AI agents often guess because generating a plausible continuation is the language model’s default operation, while searching is a separate decision that can be costly, poorly calibrated or badly integrated. If an evaluation rewards a correct guess but gives no credit for saying “I don’t know,” the system has a measurable incentive to answer—even when checking a source would be safer.

What the model is optimizing when it answers

Pretraining teaches a language model which words and ideas are likely to follow one another. It does not attach a truth label to every sentence in its parameters. At answer time, producing a fluent continuation is therefore the path of least resistance; recognizing uncertainty and initiating a search require additional policy decisions.

For an arbitrary fact that cannot be determined from the model’s training data, there may be no reliable internal signal that separates a plausible statement from a true one. Microsoft Research’s Adam Tauman Kalai and Santosh S. Vempala describe a statistical limit: “For ‘arbitrary’ facts whose veracity cannot be determined from the training data, we show that hallucination is necessary for language models that satisfy a statistical calibration condition appropriate for generative language models.” That does not make every hallucination unavoidable, but it explains why better wording alone cannot eliminate all unsupported answers.

Why benchmarks can reward the wrong behavior

Accuracy-only scoring creates a multiple-choice-test incentive. Leaving a question blank guarantees zero; guessing earns a point if the guess happens to be right. OpenAI summarizes the problem this way: “standard training and evaluation procedures reward guessing over acknowledging uncertainty.” It also warns that “Strategically guessing when uncertain improves accuracy but increases errors and hallucinations.”

Model in OpenAI’s SimpleQA example Abstention Accuracy Error What the figures show
gpt-5-thinking-mini 52% 22% 26% It declines often and makes fewer wrong claims.
o4-mini 1% 24% 75% It scores slightly higher on accuracy while producing many more errors.

These are reported results for that SimpleQA example, not a universal ranking of the models. The important distinction is between raw accuracy and decision quality: a system that knows when not to answer can be more useful than one that raises a narrow accuracy score by making confident guesses.

Searching is a learned policy, not an automatic reflex

Giving an agent a web or database tool does not guarantee that it will use it. The agent must first detect that outside evidence is needed, select a tool, formulate a query, interpret the returned material and incorporate it without contradicting the evidence. An error at any stage can result in an unsupported answer.

Prompt wording and the examples shown in an execution trace can shift the probability of calling a tool. The ProbeCal work in ACL Findings 2024 (pages 16,781–16,805) specifically studies uncertainty calibration for tool-using agents and identifies prompt design and trace selection as places where tool-use probabilities can be miscalibrated.

Cost and latency also influence the policy. A search request consumes time, money and context space, so a system trained or prompted to be fast may skip it when the model believes—correctly or not—that its internal knowledge is sufficient. Conversely, a “search everything” rule can waste resources and create new errors when retrieved pages are irrelevant.

What tool-use evaluations actually find

Google Research’s 2024 evaluation covered more than 340 experiments. Across its setups, few-shot tool assistance generally did not outperform a no-tool baseline; results varied substantially with the strategy and task. For knowledge-seeking questions, retrieving documents and then using them to edit or verify an already generated answer performed better than relying only on tool calls during generation.

The finding is not that tools are useless. It is that an exposed search API is not a complete workflow. Retrieval needs a trigger, a query strategy, source-quality checks and an evidence-aware answer stage. Those components must be evaluated together rather than assuming that any tool call is progress.

What retrieval-augmented generation fixes—and what it cannot

Retrieval-augmented generation (RAG) gives the model access to current or domain-specific documents, reducing dependence on stale parametric knowledge. It does not prove that the answer is grounded. A retrieved set may be incomplete, irrelevant, outdated, unauthoritative or misread by the generator.

An AAAI 2026 study found that irrelevant retrieved documents could make retrieval-augmented models refuse questions they were capable of answering. It also found that stronger refusal behavior did not necessarily produce better calibration or higher accuracy. A model can therefore be too willing to answer, or too willing to refuse, depending on the evidence it receives.

The practical distinction is between knowledge access and epistemic control. Search supplies candidate evidence. A separate policy must decide whether evidence is necessary, whether the retrieved material is sufficient and authoritative, how conflicts are resolved, and when the correct action is to abstain or ask for clarification.

