Record the project, precise location, inspection date and time, work activity, visible condition, and any authorized measurements or follow-up. A photo alone rarely establishes when a condition began, what caused it, or whether it violates a specification.
What to record at the site
Build a record that lets someone who was not present understand what was inspected, where the condition appeared, and what the image shows. The Federal Highway Administration (FHWA) recommends recording observations on the project or immediately after inspection, and using photographs and sketches to depict details and document the inspection. Its guidance is general; the project contract and applicable agency procedures control the required form and process. FHWA inspection guidance
- Project and location: Record the project or contract identifier and the location reference used on the job, such as road segment, lane, direction, station, or another accepted reference.
- Date and time: Record when the inspection occurred. Use local time and include the time zone if records may be compared across zones. A camera timestamp can help, but the inspection note should identify the date and time too.
- Inspector and role: Identify who made the observation and their role.
- Work phase and activity: Note what work was underway or had been completed, and the relevant inspection phase.
- Conditions affecting the view: Record relevant weather, roadway, lighting, or other conditions when they help explain what the photo can and cannot show.
- Image references: Give each photo a consistent identifier and cite that identifier in the note.
- Visible condition: Describe what is actually visible without assuming a cause, responsible party, formation date, or specification violation.
- Measurements or tests: If appropriate and within your role, record the method, units, and who performed the measurement or test.
- Follow-up: Record who was notified, any response or action requested or agreed, and when the condition will be checked again, following project procedure.
FHWA’s inspection-reporting guidance emphasizes identifying the project and location and describing the work and inspection phase. Its road safety audit field-note worksheet also includes fields such as date, time, roadway and roadside conditions, pavement markings, and road-user conditions. How to take useful, dated site photos
- Confirm the visit details. Before or during the inspection, note the project, location reference, date and time, inspector, work activity, and conditions relevant to the observation.
- Take a context view. Photograph enough of the roadway or work area to help a later reader orient the condition. Include recognizable project context where practical.
- Take one or more detail views. Move closer to show the visible condition clearly. A context view and a detail view answer different questions; neither should be expected to do both jobs.
- Show scale when it matters. If a dimension is relevant and you are authorized to measure it, record the measurement and method in the notes. Include a scale in the image only when it will clarify the record. - Write the observation promptly. Describe the visible condition in plain, specific terms while on site or immediately afterward. FHWA recommends timely recording; delay makes it harder to reconstruct location and context accurately. FHWA guidance on inspection observations and photographs
- Link the images to the note. Use the project’s required records system and naming rules. If no rule is specified, use a consistent identifier and include it in the note so the photo can be found later. The cited guidance supports factual documentation but does not set a universal filename or metadata standard.
- Track notification and reinspection. Note the person informed, the response or next step, and the follow-up date or status, according to the project’s procedures. Describe observations without overstating what the photo proves
Separate observable facts from interpretations. For example: “At the eastbound outside lane near station [project station], a visible longitudinal crack extends approximately [measured dimension].” Replace bracketed text only with information actually observed or measured. If no measurement was made, do not imply a precise length.

A photograph can document the visible condition at the time it was taken. By itself, it does not establish the condition’s cause, when it first formed, who is responsible, or whether a contractual requirement was breached. If a technical or disputed issue needs verification, use measurements or tests that are appropriate to the inspector’s role and the project procedure, and identify their method and source in the record. Apply the requirements for the project and jurisdiction
FHWA’s Construction Program Management and Inspection Guide is dated August 2004. It provides general federal inspection guidance, not a substitute for current contract documents or state-agency procedures. FHWA Construction Technical Guidance

FP-24 includes daily quality-control and construction-operations reporting provisions that call for documenting work locations, inspection results, and observed deficiencies.
