A Guardian Science Weekly episode published on 6 October 2026 discusses the condition and a major treatment development: the FDA’s approval of oveporexton for adults with narcolepsy type 1. The approval is significant, but it does not make the drug a cure or extend its US indication to every form of narcolepsy.
What is narcolepsy?
Narcolepsy is a chronic sleep disorder. Its symptoms differ from person to person and may include excessive daytime sleepiness, disrupted nighttime sleep, sleep paralysis, and hallucinations as someone falls asleep or wakes. Some people also experience cataplexy, a sudden loss of muscle tone while awake. The condition is not simply a tendency to nod off at inconvenient moments. The NIH overview of narcolepsy describes its symptoms, evaluation, and treatment.
What is cataplexy?
Cataplexy is a sudden loss of muscle tone while a person is awake. It is one of the symptoms associated with narcolepsy type 1, but symptoms vary and not everyone with narcolepsy experiences it.
Why orexin matters
Orexin is involved in regulating wakefulness, sleep, and muscle tone. The FDA describes narcolepsy type 1 as resulting from the loss of orexin-producing brain cells. That explanation applies to type 1; it should not be generalized to every case of narcolepsy. The FDA’s 5 August 2026 announcement estimates that narcolepsy type 1 affects 1 in 2,000 people in the United States.

How is narcolepsy diagnosed?
Diagnosis involves clinical evaluation, not a sleep journal alone. A clinician may review symptoms, medical and family history, and a physical examination. They may ask the person to keep a record of sleep times and symptoms for one to two weeks; this helps document patterns but is not itself a diagnostic test.
Evaluation can also involve an overnight polysomnogram and a multiple sleep latency test, typically performed in a sleep disorders clinic. A clinician determines which assessments are appropriate. The NIH explains the evaluation process.

What is the new narcolepsy drug approved in 2026?
On 5 August 2026, the US Food and Drug Administration approved Orzeyful (oveporexton) tablets for adults with narcolepsy type 1. Oveporexton is an orexin receptor 2 agonist, intended to act on orexin signaling. The FDA called it the first medicine approved for narcolepsy type 1 as a complete disorder and the first to directly target the loss of orexin signaling. Those descriptions do not mean it cures narcolepsy. The FDA prescribing information identifies the medicine as an OX2R agonist.
What the FDA reported about the studies
The FDA’s approval announcement summarizes two randomized, double-blind, placebo-controlled studies lasting 12 weeks and enrolling 273 adults. It reports improved ability to stay awake, reduced patient-reported daytime sleepiness and fewer cataplexy episodes, along with improvement in other symptoms. These findings are not a head-to-head comparison with other medicines, and the announcement does not establish that oveporexton is superior to them.

Who the approval covers
The approval described here is in the United States and applies to adults with narcolepsy type 1. The FDA says safety and effectiveness have not been established for people under 18. The approval does not cover narcolepsy type 2 or children. Availability and regulatory status may differ outside the United States; consult local regulators and a clinician for current information.
Does oveporexton treat narcolepsy type 2?
The FDA approval described here is for adults with narcolepsy type 1, not type 2. Do not assume the approval applies to type 2; treatment decisions depend on an individual’s diagnosis and current local labeling.

What are the side effects of Orzeyful?
The FDA’s announcement lists insomnia, increased urinary frequency, urinary urgency, and increased saliva production among common side effects. The prescribing information also warns against taking oveporexton with strong CYP3A inhibitors. Oveporexton adds an option that targets orexin signaling for a defined group, but it does not make other care irrelevant. The NIH describes narcolepsy treatment as often combining medicines and lifestyle measures, tailored to symptoms. Existing symptom-directed approaches may still be part of care; the FDA announcement does not compare oveporexton directly with those options or establish a treatment ranking. The NIH overview provides general treatment context.
