Google may omit review stars from a healthcare page because the provider’s own website controls the reviews about that provider. Google treats those as self-serving reviews, and pages using Organization or LocalBusiness structured data are ineligible for the review-stars feature in that situation. A medical practice, clinic, hospital, or specialist does not receive a documented exemption. If the reviews are not self-serving, stars can still be absent because the markup is inaccurate, the review content is not visible, the page cannot be crawled, or Google chooses another search presentation even when the implementation is valid.
The most likely reason: your site is marking up reviews about itself
Google’s review-snippet policy excludes a page when the entity being reviewed controls the reviews about itself. Google states that if an entity controls reviews about itself, pages using LocalBusiness or any Organization type are ineligible for the star review feature. That rule applies to provider-owned healthcare websites when they mark up patient reviews or ratings about the same provider.
The restriction is about control and subject matter, not about whether the testimonials are genuine or whether the healthcare organization is large, regulated, or professionally licensed. A clinic that collects patient feedback, selects testimonials, and publishes them on its own domain is still the entity being reviewed. Passing a structured-data test cannot override that eligibility rule.

Google’s September 18, 2019 explanation says the reviews are considered self-serving because the entity chose to add the markup to its own pages about its own business or organization. The announcement is available in Google’s Search Central Blog.

An embedded widget does not automatically change ownership
Replacing manually entered testimonials with a widget does not necessarily solve the problem. Google’s review-snippet documentation gives a third-party widget embedded on the entity’s own site as an example of self-serving review markup. The important question is still who is being reviewed and whose page is displaying the markup, not whether another service supplied the interface.
Copying a rating from an external review platform, loading it with JavaScript, or embedding a badge may make the rating look independent to visitors. It does not automatically make the provider’s own page eligible for Google’s review stars. The page remains a provider-controlled page about that provider.
Healthcare does not create a documented exception
Google’s cited review-snippet guidance does not identify healthcare as a special exception to the Organization or LocalBusiness rule. Apply the same general policy to a physician practice, dental office, therapy center, hospital department, urgent-care site, or other healthcare organization unless Google publishes a different requirement for the specific feature.
A specialty designation also does not change the analysis. Marking a page as a medical organization, physician, dentist, or another healthcare subtype may improve the semantic description of the business, but it does not turn first-party reviews about that business into eligible third-party review snippets.
This distinction matters because healthcare sites often contain several kinds of content:
- Reviews about the provider: patient ratings, testimonials, and comments about the clinic or practitioner. These are the reviews implicated by the self-serving restriction when the provider controls the page and markup.
- Reviews of another entity: an independent publisher’s article evaluating several hospitals, practices, or services. The publisher may be in a different eligibility position, provided it follows all review-snippet requirements.
- Information about a treatment or product: a service description or product page is not automatically a review page. Adding ratings that do not represent visible, relevant review content can create a structured-data mismatch.
Review eligibility is separate from whether Google displays stars
Even an eligible page with valid structured data is not guaranteed to receive stars in every search result. Google describes structured data as a way to make a page eligible for a search feature, not as an instruction that forces a particular visual treatment. Google can select a different result format based on the query and other factors, including search history, location, and device.

Consequently, two people can search for the same healthcare organization and see different result presentations. A star result that appeared previously can disappear after Google recrawls the page, changes the result layout, or determines that another presentation is more useful. A passing test confirms that Google can parse the markup; it does not promise a star line in live search.

Google’s General Structured Data Guidelines also require the markup to represent the page’s main content and not be misleading or hidden. A technically valid object can still fail feature policies if it describes content users cannot find or if it is unrelated to the page’s principal subject.
Other reasons stars can be missing
The markup describes content that visitors cannot see
Review text and its rating should be readily available to users on the marked-up page. If the review exists only in JSON-LD, in a collapsed element that users cannot access, behind a login, or in a component that fails to load, Google may determine that the structured data does not match the visible page. The text, score, and entity described in the markup should be understandable from the rendered page itself.
The rating concerns the wrong entity
A review snippet must concern a specific item. A healthcare page about one clinic should not use an aggregate rating for a parent network, a separate physician, an unrelated product, or a treatment unless the page clearly and visibly reviews that exact subject. Entity names, URLs, and the page’s principal content need to agree.
Ratings were improperly aggregated
Google’s guidance requires ratings for a local business or organization to come directly from users. A page should not combine scores copied from several other websites into one synthetic average, and it should not rely on human editors to compile an organization’s rating while presenting it as a user rating. Reviews from other websites must not simply be aggregated into the provider’s own rating markup.
The page is not accessible to Google
A blocked or restricted page cannot reliably qualify for a search feature. Check for a robots.txt rule that disallows crawling, a noindex directive, authentication or login requirements, and accidental blocking of scripts or resources needed to render the review content. Google’s review-snippet documentation specifically calls out crawl access and page availability as prerequisites.

