerance skills, attention strategies, and behavioral experiments. Some approaches may use carefully designed exposure-related procedures, but exposure is not automatically suitable or sufficient for everyone.

Oxford Health NHS describes a tailored CBT-informed service that may use the S-Five questionnaire to understand and monitor experiences, test ideas through experiments, identify emotions, address relevant memories, and help people communicate support needs and work out accommodations. Other psychological approaches
A 2024 pilot study evaluated the Unified Protocol, a transdiagnostic CBT approach, in two small groups: eight patients in an initial feedback study and 10 in a revised-treatment study. The authors reported that the approach was acceptable and described preliminary improvement, particularly in emotional and behavioral responses. A PubMed-indexed 2025 trial abstract describes 60 adults assigned either to individualized acceptance and commitment therapy (ACT) plus audiologically informed behavioral strategies or to education plus progressive relaxation training. The abstract describes the study design and follow-up schedule but does not provide results, so it cannot show which approach was more effective. 2025 ACT trial abstract

Medication and related concerns
The sources do not establish medication as a specific treatment for misophonia. The American Psychiatric Association notes that medication or talk therapy may help address underlying mental-health issues. That is different from showing that a medicine treats misophonia itself. - 2023 systematic review: The review covered 33 treatment studies: one randomized controlled trial, one open-label trial, and 31 case studies. It described CBT incorporating different components as the most frequently used and effective approach among the included reports, while also noting limited rigor, small samples, few comparisons, and limited replication. This is a summary of a weak and developing evidence base, not proof of universal benefit. 2023 systematic review
- 2026 scoping review: The review identified 30 publications with pre-to-post treatment measurement. It found that a range of CBT-family procedures appeared deliverable and potentially helpful, but highlighted uncontrolled designs, small samples, varied procedures, and inconsistent measures. Study counts and trial sample sizes describe the research, not how common misophonia is in the population.
What can help with trigger sounds day to day?
Practical sound-management options may make difficult situations more manageable. The American Psychiatric Association suggests learning one’s triggers, finding or creating spaces where they are less likely, trying earplugs or noise-cancelling headphones, and using pleasant or distracting sounds. The International OCD Foundation describes protective equipment and quiet zones as possible accommodations while adaptive strategies are developed, rather than stand-alone treatment. It cautions against relying on extensive avoidance—such as eating in isolation—as a treatment strategy, because it may affect adaptive skills and social life. How can family, school, or work support participation?
Useful support starts with identifying the situations that are most difficult and asking what limited accommodations would help the person take part. The aim is to communicate needs and make activities more manageable without unnecessarily narrowing access to ordinary routines, relationships, or responsibilities. Oxford Health NHS includes communication tools and problem-solving around accommodations among possible parts of a CBT-informed approach.

How do you choose a provider or approach?
- Ask what the evidence supports. A controlled trial, a small pilot, case reports, and clinical guidance provide different levels of certainty; the current sources do not establish a definitive treatment.
- Look for an individualized plan. Ask how the clinician will address your triggers, distress, emotions, behavior, and the impact on daily life.
- Clarify the provider’s approach. Ask about relevant experience, goals, and how progress will be monitored. Duke’s resource page lists clinician and researcher resources, but it does not establish local availability. Duke Misophonia and Sound Sensitivity Resource page
- Consider whether coping strategies support function. Accommodations may help, but a plan built around avoidance alone can carry costs for participation and social life.
The World Health Organization’s ICD-11 Clinical Descriptions and Diagnostic Requirements, published March 8, 2024, is a diagnostic resource for mental, behavioural, and neurodevelopmental disorders. It is not a misophonia-specific treatment guideline.
