An agent can take actions through software tools, but people and organizations set its access, decide which actions need approval, retain the ability to intervene, and must be able to reconstruct what happened. The right controls depend on the task and its risk; the EU AI Act’s human-oversight duties apply to high-risk AI systems within the Act’s scope, not to every AI agent or editorial tool.
How do AI agents and human editors differ?
A human editor can exercise judgment, approve or change work, and act within an organization’s authority structure. An AI agent can perform assigned work through connected tools, but its practical authority comes from the permissions and operating conditions people configure. It may be able to draft, retrieve, edit, or publish only if its tools and access allow those actions.
So the useful comparison is not whether an agent can “replace” an editor in the abstract. It is whether the agent’s access is appropriate, when a person must review an action, who can stop it, and whether the resulting record shows what happened and who authorized it.

This is an operational comparison, not a claim that all agents or editors work alike. A human reviewer who cannot understand the output or stop an action is not providing meaningful oversight merely by being present in the workflow.

What permissions should an AI agent have?
Grant access according to the specific task rather than giving an agent broad access by default. Decide which sources it may read, which systems it may write to, and which actions it may take without another person’s authorization. Separate low-impact work from actions that could have significant consequences, and place the latter behind explicit approval where appropriate.
- Limit data access: allow only the information needed for the assigned task.
- Limit tools and actions: distinguish reading or drafting from changing records, communicating externally, or publishing.
- Set approval gates: require a named human decision before actions whose consequences warrant review.
- Keep boundaries effective: configure constraints the agent cannot simply override, where the system and use case call for them.
- Record the workflow: retain the initial request, tool calls, approval or denial, tool results, and relevant policy decisions.
OpenAI’s 8 May 2026 account of how it runs Codex describes technical boundaries for coding agents, human approvals, and telemetry. It is an example of one organization’s operating approach—not a universally binding standard and not a prescribed implementation of the EU AI Act.

When should an AI agent need human approval?
Use approval gates when the action’s potential impact, uncertainty, or difficulty of reversal makes unattended execution inappropriate. There is no single approval threshold established for every agent. Set it in light of the task, the agent’s autonomy, the context, and the consequences of an error.
Actions that may warrant an approval gate
- Publishing or sending material to an external audience.
- Changing important records or settings rather than preparing a proposed change.
- Taking an action with significant consequences that is difficult to reverse.
- Proceeding when the agent encounters unexpected results, conflicting information, or a task outside its assigned boundaries.
These are practical examples for designing a workflow, not a list of actions that a particular law requires every agent to gate. A useful approval request gives the reviewer enough context to make a decision: what the agent intends to do, what information or tool result it relied on, and what may happen if the action proceeds. Approval should be an actual decision point, not a routine click that conceals risk.

What does meaningful human oversight require?
For high-risk AI systems within its scope, Article 14 of Regulation (EU) 2024/1689 says systems must be designed so they can be effectively overseen by natural persons while in use. It states that “Human oversight shall aim to prevent or minimise the risks to health, safety or fundamental rights that may emerge when a high-risk AI system is used in accordance with its intended purpose or under conditions of reasonably foreseeable misuse”.
The oversight measures are to be proportionate to risk, autonomy, and context. In practical terms, the people assigned to oversight need to be able to monitor operation, understand relevant capabilities and limitations, interpret outputs, recognize potential over-reliance, and address anomalies or unexpected performance. Recital 73 adds context on operator competence and authority, built-in constraints where appropriate, and guidance to help people decide when and how to intervene or stop a system that is not performing as intended.

These provisions do not mean that every AI agent or editorial tool is automatically a high-risk system. First establish whether the system and its use fall within the Act’s scope and classification. The European Commission’s AI Act Service Desk displays Article 14 based on the consolidated text as of 27 July 2026; the corresponding EUR-Lex consolidated record is also dated 27 July 2026.

An agent’s action does not erase the responsibilities of the people and organizations that build, provide, deploy, configure, or use it. The applicable duties depend on the legal framework, the actor’s role, the system’s classification, and its use. Under the European Commission’s high-level AI Act overview, deployers have human-oversight and monitoring responsibilities after a system is placed on the market, while providers have post-market monitoring duties. Those roles are distinct; the relevant provision and role must be checked for the specific system and use.

Inside an organization, define responsibility rather than assuming it. Specify who owns the agent’s configuration and access, who is authorized to approve consequential actions, who monitors operation, and who can pause or stop it. NIST’s AI Risk Management Framework Core calls for defined and differentiated roles and responsibilities for human-AI configurations and oversight. It also treats documentation of risks and impacts as support for transparency, human review, and accountability. The framework is governance guidance, not a replacement for applicable law.
What should an accountability record contain?
A useful record should let an authorized reviewer reconstruct the path from request to outcome. OpenAI’s Codex operating example describes preserving information about the original request, tool activity, approval decisions, tool results, and relevant policy decisions. That is a practical model for an audit trail, not a universal legal checklist.
- The request or instruction that started the task.
- The tools the agent used and the actions it attempted.
- Any approval, denial, or change made by a human, including who made the decision.
- The results returned by tools and the final action or output.
- Relevant policy decisions or boundary checks that affected the task.
Keep the record useful for review: it should distinguish an agent’s activity from a person’s approval and make the sequence of events understandable. Access to logs alone is not enough if the person responsible for review cannot interpret them or act on what they show.
How should teams set up review and intervention?
- Define the task and risk: describe what the agent may accomplish, the likely consequences of errors, and which actions should not be automatic.
- Assign permissions: limit the agent’s data and tools to what the task needs; distinguish proposed work from actions that change systems or reach other people.
- Name the reviewer: assign review to someone with sufficient task knowledge, the information needed to judge the output, and authority to reject or pause the action.
- Set clear intervention points: explain what conditions require a pause, override, or stop, including anomalies and unexpected performance.
- Preserve evidence: record the request, tool activity, human decision, results, and relevant policy decisions so the sequence can be reconstructed.
- Check the workflow in use: review whether controls and records work as intended, and update them when the task, system, or context changes.
For systems that may fall under the EU AI Act, determine the system’s scope, classification, and the organization’s role before treating any general workflow as a compliance answer. The European Commission AI Act Service Desk says transparency rules applying from 2 August 2026 cover certain systems intended to interact with natural persons or generate content; that timing concerns those transparency rules and should not be confused with Article 14’s high-risk human-oversight requirements.

What is changing for AI agents?
NIST announced its AI Agent Standards Initiative on 17 February 2026. It is an initiative to advance agent-related standards work, not a finished agent standard. Teams should not treat the announcement as a completed set of technical requirements or as a substitute for existing governance and legal obligations.
