To get a new Verizon Fios TV set-top box, sign in to My Verizon and choose Manage home internet, or contact Verizon Customer Service. Verizon may require a plan or equipment change, and additional fees may apply. Confirm your eligibility, price, and return instructions before ordering; the process can differ for a new box, a defective unit, or an older model.
Request a new Fios TV box through Verizon
- Sign in to My Verizon and select Manage home internet, or contact Verizon Customer Service.
- Explain whether you need an additional set-top box, a replacement for a defective box, or an upgrade from an older model. Those situations may follow different account and return instructions.
- Before confirming the order, ask whether your plan or other equipment must change, what one-time or recurring charges apply, what accessories are included, and when any existing equipment must be returned.
Verizon’s Fios TV Home App FAQ describes the request route and warns that plan or equipment updates and additional fees may be required. It does not establish that every account or plan is eligible, or specify your account’s price.
If the box is defective or you own it
Contact Verizon before buying another device. Verizon’s Fios Equipment Return Policy says qualifying defective purchased equipment may be returned within one year of purchase or activation, whichever comes first. Depending on the policy and its limitations, Verizon may repair the equipment, replace it with new or refurbished equipment, or refund the purchase. This policy concerns purchased equipment; do not assume it applies in the same way to a rented or Verizon-owned box.
Keep the unit and its accessories until Verizon tells you what to send back and gives you the applicable deadline. Do not assume a retail or used set-top box will work with your Fios account: request account-managed equipment through Verizon.

Check whether an older box needs an upgrade
Look at the manufacturer and model on the equipment label, then ask Verizon about the available upgrade path. Verizon’s Fios set-top-box FAQ and equipment guide says Cisco set-top boxes stopped transmitting Fios TV video content on or after August 23, 2024, and Motorola set-top boxes on or after April 15, 2025. These cutoffs refer to those manufacturer families; an old-looking case alone does not identify the model or determine eligibility.
The same guide describes the Fios TV One Video Media Server (4100) as supporting up to 200 hours of HD recording and simultaneous recording of up to 12 programs, and the Fios TV One Mini as pairing with a Fios TV One VMS. Treat these as descriptions of those devices, not a promise that either is currently available for your account. Ask Verizon which options your plan supports.

Connect and activate the replacement
Verizon’s Fios TV activation instructions describe connecting the box to a coax outlet, connecting the box to the TV with HDMI, and plugging in power. For the described activation process, the Fios router should already be installed.
- Connect a coax cable from the VMS or set-top box to a coax outlet.
- Connect HDMI from the box to the TV.
- Plug in the power cord, then turn on the box and TV.
- Follow the on-screen steps or use Verizon’s online or My Fios app activation options.
The activation instructions establish that an HDMI connection is needed, but do not say whether your order includes an HDMI cable. Check the order confirmation or ask Verizon before buying one.

Return old Verizon equipment as instructed
Contact Verizon to start a Fios equipment return; do not assume that a replacement order automatically begins one. Verizon’s Fios Equipment Return FAQ says it will identify participating UPS Store locations near your address. At the store, the equipment’s serial numbers are scanned and the equipment is packed and shipped. Keep the receipt; Verizon says it emails confirmation after processing. Follow the deadline and instructions for your specific order. A 30-day return instruction in Verizon’s app FAQ applies to the described situation where a customer uses their own device instead of an existing additional set-top box, not automatically to every replacement.
Verizon currently lists these unreturned or damaged equipment amounts on that FAQ page. They are posted figures, not a determination of what you owe; check the page and your account for applicable terms.
