CIOs should replace rules built for human-operated software with an operating model for AI agents that can act with delegated authority. That means sharing leadership across business functions, enforcing policy while agents run, limiting each agent’s authority, assigning owners throughout its lifecycle, and measuring outcomes alongside risk and cost.
Why the old rules are breaking down
AI agents can operate continuously, use connected tools, and take actions across departmental boundaries. That creates a control problem as much as a technology one: leaders may be accountable for systems they cannot fully see or constrain.
An IBM Institute for Business Value survey of 2,000 senior technology executives, conducted with Oxford Economics across 33 geographies and 19 industries from January to April 2026, found that two-thirds of surveyed CIOs and CTOs said they were accountable for AI systems they did not fully control. In the same survey, 70% said business teams were deploying technology faster than IT could track, 77% said AI adoption was already outpacing current governance capabilities, and 11% said they were fully ready for the scale of agent deployment expected in the next year. Surveyed organizations reported an average of 54 AI agent incidents in the prior year; IBM defined these as unintended or harmful occurrences requiring human correction. These are respondent findings, not universal rates. IBM’s June 2026 study release describes the survey and its results.
Rule 1: Replace “AI belongs to IT” with shared business leadership
The CIO should set technology standards and coordinate deployment, but should not be the sole owner of decisions that change business processes, costs, workforce responsibilities, or liability. Centralize guardrails; distribute accountability to the leaders responsible for the outcomes and risks.

Gartner recommends an AI agent layer council co-led by the CIO, CFO, COO, CHRO, and general counsel. In practice, that gives the group a place to align technology standards and deployment, cost transparency, business-outcome baselines, workforce change, and risk and liability. Gartner also recommends clear accountability and a board-approved RACI. This does not mean every agent must be built centrally: business teams can own use cases within enterprise standards. Gartner forecasts that 45% of CIOs will lead AI agent systems outside IT by 2028; that is a forecast, not a measured current adoption rate. Rule 2: Replace “publish policy and audit later” with runtime enforcement
A policy document can state what an agent may do, but it cannot stop an out-of-bounds action by itself. Translate business rules into controls that can allow, pause, redirect, or block actions during execution. Define data boundaries, approval thresholds, and escalation paths before the agent is connected to operational systems.
Keep observability and enforcement distinct. Logs and traces help people understand what happened; governance determines what is allowed and intervenes when a proposed action crosses a boundary. For example, a customer-service agent could handle routine refunds but pause or route a refund above the company’s chosen threshold for approval. The amount should come from the organization’s own risk and business rules, not a generic agent setting. IBM’s enterprise guidance on governing third-party agents discusses this distinction and the approval example.

Rule 3: Replace broad permissions with least privilege and least agency
Give an agent only the data, tools, and decision rights required for its assigned job. Least privilege limits what it can access; least agency limits what it can decide or execute. If a task only requires a recommendation, the agent may not need permission to carry it out. Expand autonomy deliberately when the expected value warrants the added risk.
Set controls according to both autonomy and impact. An internal productivity agent generally calls for lighter controls than a customer-facing or decision-making agent; a mission-critical agent warrants greater rigor. Microsoft’s maturity guidance describes this risk-based approach. Microsoft’s agentic AI maturity model sets out the governance and security considerations.
For multi-agent workflows or third-party services, do not assume that one agent’s trust or authority should transfer to another. Pass only the context and permissions needed for the delegated task, and keep the limits clear at each handoff.

Rule 4: Replace one-time approval with accountable lifecycle ownership
Approval at launch is only a starting point. Keep an inventory of agents and name both an accountable owner and an operator for each one. Record the agent’s purpose, environment, third-party dependencies, autonomy, access, authority, and likely impact so reviewers can understand what it is meant to do and what could go wrong.
Monitor behavior and retain evidence of decisions and actions. Reassess an agent after a material change to its model, tools, permissions, provider, or business purpose; a change in any of these can alter its risk profile even if the agent’s name and user interface stay the same.
Standards for agent security, identity, and interoperability are still developing. NIST’s February 2026 announcement describes an active AI Agent Standards Initiative, with research and guidelines forthcoming; it is not a completed final standard. Enterprises should build controls they can operate now rather than treat that initiative as an existing certification or a substitute for their own accountability.

Rule 5: Replace adoption counts with outcome, risk, and cost discipline
Counting deployments, licenses, or prompts can show activity, but not whether an agent is improving the work or creating costs and risks that outweigh the benefit. Establish a departmental baseline before deployment, then track a business outcome alongside the control signals relevant to that use case.
- Measure the result: use task-specific measures such as cost per resolved claim or completed policies, rather than a single enterprise-wide usage number.
- Track control performance: review overrides, approval escalations, and incident reports to see where human intervention remains necessary.
- Include contractual exposure: monitor vendor indemnity and other risk terms alongside operational performance.
- Make spending visible: connect agent costs to the business process and outcome they support, including relevant model, tool, and provider costs.
Cost visibility is becoming more important as deployment scales. In the same IBM survey, technology executives projected AI spending to rise from just under 15% of IT budgets in 2025 to nearly 25% by 2027. Those figures are respondent-based projections, not a forecast for every company. The practical implication is to tie investment reviews to measured outcomes and risk controls, rather than assuming that higher usage means higher value. IBM’s survey release reports the projection.
