Newer medicines that act on incretin hormone pathways have drawn clinical attention because trials found large average weight reductions compared with placebo. In separate studies, semaglutide and tirzepatide produced substantial mean losses over roughly 16 to 17 months. Those results help explain the interest; they do not guarantee an individual result, show that every patient is eligible, or prove the drugs should be compared as if they were tested head to head.

Why are doctors paying attention to these medicines?

The interest is grounded chiefly in results that stand out against placebo in clinical trials, alongside product labels that include uses for certain weight-related conditions. Semaglutide is a GLP-1 receptor agonist. Tirzepatide acts through both GIP and GLP-1 receptor pathways. These medicines are not simply interchangeable versions of one another: their labeled uses, dosing, safety information and practical access depend on the specific product and patient.

“Frenzy” describes the public framing of the topic, not a measured finding about how enthusiastic doctors are or how often they prescribe these drugs. The evidence summarized here establishes clinical trial outcomes and regulatory information, not physician sentiment or current prescribing rates.

How much weight did people lose in the trials?

The following are average results at the stated follow-up points. They describe groups under study protocols, not a forecast for any one person.

The STEP 1 and SURMOUNT-1 figures come from separate trials with different protocols and follow-up periods. They are not a randomized comparison of semaglutide against tirzepatide, so the percentages should not be read as a direct ranking of the medicines.

What does a direct comparison show?

SURMOUNT-5 was an open-label controlled trial comparing maximum tolerated doses over 72 weeks in adults with obesity without type 2 diabetes. Participants received tirzepatide at 10 mg or 15 mg, or semaglutide at 1.7 mg or 2.4 mg, once weekly. This design directly compared the medicines, unlike STEP 1 and SURMOUNT-1. The study information summarized here confirms the design but does not provide numerical outcome results, so no head-to-head weight-loss figure is stated here.

Even a direct trial result would need to be interpreted in light of its population, protocol and endpoints; it would not determine which medicine is right for a particular patient.

What are the labeled uses, and who may qualify?

Eligibility is specific to the medicine, indication and patient. The 2026 FDA prescribing information for Wegovy includes long-term weight reduction and maintenance for specified patients, as well as certain cardiovascular-risk and metabolic dysfunction-associated steatohepatitis (MASH) indications. These are distinct labeled uses, not evidence that losing weight automatically prevents every condition associated with obesity.

The FDA-approved Wegovy injection dose of 7.2 mg, approved March 19, 2026, is for weight loss and long-term maintenance in certain adult patients. That dose is not a general recommendation for everyone taking semaglutide; clinicians must use the current product label to determine whether a specific patient and indication fit.

The available information does not establish a single eligibility rule that applies across all products and indications. A clinician can assess the relevant label, the person’s medical history and treatment goals rather than inferring eligibility from trial averages alone.

What side effects and warnings matter?

Wegovy label information

The 2026 Wegovy label lists nausea, diarrhea, vomiting, constipation and abdominal pain among common adverse reactions. Its boxed warning describes thyroid C-cell tumors in rodents; whether semaglutide causes these tumors in humans is unknown. Wegovy is contraindicated for people with a personal or family history of medullary thyroid carcinoma or with multiple endocrine neoplasia syndrome type 2 (MEN 2). The label includes additional warnings and precautions, so this short list is not a substitute for reviewing it with a prescriber.

FDA review of suicidal ideation and behavior

On January 13, 2026, the FDA said its review found no increased risk of suicidal ideation or behavior with GLP-1 receptor agonists and requested removal of that warning from affected labels. This is a specific FDA review and labeling update; it does not mean these medicines have no other risks.

Are compounded versions the same as approved medicines?

No. FDA says compounded drugs are not FDA-approved and are not reviewed before marketing for safety, effectiveness or quality. It has reported dosing errors with compounded injectable semaglutide, including reports requiring hospitalization, and warns that semaglutide salt forms differ from the active ingredient used in approved drugs. FDA advises patients to obtain a prescription from a doctor and use a state-licensed pharmacy.

On April 30, 2026, the FDA announced a proposal to exclude semaglutide, tirzepatide and liraglutide from the 503B bulks list, saying it found no clinical need for outsourcing facilities to compound them from bulk drug substances. The announcement invited comments and said the agency would consider them before making a final determination. It describes a proposal, not a final rule.

What should patients know about availability and coverage?

Availability can change, and shortage information is time-sensitive. FDA records that it determined the semaglutide injection shortage was resolved on February 21, 2025; that dated update does not establish the current status of every product or local supply. Check live FDA shortage information and ask a pharmacy or prescriber about current availability.

A useful decision is about more than which trial reported the largest percentage. Trial averages can inform that conversation, but they cannot decide it for an individual.
