Call emergency services now for trouble breathing, swelling of the face or throat, collapse, severe confusion, or severe low blood sugar that cannot be treated safely. Seek prompt medical assessment for persistent, severe abdominal pain (especially pain spreading to the back), ongoing vomiting or diarrhea, suspected gallbladder disease, or other symptoms that are worsening or causing dehydration.
Nausea, diarrhea, vomiting, constipation, and abdominal discomfort can occur with GLP-1 medicines. “Common” does not mean every episode is harmless: urgency depends on severity, persistence, dehydration, other symptoms, the specific medicine, and whether insulin or an insulin-releasing medicine is also being used. This U.S.-focused guide explains when to contact a prescriber, when to seek urgent care, and when to use emergency services. It cannot diagnose a condition or replace your product label and clinician’s instructions.

Emergency warning signs
Severe allergic reaction
Get emergency help for difficulty breathing or swallowing, rapidly developing swelling of the lips, tongue, face, or throat, widespread hives with breathing symptoms, fainting, or rapidly worsening symptoms after an injection. The U.S. Food and Drug Administration (FDA) instructs patients to stop Zepbound immediately and seek medical help if a severe allergic reaction is suspected. Mounjaro labeling identifies anaphylaxis and angioedema as serious hypersensitivity reactions. Other GLP-1 products have their own instructions, so follow the label for the medicine prescribed to you while arranging emergency care.

Severe hypoglycemia or inability to self-treat
Low blood sugar is especially important when a GLP-1 medicine is combined with insulin or a medicine that stimulates insulin release (an insulin secretagogue). That combination can increase the risk of hypoglycemia. Follow the individualized diabetes action plan you were given. Emergency help is appropriate when the person is severely confused, unconscious, having a seizure, unable to swallow safely, or too impaired to take the instructed treatment. Do not put food or drink in the mouth of an unconscious or convulsing person.
Symptoms that need prompt medical assessment
Persistent, severe abdominal pain
Persistent, severe abdominal pain is not something to dismiss as ordinary indigestion. The Mounjaro prescribing information describes possible pancreatitis as “persistent severe abdominal pain, sometimes radiating to the back and which may or may not be accompanied by vomiting.” Wegovy labeling gives a similar warning pattern. Pain can occur with or without vomiting, so the absence of vomiting does not rule out a serious problem.
Contact a clinician promptly or seek urgent assessment if this pattern occurs. The Wegovy label tells patients to discontinue promptly and contact their physician if pancreatitis is suspected; Mounjaro labeling says to discontinue if suspected and initiate appropriate management. Do not independently restart, change, or permanently stop treatment outside the instructions for your product and the advice of your clinician.

Ongoing vomiting or diarrhea
FDA patient information for Wegovy says to contact a healthcare provider for severe or persistent gastrointestinal symptoms. Repeated vomiting or diarrhea can remove enough fluid to cause dehydration, and product labeling links volume depletion with worsening kidney function. Warning signs include inability to keep fluids down, very little or dark urine, faintness when standing, unusual sleepiness, confusion, a racing pulse, or increasing weakness.
Even if nausea began after a dose increase, do not assume that duration makes it safe. Call the prescribing team when symptoms continue, interfere with drinking or eating, prevent you from taking other essential medicines, or are accompanied by severe pain, fever, blood, fainting, or reduced urination.
Possible gallbladder disease
Gallbladder problems require clinical evaluation rather than self-diagnosis from pain location alone. Contact a healthcare provider for persistent or intense upper-abdominal pain, pain after meals, fever, repeated vomiting, yellowing of the skin or eyes, pale stools, or unusually dark urine. FDA labeling notes acute gallbladder disease with GLP-1 medicines and recommends follow-up when it is suspected. Do not assume gallbladder disease occurs only after rapid or substantial weight loss; it can occur without that history.

