Start with the settings that can help another person reach you or emergency services: complete Medical ID and emergency contacts, learn Emergency SOS, and review Fall Detection and Crash Detection on a compatible Watch. Then add Check In and the health alerts or tracking features that fit your needs. These tools are aids, not guarantees: Apple says Apple Watch cannot detect every fall or car crash, and most emergency calls require cellular service or Wi‑Fi calling.

Set up the emergency essentials first

Use the paired iPhone to establish the information your Watch can present when you cannot explain your situation. Keep it current after a medication change, diagnosis, move, or change in family contacts.

Medical ID and emergency contacts

  1. On the paired iPhone, open the Health app, tap your profile picture, then choose Medical ID.
  2. Enter relevant conditions, allergies, medications, blood type, date of birth, and other details. Add people under Emergency Contacts.
  3. Turn on Show When Locked so responders can view the ID from the iPhone lock screen, and consider Share During Emergency Call where it is offered.
  4. Tap Done, then revisit the record whenever the information changes.

On Apple Watch, hold the side button until the sliders appear and slide Medical ID. You can also open Settings > SOS > Medical ID. Apple says that, in the United States and Canada, shared Medical ID information may be sent to emergency services when you call or text 911 or use Emergency SOS. That geographic qualification does not mean the same sharing is available everywhere.

Know how to use Emergency SOS

Press and hold the Apple Watch side button to display the emergency controls, then use the emergency-call slider. Siri or Messages can also be used to contact emergency services where those services are supported. Your emergency contacts can be alerted after an emergency call, so add contacts you trust and explain that they may receive your location or an alert.

Calling normally needs an active cellular connection or Wi‑Fi calling. A Watch without a usable connection may not be able to reach emergency services. Satellite emergency communication is a separate, limited capability available only on supported devices and in supported regions; do not treat it as a universal substitute for cellular or Wi‑Fi service.

Review Fall Detection

Fall Detection is worth checking if you have a compatible Apple Watch and may be at risk from a hard fall. It can alert you, contact emergency services, and notify emergency contacts, but Apple warns that it cannot detect every fall.

Turn it on and choose when it operates

  1. On Apple Watch, open Settings > SOS > Fall Detection.
  2. Choose Always On for protection throughout the day, or Only During Workouts if you prefer to limit detection to exercise sessions.
  3. On the iPhone, the equivalent path is Watch app > My Watch > Emergency SOS > Fall Detection.

Automatic calling after a hard-impact fall depends on Wrist Detection being enabled. If Wrist Detection is off, the Watch cannot use the documented automatic-call behavior after a hard-impact fall. High-impact activities can also trigger an alert even when you are not injured, so respond to the prompt rather than assuming every alert is an emergency.

What happens after a detected fall

The Watch presents an alert and lets you indicate that you are fine or start an emergency call. If it detects that you are immobile and unresponsive for about one minute, Apple documents a 30-second countdown before it automatically tries to contact emergency services. The call still depends on the available cellular or Wi‑Fi calling route. Make sure your emergency contacts and Medical ID are complete so the alert is useful if a call is placed.

Check Crash Detection on supported models

Crash Detection is designed to respond to a detected severe car crash. Apple says it is on by default on supported Apple Watch models. Its control is available at Settings > SOS > Crash Detection > Call After Severe Crash.

Understand the response sequence

After a suspected severe crash, the Watch issues an alert. If you do not respond or cancel it, Apple documents a 10-second interval followed by a 30-second countdown before an automatic emergency call. The feature cannot detect every crash, and reaching emergency services normally requires cellular service or Wi‑Fi calling. Keep the setting enabled unless you have a specific reason to change it, and check the model and regional requirements for your Watch.

Use Check In for planned safety updates

Check In is for situations where someone should know that you arrived, finished an activity, or remained safe for a set period. Start a Check In from the Watch when the feature is available, select a friend, and choose the trigger: arrival at a destination, completion of a workout, or expiration of a selected time period.

If the check-in is not completed, Apple lets you choose what information is shared. Review those choices before relying on the feature, especially if location, battery level, or other status information could reveal more than you intend. Check In is a communication aid, not continuous professional monitoring.

Choose health notifications that answer a real need

Apple Watch health features can highlight changes and help you keep records. The reviewed Apple documentation presents them as tracking and notification tools; it does not establish them as diagnostic tests. Feature availability varies by model, region, and software version.

