There is no evidence here that one smartwatch is the most accurate across all health sensors. Accuracy depends on the metric, activity, person, and reference used to check the reading. The strongest available findings are about specific Apple Watch measurements, older exercise tests, and ECG classification—not a current, head-to-head test of four watches.
That means a responsible recommendation has to be conditional: choose for the measurement you care about, and treat a watch reading as a wellness estimate or screening result unless a clinician and an appropriate medical test confirm it.
Why there is no single “most accurate” smartwatch
Heart rate, blood oxygen, ECG classification, sleep estimates, and calorie estimates are different measurements. A watch that performs well on one task is not automatically best at the others. Results also depend on the activity and measurement conditions, how the watch fits, the person wearing it, and what reference device or clinical method researchers used.

The available evidence does not establish a current, four-watch winner. It also does not verify hands-on testing of four models, so this is an evidence-based guide rather than a personal test or a ranking of four products.

What the studies say about specific health measurements
Heart rate: average accuracy can hide individual error
A 2025 living systematic review and meta-analysis of Apple Watch measurements reported a mean heart-rate bias of −0.27 beats per minute (95% confidence interval −0.72 to 0.17). Its limits of agreement were −7.19 to 6.64 bpm. In other words, the average difference from the reference was small, but individual readings could differ by several beats per minute. The authors also concluded that accuracy varied across models and was influenced by measurement conditions and individual physiology. Read the review in npj Digital Medicine.
Blood oxygen: a near-zero average bias is not a guarantee for each reading
The same review reported Apple Watch blood-oxygen mean bias of −0.04 percentage points (95% confidence interval −0.42 to 0.35), with limits of agreement from −4.00 to 3.94 percentage points. Those results describe the study evidence; they do not mean every wearer’s reading will be within four points of a clinical reference, or that a watch reading can rule out a health problem. See the review’s methods and findings.
Exercise heart rate: movement and activity type matter
A 2017 study comparing wearable heart-rate monitors during aerobic exercise found the Polar H7 chest strap had the best agreement with ECG among the devices tested (concordance correlation coefficient 0.996). Apple Watch followed at 0.92, TomTom Spark at 0.83, and Garmin Forerunner 235 at 0.81. The researchers reported the optical monitors agreed best on a treadmill and least on an elliptical. These are results for the study’s devices and protocol—not a ranking of current watch models. View the study record on PubMed.

For workout heart rate, this is a reason to check evidence for the exact watch and activity you care about. A chest strap was the closest comparator to ECG in that particular study; it does not make the strap a universal substitute for clinical assessment.
ECG and rhythm features: classification is not diagnosis
The 2022 BASEL Wearable Study evaluated five direct-to-consumer wearables. Its abstract reports correctly classified tracings of 92% or higher for each assessed device, while the proportion of inconclusive classifications was 18% for Apple Watch, 17% for Samsung Galaxy Watch 3, 24% for Withings ScanWatch, and 21% for Fitbit Sense. Overall inconclusive rates did not differ significantly among devices. These results describe the study’s devices and participants; they are not guarantees for an individual or head-to-head proof that a current model is best. An inconclusive result is not a diagnosis, and a watch classification should not replace medical care. Read the BASEL Wearable Study.
Older device evaluations do not settle newer models
A 2022 evaluation specifically examined the Fitbit Charge 4 and Samsung Galaxy Watch Active2. Its scope is those devices, not every later product from either brand. The study’s existence alone is not enough to make a claim about which current watch is more accurate. View the device evaluation on PubMed.

Which watch should you choose for your use?
If exercise heart rate is your priority
- Look for independent results for the exact model and the activity you do; treadmill performance does not establish accuracy during elliptical, interval, or strength workouts.
- Pay attention to agreement with a reference, not just whether two measures move together. A correlation or concordance statistic is not a promise that each reading is close.
- Check fit and comfort in real use. A sensor’s result is only useful if the watch stays in consistent contact with your wrist.
If you want ECG or rhythm screening
- Check whether the feature is available for your exact watch, phone, and region before buying.
- Expect that a result may be inconclusive. Do not treat a watch classification as a diagnosis or use it to delay medical attention.
If blood oxygen or sleep estimates matter most
Do not infer overall sensor quality from a heart-rate study. The cited evidence gives Apple Watch blood-oxygen agreement figures, but it does not establish the best current watch for oxygen measurement or sleep staging. For any health-related decision, use a measurement method appropriate to the question and seek clinical advice when needed.
How to judge an accuracy claim
Before trusting a headline number or buying on the strength of a sensor claim, check what the evidence actually covers:
- Metric: Is it heart rate, oxygen saturation, ECG classification, sleep, or another estimate?
- Condition: Was it tested at rest, during steady exercise, during intervals, or in daily life?
- Reference: Was the comparison against ECG, a chest strap, a clinical instrument, or another appropriate standard?
- Agreement: Does the report give bias and limits of agreement, or only a correlation or average? A small average error can coexist with larger differences for individuals.
- Population and fit: Who participated, and how were the watch and sensor positioned? Study results may not transfer equally to every person.
- Exact product and access: Does the evidence concern the same model and software you can buy, and can you use its features with your phone and in your region?
Compatibility can change what a watch offers in practice. In a September 30, 2026 review of the Samsung Galaxy Watch 9, TechRadar contributor Tom Bedford noted that some features in the tested configuration required a Samsung Galaxy phone. That is a reviewer observation about that model and setup, not a universal rule for every Samsung watch or feature. Read the Galaxy Watch 9 review.
