Treat every AI-generated vulnerability finding as an unverified claim. Before you patch, escalate, or dismiss it, confirm that it applies to your actual code, version and configuration. Then reproduce the behavior in an authorized environment. Finally, check that what you observed supports the impact the report states. Write down the evidence and your decision so another reviewer can follow your reasoning.
This applies to output from an AI coding assistant, an LLM-based scanner, or a workflow where a model summarizes other tools’ results. OWASP’s guidance on LLM overreliance warns that model output can be wrong while sounding authoritative. It recommends human oversight and continuous validation.
The validation workflow, step by step
1. Capture the claim exactly as received
Save the finding before you edit or reinterpret it. Record:
- the affected component and version
- the file or endpoint location
- the claimed weakness
- the stated preconditions and attack path
- the claimed impact and the reasoning behind the severity
- any exploit or fix the model suggested
Then separate what the model explained from what a scanner, test, source file or runtime observation actually showed. Only the second category is evidence.

2. Check provenance and scope
- Right project, right version. Models can describe code that resembles yours but isn’t yours, or a library version you don’t run.
- Present, reachable, enabled. Confirm the flagged code ships in your build, can be reached from an input an attacker controls, and isn’t switched off in your configuration.
- Authorization. Make sure you are permitted to test the system. OWASP’s vulnerability disclosure guidance tells researchers to understand applicable law, and a model’s suggestion is not permission.
3. Reproduce safely
Use the least invasive method that can test the claim, in a controlled environment such as a local build, staging copy or container. Don’t run an AI-suggested exploit against a live system just because the model proposed it. Save the command or test case, the input or request, the observed output, and the environment details (version, configuration, OS, dependencies). This is the same standard OWASP’s disclosure cheat sheet sets for human reporters: provide sufficient details for the vulnerability to be verified and reproduced.
4. Trace the full security condition
“This pattern looks risky” is not a demonstrated vulnerability. Follow the claimed input through the code and controls, and ask:

- Do the stated preconditions actually exist?
- Do authentication, authorization, input validation, sandboxing or framework defaults block the path?
- Does the behavior you observed lead to a security consequence, or only to an error or odd behavior?
5. Judge validity before severity
Decide first whether the issue is real. Only then rate impact and urgency in the environment where it occurs. A confident explanation or a “critical” label from a model does not establish exploitability, and a real issue may still be low priority if it sits behind strong controls or affects nothing sensitive.
6. Reach and record a triage outcome
Pick one of three outcomes:
- Confirm and route to the owner with the evidence attached.
- Request more evidence when the claim is plausible but you can’t verify it yet. Mark it unconfirmed rather than asserting certainty either way.
- Document as a false positive or accepted exception, with the basis for the decision.
OWASP’s Vulnerability Management Guide (July 2020 version) recommends obtaining evidence from the source, documenting false-positive submissions, and setting a timeframe to reevaluate them.

7. Retest after the fix
Once a remediation lands, rerun the original reproduction and record whether the condition is gone. OWASP’s disclosure guidance likewise describes confirming resolution and retesting where needed. Don’t accept an AI-generated patch as proof of a fix without this step.
What good evidence looks like
A reviewer should be able to identify the affected component and verify the issue without trusting the author. Look for a reachable code path, a controlled test result, relevant configuration, or observed behavior tied to the claimed impact. Record the version and conditions under which you gathered it.

Failure to reproduce is ambiguous. The report may be wrong, or a precondition, environment detail or input may be missing. Document which explanation your evidence supports rather than treating a failed attempt as proof of either.

OWASP’s Vulnerability Disclosure Cheat Sheet notes that “Reports may include a large number of junk or false positives.” That makes dismissal a routine task, but it needs the same discipline as confirmation. A defensible dismissal records:

- why the claim was rejected, citing specific evidence
- the scope and version reviewed
- who decided and when
- what new evidence would reopen it
Don’t use “false positive” as shorthand for “not investigated.”
Comparing several findings
When a model hands you a long list, these axes help you order the review. They are a practical framework derived from the verification guidance above, not a published scoring standard.

| Demonstrated impact | Which assets are affected, and was the impact shown or only asserted? |
| Report completeness | Does it give enough detail to verify without guessing? |
| Residual uncertainty and effort | How much work remains to settle it either way? |
What AI changes, and what it doesn’t
Models are useful for generating hypotheses, summarizing scanner output and pointing you toward code worth reading. They don’t change the evidence standard. OWASP defines overreliance as trusting erroneous LLM output without oversight or confirmation, so apply the same bar you would to a report from an anonymous stranger. If AI assists your triage, keep the underlying source evidence and the human decision visible in the record.
No source reviewed here establishes a universal AI-specific threshold for accepting a finding, or a single accuracy or false-positive rate that holds across models, scanners and codebases. Be skeptical of any blanket figure you see quoted. NIST SP 800-216 (May 2023) recommends formal processes to accept, assess, manage and communicate vulnerability disclosures. It applies to systems under federal control and is process guidance, not a test for AI output, but it is a useful model for organizing intake.
