Ask your doctor about a new high blood pressure treatment when your confirmed readings remain above target, your current treatment is not working well or is causing problems, or your health circumstances have changed. The right choice depends on a pattern of readings, your overall cardiovascular risk, other conditions and medicines, and your preferences—not one reading in isolation. Don’t start, stop, or change a prescription on your own.
When is it worth discussing a new treatment?
Bring up treatment options at a routine appointment if repeated, properly taken readings remain high, or sooner if you have concerns about your current plan. A conversation is also appropriate when side effects, difficulty taking a medicine, cost, access, or possible interactions make the plan hard to follow. Your clinician can assess whether the issue calls for a different dose, another medicine, a combination, closer monitoring, or another approach.
The 2025 American Heart Association (AHA)/American College of Cardiology (ACC) adult high blood pressure guideline replaces the 2017 guideline. It retains these blood pressure categories for adults:
| Category | Blood pressure |
|---|---|
| Normal | Below 120/80 mm Hg |
| Elevated | Systolic 120–129 mm Hg and diastolic below 80 mm Hg |
| Stage 1 hypertension | Systolic 130–139 mm Hg or diastolic 80–89 mm Hg |
| Stage 2 hypertension | Systolic at least 140 mm Hg or diastolic at least 90 mm Hg |
These categories do not diagnose you or determine treatment by themselves. A healthcare professional confirms the diagnosis and considers your circumstances. For stage 1 hypertension, the 2025 framework uses the PREVENT-CVD equations to estimate 10-year total cardiovascular disease risk; the guideline’s cited risk assessment uses 7.5% or greater as a threshold for starting drug therapy in this group. That threshold is one part of a clinical decision, not a self-prescribing rule. Other clinical factors, social circumstances, and your preferences matter, particularly when a risk estimate is near the threshold. Read the AHA/ACC 2025 guideline summary and AHA’s explanation of the updated risk assessment.

What if my blood pressure is still high on medication?
Share your readings and how you take your medicines with your care team. They can check whether the readings reflect a persistent pattern, review the treatment plan, and consider whether another option or a different combination is appropriate. The 2025 guideline discusses medication options and single-pill combinations, but what fits depends on your health and other medicines.
Do not stop or change a prescribed medicine without talking to a healthcare professional. The AHA advises discussing concerns with a doctor, nurse, pharmacist, or another member of the care team; finding a useful dose or combination can take time, and more than one prescription may be needed. Its patient guidance puts it this way: “The important thing is to talk with them and to follow their treatment plan.” See the AHA’s guidance on managing high blood pressure medications.

How to prepare for the appointment
Home monitoring can help you and your clinician understand the pattern rather than relying on a single office reading. Bring your recorded results and the monitor, if practical, so the care team can discuss how you measure and what the readings show. The guideline supports home monitoring and team-based care. The reviewed guidance does not establish a particular monitor model, so ask your clinician or pharmacist for help choosing a device if needed.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errors- Bring a record of home readings, including when you took them, and note any unusual circumstances.
- List every prescription medicine, over-the-counter drug, and supplement you use, including how often you take it.
- Describe side effects, missed doses, difficulties with the schedule, and any cost or refill-access barriers.
- Tell the clinician about other health conditions or changes that may affect the decision.
Useful questions include: Are these readings reliable, and what pattern do they show? How do my overall risk and other conditions affect treatment? What benefit should I expect, and what side effects or interactions should I watch for? If we change the plan, when should we review my response?

How to compare treatment options
If your clinician offers more than one approach, ask how each fits your health and day-to-day life. Consider the expected blood pressure and cardiovascular benefit alongside practical drawbacks:
- Possible side effects, contraindications, and interactions with your current medicines or supplements.
- How often you need to take it and whether a simpler regimen or single-pill combination is appropriate.
- Cost, insurance coverage, pharmacy access, and whether you can keep obtaining it reliably.
- What monitoring is needed and when the care team will reassess readings, benefit, and side effects.
Some over-the-counter medicines and supplements can interact with treatment or raise blood pressure. Don’t use a product marketed as a “natural” remedy in place of prescribed treatment; the AHA cautions that such claims may not be reliable and products can affect other medicines or raise blood pressure. Ask your care team before adding one.

Is a reading over 180/120 an emergency?
AHA patient guidance treats a reading above 180 systolic and/or 120 diastolic differently depending on symptoms. If that reading comes with chest pain, shortness of breath, back pain, numbness, weakness, vision changes, or difficulty speaking, call 911. If it is above that threshold without those symptoms, contact a healthcare professional promptly for instructions. Do not try to manage a very high reading by adjusting your medicine yourself; follow local emergency guidance. See the AHA’s hypertensive-crisis guidance.

For context, the AHA estimated that 46.7% of U.S. adults had high blood pressure in its August 14, 2025 announcement of the updated guideline. That population estimate does not indicate an individual person’s risk. AHA, August 14, 2025.
