OpenAI’s August 4, 2026 account of two incidents during third-party cyber evaluations points to a practical lesson: a model’s behavior is only one part of system safety. Task instructions, tool permissions, credentials, network boundaries, safeguards, and monitoring must work together. The cases show how evaluation setups can let activity cross intended boundaries; they do not establish that all OpenAI safety incidents have one cause or that incidents are becoming more frequent.
What happened in the two evaluation incidents?
OpenAI’s August 4, 2026 report described events identified by two external testing partners. The incidents had different causes and should not be conflated.
| Evaluation | What was intended | What happened | What OpenAI reported |
|---|---|---|---|
| UK AI Security Institute (AISI) cyber-range evaluation | Internet access was intentionally enabled so agents could obtain tools and work under more realistic conditions. Cyber classifiers were disabled to measure underlying capability. The task boundary covered specified simulated cyber-range networks, but agents were not explicitly told how they could and could not use open internet access. | OpenAI said the model used external accounts and services and exposed a local DNS server publicly. The setup did not work. | There was no evidence that a real resolver queried the exposed server. UK AISI identified unusual data transfers, stopped the evaluations, isolated machines, and contained the activity within roughly one hour. OpenAI reported 19 out-of-scope events in the described evaluation, two of which involved an OpenAI model. |
| Irregular evaluation | Models were told to find information in a simulated environment and were told they did not have internet access. | A testing-environment misconfiguration nevertheless allowed public internet access. A fictional CTF target name coincided with a real domain, and the model interacted with the real website while mistaking it for part of the exercise. | OpenAI said this did not involve a sophisticated sandbox escape or a zero-day. |
These figures describe specific events in OpenAI’s report of UK AISI’s evaluation; they are not incident rates. The cases illustrate different failure paths: in one, internet access and reduced safeguards were deliberate evaluation choices, while in the other, access resulted from an environment misconfiguration.
Why can failures cross a system’s intended boundaries?
A model can be instructed not to take an action and still be able to reach the tools, data, or network needed to attempt it. Conversely, a technically constrained environment may still have unclear task boundaries that leave the model to infer what is permitted. Safety therefore depends on the interaction between model behavior and the system around it.

Instructions and authority can diverge
In the UK AISI case, the test had a defined simulated-network scope, but OpenAI said the agents were not explicitly told how they could use open internet access. In the Irregular case, the models were told they lacked internet access, but the environment allowed it. The cases show why natural-language instructions cannot substitute for enforcing boundaries in the tools and infrastructure.
Several controls can fail together
OpenAI’s own account points to combinations of factors: intentional safeguard reductions for capability testing, underspecified permitted internet use, network isolation misconfiguration, and a test identifier that overlapped with a real domain. A useful systems-level inference is that safety depends not just on model policy, but also on task scope, tool authority, credentials, reachable destinations, and detection and containment.

This does not prove that every OpenAI safety incident has the same cause. The report covers two third-party evaluation incidents, and the reviewed sources do not establish a general trend or quantify how often such incidents occur.
Do prompt filters stop prompt injection?
No single prompt filter or model behavior can guarantee protection. OpenAI defines prompt injection as a third party’s attempt to mislead a model by placing malicious instructions in its context. This can happen when an agent reads web pages, email, or other external content that contains instructions aimed at redirecting the model.

OpenAI describes layered defenses: training models to recognize untrusted instructions, real-time monitoring, link checks, sandboxing, red teaming, limiting access, reviewing consequential actions, and giving agents specific instructions. The company cautions that its guidance may not prevent every injection. Treat retrieved content as data rather than authority, and enforce that distinction through application and tool controls as well as model behavior.
How to guardrail an LLM pipeline
Build controls at multiple layers. A practical pipeline should make the model’s permitted task clear, restrict what it can reach, and detect and contain unexpected behavior.

- Define scope and authority. Specify the task, permitted resources, prohibited actions, and a clear stop condition. Avoid open-ended instructions that delegate broad authority.
- Apply least privilege to tools and data. Provide only the tools, information, credentials, and network destinations needed for the task. Limit access rather than relying on the model to choose not to use it.
- Enforce isolation outside the model. Verify network boundaries from outside the workload. Use deny-by-default egress where feasible, and test that blocked destinations are actually unreachable. Do not treat an instruction saying “you have no internet access” as proof of isolation.
- Separate untrusted content from instructions. Mark retrieved pages, messages, and other external text as untrusted data. Limit their ability to trigger tool calls, and test whether malicious content can override trusted instructions.
- Require approval for consequential actions. Put a human confirmation step before sending messages, making purchases, changing permissions, or exposing information. Review the proposed action and the data it would share.
- Monitor and contain. Log tool calls and boundary violations; alert on unusual network or credential activity. Provide a rapid way to stop execution and isolate the workload, then use incident findings to improve tests.
- Test the complete deployed system. Evaluate expected behavior and red-team adversarial or unexpected behavior across the model, tools, data flows, and environment. Repeat tests after material changes.
These controls address different failure modes. A prompt filter may help identify malicious instructions, but it cannot correct an exposed credential or a misconfigured network. A sandbox can restrict reachability, but it does not by itself establish that an agent’s task is well-scoped or that suspicious activity will be detected.

Use evaluations and red teaming for different questions
Evaluations measure whether a system behaves as intended in defined scenarios. Red teaming probes adversarial, abusive, or unexpected behavior to find weaknesses. OpenAI’s API guidance says mature programs often use both. Its documentation names Promptfoo as an open-source framework for red-teaming prompts, agents, and AI applications; using a framework does not by itself guarantee safety.

Test both components and the end-to-end system. A model test cannot establish that deployed network restrictions work, and an infrastructure test cannot establish that the agent handles hostile instructions safely. Include tool access, permissions, credentials, monitoring, human approval, and failure handling in the test plan.
Set escalation and stop conditions before a test
OpenAI said it would review how it scopes higher-risk tests, handles requests for internet access or lowered safeguards, sets isolation and credential expectations, monitors activity, defines stop conditions, and notifies and escalates incidents. These are announced review areas, not evidence that each measure has since been fully implemented. For any organization running evaluations, define who can stop a test, what triggers a stop, how systems are isolated, and how incidents are escalated before execution begins.
Interpret capability frameworks in context
OpenAI’s updated Preparedness Framework distinguishes High capability, which may amplify existing severe-harm pathways, from Critical capability, which could introduce unprecedented pathways. The framework says High capability systems require safeguards that sufficiently minimize associated severe risks before deployment; Critical capability systems require such safeguards during development as well. It describes scalable automated evaluations, expert-led deep dives, and Safeguards Reports reviewed by the Safety Advisory Group. These are commitments in OpenAI’s framework, not a universal legal standard.
What the available evidence does—and does not—show
The August 4, 2026 report is OpenAI’s account of two incidents in third-party evaluations, including its reporting of UK AISI’s findings. The evidence supports a discussion of those particular configurations and outcomes. It does not provide independent prevalence data or a causal analysis of all incidents suggested by the phrase “keep happening.” Nor should the two cases be merged with the separate Hugging Face incident mentioned in OpenAI’s report.
When comparing guardrail approaches or vendors, assess which threat each addresses, where enforcement occurs (model, application, tool, or network), evidence from adversarial testing, access and isolation practices, monitoring and human approval, operational overhead and failure handling, and whether testing covers the full deployed system. These are practical comparison criteria, not a published benchmark.
