Agent memory can fail at four different points: a system may discard working context during compaction, preserve a fact too poorly for retrieval, keep stale records, or make earlier passages hard to address. Public issue reports describe examples of each, but they do not show how common the problems are or whether the cited bugs remain present in current versions.

The article behind these examples was written by a builder of HyperMarrow, a proposed local-first memory layer. Its central question is useful beyond that product: “The useful question is not whether your agent has memory. It is where that memory lives, and what happens to it when the session ends.”

What idle compaction can lose

A context window is temporary working state. When it fills, an agent runtime may compact the conversation by summarizing or otherwise reducing it. That can preserve the broad task while dropping a detail the agent needs later. The risk is especially visible in a long-running session when compaction happens while the user is away and no one can review the summary at the time.

The HyperMarrow builder’s article attributes this scenario to a report titled “Idle compaction silently discards working context in long-running sessions; no opt-out,” identified as anthropics/claude-code issue 98747. The exact tracker page and its current resolution were not independently retrieved, so this is an account of a reported failure, not confirmation of the issue’s present status.

The author’s design conclusion is: “A write decision has to happen before compaction, not after it.” In practical terms, a memory system should decide which decisions, constraints, and unresolved questions must survive before reducing the active context. Asking a summarizer to reconstruct a missing detail after the fact cannot recover information that was already discarded.

Four distinct failure surfaces

The reports point to different stages in an agent-memory lifecycle. Treating them all as one generic “memory bug” hides what needs to be checked.

The article identifies the OpenClaw report by the title “memory_search hybrid ranking drops the only chunk that contains the whole query.” It identifies the Claude Code report as “Subagents persist in memory after stopping without manual removal,” and describes issue 98768 as a request for “Paragraph anchors with cross-session references.” The exact tracker records and their current status were not retrieved; these titles should not be read as independently verified findings or implementation details.

Persistence is not the same as useful memory

A durable file or database solves only one part of continuity. A working system also needs to preserve information accurately, decide when to consult it again, handle stale or conflicting records, and let a user trace an important recollection to its source.

  • Write timing: Does the system preserve important facts and decisions before compression, or ask a summarizer to reconstruct them at the threshold?
  • Fidelity and provenance: Can a durable fact be traced to the original observation, and can the system distinguish that observation from a later reflection?
  • Post-compaction retrieval: Does the runtime actively consult stored memory after compaction, or is information merely left available for manual lookup?
  • Retention and concurrency: Can low-value records expire without removing pinned or referenced knowledge? Can simultaneous writers avoid overwriting one another?
  • Addressability and privacy: Can the user point to a precise passage across sessions, and where is the durable store located?

These are evaluation questions, not a universally validated checklist. Which trade-offs matter depends on the runtime, task, privacy requirements, and consequences of a mistaken or missing recollection.

Related reports reveal other ways continuity can break

Additional public reports describe related concerns, but they are separate user accounts—not independent verification of the four examples above.

  • One Claude Code report says persistent memory files may not be consulted after compaction and describes an agent acting on an old crash log despite newer operational notes. That distinction matters: having a memory available does not ensure the runtime will retrieve the right one.
  • Another report says concurrent agents may update a shared plain-text memory file without concurrency control, creating a risk of lost writes.
  • A user report says a setting intended to disable automatic compaction was ignored near approximately 78% context usage. That figure is the reporter’s observation, not a general threshold or official specification.
  • A separate report describes repeated compaction after large agent-listing data was resent, calling the pattern “thrashing.” Its transcript measurements describe affected sessions and should not be generalized into an industry statistic.

Issue reports are useful signals for what to investigate. On their own, they do not establish prevalence, reproducibility on current releases, or whether a problem has been fixed.

An implementation example: observational memory

Pi’s observational-memory extension documentation describes an approach that captures observations and reflections, prepares memory before compaction, and supports recall backed by sources. It provides a concrete example of how a project documents some of the design questions above; it is not an independent evaluation of effectiveness.

The project notes that its active development branch can differ from the stable package. Check the project’s current release and compatibility information before relying on a particular behavior or integrating it into a runtime.

What to verify in your own agent runtime

For a long-running coding task, examine the whole path from active context to later recall rather than looking for a feature labeled “memory.” A useful review can follow the lifecycle:

  1. Before compaction: Identify what the runtime preserves, what it summarizes, and whether important decisions or constraints can be reviewed before the original context is reduced.
  2. After compaction: Confirm whether the runtime actually loads or searches durable memory again. A stored note that is never consulted cannot restore continuity.
  3. When a fact is recalled: Check whether the system can identify the source passage or distinguish original observations from summaries and reflections.
  4. As records age: Find out how obsolete entries are removed, whether important records can be pinned or referenced, and how conflicts are handled.
  5. With concurrent agents: Determine how simultaneous updates are serialized or merged, and whether one writer can silently replace another’s changes.
  6. For sensitive work: Establish where memory is stored and what controls govern its retention and access.

The HyperMarrow builder advocates a local-first memory layer. That is the author’s product perspective and design proposal, not an independently validated claim that local storage is best for every user or workload.