A workflow that makes “look first” measurable

  1. Classify the question. Mark requests that depend on current facts, private data, exact figures, regulations, medical or financial details, or information outside the model’s verified scope. These are search-required or search-preferred cases. Stable explanations and transformations may not need retrieval.
  2. Set a trigger policy. Record whether the agent searched, why it searched or did not search, and what confidence threshold or question category caused the decision. Do not treat a tool call by itself as success.
  3. Retrieve before committing to a final answer. Use a query that reflects the user’s exact entities, dates and constraints. Keep source metadata such as publication date, author and document location.
  4. Filter the evidence. Check relevance, authority, freshness, completeness and contradictions. If the results are empty or unrelated, do not silently convert that failure into certainty.
  5. Draft with claim-level support. Map each material factual claim to the document or passage that supports it. Separate what the source says from the model’s inference.
  6. Verify or edit the draft. Run a second pass that checks whether every important claim is entailed by the retrieved material, whether numbers retain their qualifications, and whether conflicting sources are disclosed.
  7. Choose an uncertainty action. Answer when evidence is adequate, ask a clarifying question when the request is underdetermined, and abstain when the available evidence cannot support a reliable response.
  8. Log the decision. Store the query, documents considered, citations used, conflicts detected, final confidence and whether a human or later verifier changed the answer. This makes calibration measurable instead of anecdotal.

Architecture choices and their trade-offs

Pattern Strength Typical failure Best control
Parametric answer only Low latency and no retrieval cost Stale or invented facts Require abstention for out-of-scope or time-sensitive questions.
Tool call during generation Can fetch information while reasoning Mis-triggered searches, poor queries or badly integrated results Calibrate the call policy and inspect execution traces.
Retrieve, then edit or verify Separates evidence gathering from final wording; favored by Google’s knowledge-seeking comparison Irrelevant or incomplete documents can still mislead the editor Apply relevance, authority and entailment checks before release.
Retrieval plus independent verifier Can catch unsupported claims and numerical errors Extra latency and the verifier can share the generator’s blind spots Use separate prompts, evidence links and adversarial test cases.

How to evaluate an agent that is supposed to check its work

Do not report only the percentage of exact answers. Track at least these dimensions:

  • Trigger quality: Did the agent search when the question required outside evidence, and avoid unnecessary searches when it did not?
  • Retrieval quality: Were the returned documents relevant, sufficiently complete, authoritative and current?
  • Calibration: Did confidence match the probability of being correct?
  • Abstention quality: Did the agent decline genuinely underdetermined questions without refusing answerable ones?
  • Conflict handling: Did it surface disagreements instead of selecting a convenient source silently?
  • Evidence coverage: Can each material claim be traced to a supporting passage?
  • Operational cost: What latency, token use and tool expense does the policy add?
  • Auditability: Can an evaluator reconstruct the query, sources and verification decision?

Test with deliberately stale, irrelevant, conflicting and empty retrieval results. The AAAI findings show why: improving refusal behavior can move separately from improving calibration or accuracy. A system that refuses every difficult question may look safe while becoming less useful.

Common fixes that do not solve the root problem

“Just tell it to search”

A forceful instruction cannot compensate for an unclear trigger, weak query generation or poor source filtering. Measure the actual search decision and its outcome.

“Add more documents to the index”

More documents can increase distractors. Coverage matters, but so do ranking, authority, freshness and the ability to recognize that no retrieved document answers the question.

“Make it refuse more often”

Refusal is not the same as calibration. An agent that refuses answerable questions has reduced its usefulness without proving that its remaining answers are more accurate.

“Cite something at the end”

A citation that does not entail the claim is decoration. Verification must happen at the claim level, including units, dates, scope and exceptions.

The bottom line for builders

An AI agent guesses because fluent generation is its native behavior and many scoring systems reward an answer more than a well-calibrated admission of uncertainty. Search is a separate, fallible policy with costs and integration hazards. RAG can supply better evidence, but only an explicit control loop—trigger, retrieve, filter, verify, then answer or abstain—turns that evidence into dependable behavior. Evaluate confident errors more harshly than uncertainty, log every evidence decision, and treat “look first” as a testable product requirement rather than a prompt slogan.