Google has not recrawled the change
Removing a blocking directive or correcting markup does not update search results immediately. Google says recrawling and reindexing after publication or changes can take several days. During that interval, an old version of the page may still be represented in search, or the page may not yet be evaluated for the feature.
A diagnostic checklist for a healthcare page
Work through the checks in this order. The first question can settle the issue before any technical editing is worthwhile.
| Check | What to examine | What the result means |
|---|---|---|
| Review ownership | Is the page about the same provider or organization that collected, selected, or controls the reviews? | If yes, the self-serving restriction is likely decisive for review stars. |
| Entity and type | Does the structured data identify the same healthcare organization that the visible page describes? | A mismatch can make the markup misleading or ineligible. |
| Visible evidence | Can a visitor read the review text and see the rating on the page without logging in? | If not, the markup may not represent user-visible content. |
| Rating provenance | Do ratings come directly from users, rather than an editor’s calculation or a blend of external sites? | Improper aggregation conflicts with Google’s review requirements. |
| Crawlability | Is the URL open to Googlebot and free of robots.txt, noindex, or login barriers? |
A restricted URL cannot be reliably processed for rich results. |
| Validation | Does the Rich Results Test parse the intended review data, and does URL Inspection show the page as Google sees it? | Tools can reveal implementation and indexing problems, but neither guarantees stars. |
| Timing | Were corrections made recently? | Allow several days for recrawling and reindexing before judging the change. |
How to check the page step by step
- Identify the reviewed entity. Read the page as a visitor would. Write down the exact organization being reviewed: a named clinic, a specific practitioner, a hospital, or another entity. Compare that with the organization name, URL, and rating subject in the structured data.
- Determine who controls the reviews. Check whether the provider solicits, moderates, selects, imports, or publishes the patient reviews. If the provider controls reviews about itself, treat the review-stars restriction as a policy outcome rather than a coding bug.
- Open the rendered page, not only the source. Confirm that review text, author information where shown, and the rating are available to ordinary visitors. Test the page in a logged-out browser and with scripts enabled and disabled as appropriate for the implementation.
- Run Google’s Rich Results Test. Enter the public URL or the relevant code and inspect which structured-data items Google recognizes. Correct syntax errors and warnings that describe real content problems, but do not add fields merely to pursue stars that policy excludes. The test is available at Google’s Rich Results Test.
- Inspect the URL in Search Console. Use URL Inspection to check indexing status, the last crawl, canonical selection, and any access or enhancement information Google reports. This reveals Google’s stored view of the page rather than only what your browser displays.
- Check access directives. Review the page’s
robots.txt, HTML or HTTPnoindexdirectives, canonical URL, and authentication settings. Ensure that the public URL you validate is the URL you intend Google to index. - Request recrawling after a genuine fix. Once the page is accessible and accurately represents its visible content, request indexing in Search Console if appropriate. Expect a delay; Google notes that recrawling and reindexing can take several days.
- Evaluate live results only after processing time. Search from more than one device or location if you are checking visibility, because Google says result presentation can vary. Treat the absence of stars as a display decision unless the technical checks identify a specific problem.
What to do when the self-serving rule applies
If the provider controls reviews about itself, changing the schema vocabulary, adding more review properties, or switching plugins will not remove the eligibility restriction. Continuing to modify markup in the hope of forcing stars can create inaccurate or misleading data without changing Google’s policy decision.

