An agent that completes a carefully chosen task in a demo has shown that it can succeed under those conditions—not that it will keep succeeding across real users, changing systems, tool failures, and hostile inputs. Production readiness requires evaluating the model and the software around it together, testing multi-step outcomes repeatedly, limiting what the agent can do, and monitoring it after launch. There is no established general failure rate for the jump from agent demos to production; the risk is better understood through the ways real operating conditions differ.

Why can an agent work in a demo but fail in production?

A demo usually shows a selected task in a controlled setup. The path may be favorable: the request is clear, the relevant information is available, tools respond as expected, and the agent’s actions have limited consequences. Production brings a wider range of requests and system states, plus variable model behavior, network and tool dependencies, data boundaries, and limits on latency and cost.

These differences matter because an agent is not just a model producing one answer. It is a model operating through a harness that receives requests, orchestrates tools, processes intermediate results, and returns an outcome. A mistake in one step can change the state the next step depends on. An agent can also say that it completed a task without having produced the intended result in the environment.

Testing in a controlled environment cannot cover every real interaction. NIST’s report on monitoring deployed AI systems explains why behavior in the field may differ from behavior in smaller or simulated test settings, even after extensive pre-deployment evaluation. Its authors call for repeated testing and validation after deployment as well as before it. NIST AI 800-4, “Challenges to the monitoring of deployed AI systems”.

What should an agent evaluation measure?

Evaluate the complete system on the task the user needs done, not just the model’s final text. That means observing the run’s tool calls, intermediate results, transcript, and final environment state. If the task is to update a record, for example, the grader should verify the resulting record rather than award success because the agent reported that it updated it.

Anthropic’s guidance describes agent evaluations as multi-turn trials with tool use, intermediate results, transcripts, graders, and outcomes in the environment. It also recommends multiple trials: a single successful run can hide variation in behavior. Anthropic’s “Demystifying evals for AI agents”.

  • Task outcome: Check whether the intended change or result actually occurred, and whether it stayed within the task’s boundaries.
  • Run behavior: Inspect the sequence of decisions, tool calls, intermediate state changes, and recovery attempts—not only the final response.
  • Variation: Run the same task more than once and report the distribution of outcomes and important failure types, rather than presenting only the best run.
  • Realistic context: Include representative user requests and production-derived contexts where privacy and policy allow. OpenAI describes using de-identified production traffic to make some evaluations more representative of deployed contexts and tool traces.
  • Operational fit: Track latency and cost alongside success, since an agent that works but exceeds the deployment’s operational limits may not be usable.

Use the same representative tasks when comparing deployments or architectures. Compare outcome success, repeatability, resilience to tool failures and adversarial inputs, permissions and reversibility, traceability, latency, cost, and the ability of operators to understand and intervene. Model capability alone is not a reliable substitute for these measures: NIST’s agent red-team competition found that attack success did not uniformly correlate with model capability.

How should tool access and security be handled?

Tool access determines what a failure can affect. Reading information is not the same as changing a trusted repository, and either differs from acting in an untrusted browser or computer-use environment. NIST’s tool-use taxonomy distinguishes read-only, constrained-write, and write permissions, as well as trusted and untrusted environments. Set access to match the task and the potential impact of an action. NIST’s “Lessons Learned from the Consortium: Tool Use in Agent Systems”.

  • Constrained write: Where changes are needed, limit them to the permitted objects or operations.
  • Write: Treat broader write access as a materially greater impact surface; test the actions and consequences it enables.
  • Untrusted content: Test how the agent handles instructions embedded in emails, websites, or code repositories, rather than treating retrieved content as automatically safe to follow.

Indirect prompt injection is one reason to test content as well as tools. NIST describes attacks in which instructions in external content can hijack an agent, with possible outcomes including data exfiltration or downloading and running malicious code. A public NIST CAISI red-team competition tested 13 frontier models in tool-use, coding, and computer-use scenarios. More than 400 participants made over 250,000 attack attempts, and the competition found at least one successful attack against every target model. Those are results from that competition—not an estimate of the chance of an attack succeeding in production. They support continuing adversarial evaluation, not a universal attack-rate claim. For high-impact or difficult-to-reverse actions, make review and intervention proportionate to the consequences. Human oversight is not simply approval of every action: operators need enough visibility to understand what is happening and practical ways to pause, redirect, or intervene. A human checkpoint is most useful when it meaningfully reduces risk, rather than adding friction to low-risk steps while leaving consequential actions opaque.

What should teams test before deployment?

  1. Define success in observable terms. Specify the task outcome and the environmental evidence that proves it. Do not use the agent’s own completion statement as the sole grader.
  2. Test the whole harness. Run realistic multi-step tasks through the actual orchestration and tools, and inspect intermediate results and state changes.
  3. Repeat trials. Measure consistency and document failure categories, not just whether one run succeeded.
  4. Exercise failure and attack cases. Include unavailable or misleading tool responses, relevant edge cases, and untrusted content that could contain conflicting instructions.
  5. Review permissions and consequences. Give the agent only the access needed for the task, and account for whether an action is constrained, reversible, or high impact.
  6. Check operational limits and oversight. Measure latency and cost, preserve useful traces, and ensure operators can understand and interrupt the system when appropriate.

These checks are a practical baseline, not a guarantee that every possible failure has been found. NIST notes that monitoring methods, validated practices, and common terminology for post-deployment monitoring remain nascent and scattered. The evidence therefore supports a lifecycle of evaluation and monitoring, not one universal benchmark or control set.

How do you monitor an agent after launch?

Continue observing outcomes and behavior in the deployed system. Track reliability drift, unexpected outputs and consequences, tool or security incidents, and whether the system remains within its intended access and operating limits. Preserve traces and evidence that help explain what happened, while handling user data under applicable privacy and policy requirements.

When field observations reveal a failure or a new pattern, feed it back into mitigations and pre-deployment evaluations. This closes the loop: tests become more representative as the team learns where the deployed system behaves differently from controlled trials. NIST’s monitoring report emphasizes repeated testing, evaluation, validation, and verification after deployment because pre-deployment testing has inherent limits. NIST AI 800-4.

For traceability, NIST is developing evaluation probes that compare factual claims against curated documents and produce machine-readable audit trails. This is an example of work in progress, not evidence that a single monitoring technique is already a complete solution. NIST’s evaluation-probes project.

What the available evidence does—and does not—show

The studies and guidance here explain why a successful demo does not establish production reliability, and why repeated trials, least-necessary access, adversarial testing, traceability, and post-launch monitoring matter. They do not establish a general percentage of agents that fail when moved from demos to production, or directly comparable production reliability rates across vendors.

Anthropic reported that, in its 2026 analysis, 80% of tool calls appeared to have at least one safeguard, 73% appeared to involve a human in some way, and 0.8% appeared irreversible. The classifications were inferred from tool-call context and did not distinguish production from evaluation or red-team activity, so these figures should not be read as production-wide rates or benchmarks.