Heart-rate and rhythm alerts

  • High and low heart-rate notifications: Set thresholds that make sense for you in the Heart section of the Watch app or Health app. They are intended to flag an unusually high or low reading while you appear inactive.
  • Irregular-rhythm notifications: These can identify patterns that may suggest atrial fibrillation (AFib). They are not a continuous diagnosis and should not replace medical evaluation.
  • AFib History: This is intended for people already diagnosed with AFib who want to review an estimate of time spent in AFib. Follow the setup requirements shown in Health before enabling it.
  • Low Cardio Fitness: Turn on alerts if you want a notification when estimated cardio-fitness levels are low for your age and sex. Treat the estimate as a prompt to discuss health, not as a diagnosis.

Sleep schedules and goals

In the Sleep app, set a regular schedule, a target duration, and the days that schedule applies. Wear the Watch overnight if you want sleep history and overnight measurements. A schedule can also support a wind-down routine, but it does not guarantee that you will sleep or that every night will be recorded.

Medications

The Medications feature in the iPhone Health app lets you add prescription medicines, vitamins, and supplements, then log doses from the Watch. Enter the exact item and schedule you were prescribed. Logging is useful for a record and reminders; it does not administer medication or verify that a dose was taken.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Cycle Tracking

Cycle Tracking records menstrual-cycle information and can use heart-rate and wrist-temperature data to produce estimates. Add periods and symptoms consistently, and review which sensor data your model and region support. Estimates are not contraception and should not be treated as a medical diagnosis.

Vitals and overnight measurements

Vitals can display overnight measurements such as heart rate, respiratory rate, wrist temperature, blood oxygen, and sleep duration when the Watch, region, and software support them. Overnight wear and prior Health setup are required for the relevant readings. Look for sustained deviations from your usual range rather than reacting to one isolated value, and seek professional advice for symptoms.

Other safety features worth knowing

Siren

Apple Watch Ultra models include a Siren that can produce an audible signal to help draw attention. Learn its control before hiking, traveling, or working alone; an emergency feature is less useful if you have never practiced opening it.

Satellite communication

Apple documents satellite communication as limited to supported Ultra models and regions. The current guide states that Ultra 2 and earlier do not support it. Confirm your exact model and location in Apple’s current feature documentation before depending on satellite contact.

A practical setup order

  1. Finish Medical ID and contacts. Verify names, phone numbers, conditions, allergies, and medications.
  2. Practice Emergency SOS. Know the side-button gesture and confirm your Watch can place a call where you normally use it.
  3. Enable Fall Detection. Select Always On or Only During Workouts, and leave Wrist Detection enabled if you want automatic calling after a hard-impact fall.
  4. Check Crash Detection. On a supported model, confirm Call After Severe Crash is enabled.
  5. Try Check In. Send a test check-in to a trusted person and review the information that would be shared if you failed to complete it.
  6. Add only useful health features. Configure heart alerts, sleep, medications, Cycle Tracking, or Vitals according to your goals and the data your model supports.
  7. Recheck after updates or travel. Software changes, a new Watch, a different region, or a change in cellular service can affect availability.

Common limitations and troubleshooting

  • No emergency call: Check cellular coverage, Wi‑Fi calling, and whether your carrier supports emergency calling from the Watch. Satellite options, if any, are restricted by model and region.
  • Fall Detection did not react: Apple says some falls are missed. Confirm Fall Detection is enabled and Wrist Detection is on for automatic calling; do not expect high-impact detection to identify every incident.
  • Too many fall alerts: High-impact workouts can trigger alerts. Use Only During Workouts if that matches your risk profile, while recognizing that protection outside workouts will be reduced.
  • Health data is missing: Wear the Watch as required, especially overnight for Sleep and Vitals, and complete the corresponding Health setup. Model and regional restrictions can also explain missing metrics.
  • Contacts did not receive useful information: Update emergency contacts and Medical ID, and review Check In’s incomplete-check-in sharing choices.

Apple’s safety and health documentation emphasizes that availability depends on the Watch model, region, and software. Configure what your device supports, keep the records current, and treat every alert or estimate as assistance rather than a guarantee of detection or diagnosis.
