Mental-health conditions and medications can affect FAA medical certification, but a diagnosis does not automatically mean denial. The FAA uses condition-specific rules: an aviation medical examiner (AME) may be able to issue a certificate under stated criteria, or may need to defer the application for FAA review. The outcome depends on the applicant’s symptoms, treatment, medication history, and documentation.
This guide covers FAA airman medical certification in the United States. It is general information, not an eligibility decision for an individual pilot.
How the FAA handles mental-health conditions
The FAA’s psychiatric guidance distinguishes among conditions and their circumstances. Some cases have a defined path an AME may use; others require the FAA to make an individual decision. The FAA’s public mental-fitness overview uses broad language about automatic disqualification for psychosis, bipolar disorder, and some personality disorders. For the practical examination process, read that overview alongside the more detailed AME psychiatric disposition guidance.

|---|---|---|
| ADHD | The psychiatric table directs AMEs to separate ADHD and ADHD-medication guidance. | The diagnosis or medication alone does not settle the case; the applicable ADHD criteria must be reviewed. |
| Bipolar disorder | Pertinent medical information and a clinical status report are required; an FAA decision is required. | Do not expect routine AME issuance based only on the diagnosis being stable. |
| Depression treated with antidepressants | Pertinent medical information and a clinical status report are required. The table refers to the antidepressant policy and SSRI neuropsychological evaluation specifications; an FAA decision is required. | The underlying condition and the specific treatment pathway both matter. |
| Adjustment disorders | Conditional issuance may be possible when the condition is stable or resolved, there is no associated thought disturbance or recurrent episode, and medication-history criteria are satisfied. For the specified pathway, psychotropic medication use must have been less than six months and discontinued for at least three months; otherwise an FAA decision is required. | Check the exact table criteria with an AME; the duration and discontinuation conditions are part of the pathway. |
| Dysthymic disorder | Conditional issuance may be possible when the condition is stable or resolved, there is no associated thought disturbance or recurrent episode, and the stated medication-history criteria are met; otherwise an FAA decision is required. | Eligibility depends on the detailed criteria, not the label alone. |
| Psychosis, personality disorders, or a suicide attempt | The FAA table calls for pertinent records and FAA decision or review rather than routine issuance. | Expect individualized FAA review rather than assuming the AME can issue at the examination. |
How psychiatric medications affect certification
The AME Guide says use of a psychotropic drug is disqualifying for aeromedical certification purposes and directs the examiner to defer and forward records. The categories it names include sedatives, tranquilizers, antipsychotics, antidepressants (with SSRI exceptions), analeptics, anxiolytics, and hallucinogens. This is a general rule, not a complete medication-by-medication ruling: the FAA publishes specific policies and exceptions that apply to particular treatments.

For antidepressants, the FAA says people taking certain specified drugs can be considered for first-, second-, or third-class medical certificates. Authorization is decided case by case, and the AME may not issue the certificate directly. Symptoms must be controlled without adverse side effects, and the underlying condition must also be evaluated. Consult the FAA’s current antidepressant guidance for the current policy and medication details.
The FAA’s medication guidance emphasizes that both the medicine and the condition it treats can affect flight safety. Its MedGuide is a reference, not a stand-alone basis for an aeromedical decision; medication status must be considered with condition stability and side effects.

Symptoms matter even when a pilot is being treated
Certification and whether someone should fly on a particular day are separate questions. FAA antidepressant guidance says symptomatic pilots must not fly, regardless of treatment. A certificate or a possible certification pathway does not make active symptoms or medication side effects irrelevant to flight safety.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesWhat to do before an FAA medical examination
- Review the current AME Guide. The FAA recommends reviewing its guidance before an examination if you have a current or past medical condition or use medication. Start with the relevant psychiatric condition guidance and applicable medication policy.
- Talk with your AME and treating clinician. Discuss the condition, treatment, and likely documentation requirements before the examination. Do not stop or change prescribed treatment solely to pursue certification.
- Gather relevant records. Some cases require a detailed, current clinical progress note from a treating provider dated no more than 90 days before the AME examination. Follow the condition-specific requirements rather than assuming a general medical summary will be sufficient.
- Prepare for possible deferral. If the AME cannot issue under the applicable criteria, the application may be deferred to the FAA for review. Incomplete documentation can delay processing.
What determines the outcome
The useful questions are whether the applicable FAA table permits AME issuance or requires FAA review; whether the condition is stable or resolved and whether symptoms recur or involve thought disturbance; what medication was used, for how long, and when it was stopped; and whether symptoms or side effects remain. The required records and the AME’s authority to issue also vary by pathway. Because FAA policies can change and individual histories differ, confirm the current requirements with an AME before making treatment or examination decisions.