Keep testimonials honest, clearly identified as testimonials or reviews, and visible in the page experience. Just do not promise that those first-party reviews will produce Google’s review-stars feature for the provider’s own search result.
If an independent site publishes a genuine review or comparison of healthcare organizations, that publisher can assess its own eligibility separately. The independent page must still identify the reviewed organization correctly, show the review content, use ratings that meet Google’s requirements, and comply with all structured-data policies. Moving a provider’s own reviews to another page you still control does not by itself make them independent.
Provider website versus independent review publisher
| Question | Provider-owned healthcare page | Independent publisher reviewing organizations |
|---|---|---|
| Who controls the page? | The healthcare entity or its authorized site team. | A publisher that is not the organization being reviewed. |
| What is usually marked up? | The provider as an Organization or LocalBusiness, with reviews about that same provider. |
Another organization being reviewed in the publisher’s editorial content. |
| How does self-serving policy apply? | Provider-controlled reviews about the provider are ineligible for the review-stars feature. | The self-serving rule may not apply, but every other review-snippet requirement still does. |
| What must be visible? | Any testimonial or rating shown in markup should be available to visitors on the page. | The review, rating, and the identity of the reviewed organization should be visible in the publisher’s content. |
| Are stars guaranteed? | No. | No. Valid eligibility still does not guarantee a particular search presentation. |
Do not confuse review snippets with Google’s reviews system
A review snippet is a search-result or Knowledge Panel presentation that may include a rating or excerpt when Google finds valid review or rating markup. It is the feature affected by the self-serving Organization and LocalBusiness rule described above.

Google’s reviews system is a separate ranking system intended to reward high-quality first-party review content such as recommendation articles, comparisons, and analysis. Google says that system does not evaluate third-party customer reviews placed in the reviews section of a product or service page. A patient-review widget on a clinic site and an editorial article comparing healthcare providers therefore involve different Google systems.

Assuming a high rating should force stars
A five-star average is not a command to Google’s renderer. Eligibility, crawlability, policy compliance, and Google’s own presentation choice all matter. A high score can remain visible to site visitors while no star appears in organic search.
Treating a passing test as a guarantee
The Rich Results Test checks whether Google can parse recognized markup. It does not decide whether a provider-controlled review is eligible under the self-serving policy, and it cannot guarantee that a rich result will be shown for a particular query.
Hiding reviews in markup only
Putting ratings in JSON-LD while omitting the corresponding review content from the visible page creates a mismatch. Google requires review information to be available to users on the marked-up page.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteCombining ratings from several platforms
An average calculated from multiple external websites may look comprehensive, but Google’s guidance does not permit simply aggregating reviews from other websites for the provider’s organization rating. Use the source’s rating as displayed by that source rather than inventing a combined score, and do not assume that embedding the source makes the provider page independent.

Changing the page type to evade the rule
Relabeling a clinic as a different schema type, attaching the rating to a parent brand, or placing the same reviews on a service URL does not solve a subject-and-ownership problem. Structured data must describe the actual main content and entity of the page.
What a correct diagnosis looks like
A defensible diagnosis names the exact condition found on the page. For example, “The page is the clinic’s own site and marks up patient reviews about that clinic, so the self-serving review policy explains the missing stars” is a policy diagnosis. “The page is eligible in principle, but it contains a noindex directive and the review text is hidden behind a login” is a technical diagnosis. “The page passes validation, is crawlable, and shows visible reviews, but stars are absent in one device’s results” is a display decision that Google may make even for an otherwise eligible page.
Do not claim that a healthcare site has lost stars for a particular reason until you inspect its URL, rendered content, directives, and structured data. Without those checks, the responsible conclusion is limited to the possible causes documented by Google.

Practical expectations after a correction
Make one evidence-based change at a time, record the date, and use URL Inspection to confirm when Google has crawled the updated page. Several days may pass before indexing catches up. After that, check representative searches without assuming that one browser, location, or device is universal.
If the page is provider-owned and the reviews are self-serving, no waiting period or recrawl will make those reviews eligible for the review-stars feature under the cited policy. If the problem was crawl access, visibility, provenance, or inaccurate markup, correcting the underlying issue gives Google a compliant page to evaluate, but the final display remains discretionary.
