Include both: state the proposed action and give a concise reason in the request, then link to the specific trace or supporting record for detail. The reviewer should understand what decision is needed without having to reconstruct the case from a raw log.

What should an approval request contain?

Make the request understandable on its own, with the evidence one click away. The action and reason belong in the request; the link is for fuller context that would make the request unwieldy.

  • Action: Say exactly what you want approved.
  • Reason: Give a brief, decision-focused explanation of why the action is needed.
  • Context: Link to the relevant trace, record, or evidence when more detail matters.
  • Decision deadline: Add one if the workflow has a real deadline. It is useful in some processes, but the sources do not establish it as a universal requirement.

This balances concise approval-page fields with a record that identifies what was approved and makes fuller decision context available. Salesforce’s guidance is specific to configuring its Classic approval request page; it is not a general rule for every approval system (Salesforce Help).

Should you put details in the request or link to them?

Approach Useful when Main risk
Put supporting detail directly in the request The details are brief and necessary to understand the proposed action. Long explanations or logs can bury the decision-relevant facts.
Link to a fuller record The evidence is lengthy, includes multiple sources, or needs to be inspected separately. The link may not help if the reviewer cannot access it or cannot tell which part matters.
Use a concise reason plus a link The approver needs a clear request and access to supporting context. The summary still needs to identify the action; a link alone does not explain what is being approved.

AWS’s guidance for human review in agentic AI workflows says reviewers need enough context to decide without wading through raw logs. It recommends presenting key decision factors and making the full context available through the review interface (AWS Well-Architected Agentic AI Lens). That guidance is most directly applicable to AI workflows, but the distinction between a decision summary and supporting detail is useful for other approval processes too.

How do you make the trace link useful?

Link to a record that can be tied back to this request, rather than to a broad log or folder that leaves the reviewer searching. The linked material should let the reviewer find the relevant action and inspect the evidence behind it.

  • Check that the intended approver can open the link. AWS describes authenticated access through the review interface and, where reviewers lack access to that interface, temporary signed links as an option for its particular workflow.
  • Point to the specific record or relevant section when possible, and make its connection to the request clear.
  • Keep the decision-relevant factors in the request itself; do not rely on the reviewer to infer the ask from technical detail.

For an audit trail, the University of Mary Washington’s Internal Audit guidance emphasizes connecting the evidence to the activity under review, including identifying details and system context. An email that only says “approved” may not show what was approved; include the original request and relevant transaction details in the record (UMW Audit Documentation Request Guidance). This is institutional audit guidance, not a universal legal retention rule.

What should the approval record preserve?

Keep enough information to establish who requested the action, what was under review, who made the decision, what they decided, when they decided, and any rationale or relevant context. The cited audit and vendor materials describe these as traceability practices; they do not set one retention period that applies to every organization.

For example, an IT-access workflow may require a business justification as a field. ApproveThis documents that as a product workflow example, not a universal requirement for all approvals (ApproveThis IT access requests). Other vendor documentation likewise illustrates ways to retain approval comments and decision records, rather than establishing a standard every system must follow (AxonFlow human-in-the-loop workflows; Context audit story guidance).
