What happened in the September 29 incident?
OpenAI’s incident record describes elevated errors across ChatGPT, Codex, and the API, including the Agents API. OpenAI’s root-cause write-up says some users encountered failed requests, timeouts, or difficulty signing in to ChatGPT and Codex. Some API requests failed, and some conversations or tasks could not start, resume, or complete successfully.
The incident page lists affected components across those products, including ChatGPT conversations, login, file uploads, Voice mode, GPTs, Search, and Codex in ChatGPT Desktop; Codex Web, Codex API, CLI, and VS Code extension; and API components such as Chat Completions, Responses, Agents, Fine-tuning, Embeddings, Images, Batch, Audio, Moderations, Realtime, Files, and Login. This is the status page’s component list, not proof that every listed feature—or every user—was affected.
OpenAI cautions that “Availability metrics are reported at an aggregate level across all tiers, models and error types. Individual customer availability may vary depending on their subscription tier as well as the specific model and API features in use.”

How OpenAI explained the cause
According to OpenAI’s write-up, a feature rollout changed how routine settings requests behaved. Those requests began triggering repeated background checks across internal services. The checks opened new connections to multiple services; when checks failed, additional attempts followed. This exhausted shared network connection capacity, making it difficult for otherwise healthy services to communicate reliably.
That explanation describes an internal service-side reliability issue. OpenAI’s account does not attribute the incident to a particular user device, home network, account, or third-party network vendor.

Incident timeline: mitigation was not the same as final resolution
OpenAI’s timeline distinguishes the quick mitigation of most customer-facing impact from the end of all residual degradation and from the later final status-page entry.
- About 11:00 a.m. PDT, September 29: OpenAI says most customer-facing impact was mitigated within approximately 30 minutes.
- About 4:07 p.m. PDT, September 29: OpenAI says the remaining degradation was resolved around this time.
- October 6, 2026, 08:26 AM: The incident record’s resolved update stated that the issue had been resolved and all affected services had recovered.
The October 6 timestamp is the date of the record’s final resolved update; it is not the time OpenAI says the September 29 residual degradation ended.

What OpenAI did to mitigate and prevent a recurrence
Actions during the incident
OpenAI says engineers disabled the feature rollout, routed traffic around unhealthy infrastructure, reduced repeated requests, and increased database cache capacity.
Completed remediation and continuing work
In its write-up, OpenAI says it disabled the triggering feature, removed the code that initiated repeated checks during settings requests, and deployed a fix for expensive database queries. It also says it is expanding network capacity and reviewing other queries to reduce bottlenecks during recovery. The write-up does not report a measured post-incident reliability outcome.

What to do if ChatGPT is still showing errors
If you are seeing errors now, first check OpenAI’s incident record and the live status page for a current service issue. The September 29 incident was attributed to OpenAI’s internal infrastructure; changing your router or device would not address that reported root cause.
For errors that persist when no relevant outage is reported, OpenAI’s troubleshooting guide recommends steps that may help diagnose other problems:

- Try a private or incognito browser window, or another browser.
- Temporarily disable browser extensions, VPNs, proxies, or security filters.
- Try a different network or device.
The guide covers messages such as “Something went wrong,” “A network error occurred,” and “There was an error generating a response.” Those examples are general troubleshooting cases; they do not establish that each message occurred in the September 29 incident.