Vision changes while taking Wegovy
People taking Wegovy, particularly those with type 2 diabetes, should contact their physician about new or worsening vision changes. Sudden or major loss of vision is urgent; use emergency services if it occurs with other severe symptoms or you cannot travel safely. The Wegovy label contains product-specific warnings, so do not apply this instruction automatically to every GLP-1 medicine.
Palpitations or a racing heartbeat at rest on Wegovy
Tell your healthcare provider about palpitations or a racing heartbeat while resting during Wegovy treatment. Seek urgent help for chest pain, fainting, severe shortness of breath, or a sustained rapid heartbeat with weakness. These symptoms can have causes unrelated to the medicine, which is why an assessment is needed instead of assuming a dose effect.
Common digestive effects: when monitoring is reasonable
FDA information lists nausea, diarrhea, vomiting, constipation, abdominal discomfort or pain, and other digestive effects among common Zepbound side effects. Similar symptoms are reported with other GLP-1 medicines. A mild episode that is improving, allows normal hydration, and has no red-flag features can often be monitored using the plan from your prescriber.

Contact the care team if a “common” effect becomes severe, persists, repeatedly returns after doses, causes dehydration, or prevents normal food, fluid, or medicine intake. Do not use the word common as a reason to ignore escalating symptoms. Keep a simple record of the time symptoms began, doses taken, fluids tolerated, urine output, blood-glucose readings when relevant, and any co-administered insulin or secretagogue; this can help the clinician triage you.
Before anesthesia, surgery, or deep sedation
Tell the surgeon, anesthesiologist, dentist, or procedural team that you use a GLP-1 medicine before any planned procedure involving general anesthesia or deep sedation. Wegovy delays stomach emptying, and its labeling addresses pulmonary aspiration risk during anesthesia or deep sedation. The procedural and prescribing clinicians must decide whether timing, fasting, or other precautions are needed. Do not stop a GLP-1 medicine on your own in preparation for a procedure.
Medication mix-ups, compounded products, and wrong doses
Why product and dose verification matters
FDA says unapproved GLP-1 versions are not reviewed for safety, effectiveness, or quality before marketing. It advises obtaining a prescription from a doctor and filling it at a state-licensed pharmacy. Compounded products may use different concentrations or instructions from FDA-approved pens, making a volume-based dose difficult to verify from a vial alone.

What to do after a possible dosing error
- Do not take another dose until you have spoken with the prescriber or pharmacist about what was administered and when.
- Keep the vial, pen, packaging, pharmacy label, and written instructions available. Do not guess the amount in a syringe or try to “correct” a dose yourself.
- Call emergency services for severe abdominal pain, repeated vomiting, fainting, severe weakness, confusion, breathing trouble, or other immediate danger.
- For less severe symptoms or uncertainty about the product, contact the prescriber or pharmacist promptly for individualized instructions.
FDA reported 990 adverse-event reports associated with compounded semaglutide and more than 730 associated with compounded tirzepatide as of May 31, 2026. Reported effects included gastrointestinal symptoms, fainting, dehydration, pancreatitis, and gallstones. FDA cautions that these reports are likely underreported and do not establish that the product caused every event; they are not incidence rates.

GLP-1 medicines generally require particular attention to hypoglycemia when used with insulin or an insulin secretagogue. Ask your diabetes team for a written action plan covering your glucose threshold, fast-acting carbohydrate amount, recheck timing, glucagon use if prescribed, and when to call emergency services. Caregivers should know where the plan and rescue medicine are kept. Never delay emergency help because a glucose reading is unavailable when the person is severely impaired.
Suicidal-thought warnings and the 2026 FDA update
Older online pages or package information may contain a warning about suicidal ideation or behavior. On January 13, 2026, FDA stated: “FDA’s evaluation did not identify an increased risk of suicidal ideation or behavior with the use of GLP-1 RA medications.” The agency reviewed 91 placebo-controlled trials involving 107,910 patients and said the analysis did not show increased suicidal ideation or behavior or several other psychiatric adverse events. FDA requested removal of the suicidal-behavior warning from certain labels. Labels can change at different times, so check the current label for your exact medicine.

How to make the right call
- Identify the product. Write down the exact brand, active ingredient, dose, formulation, and whether it is FDA-approved or compounded.
- Screen for immediate danger. Check for breathing difficulty, swelling, collapse, seizure, severe confusion, inability to swallow, or severe pain. Use emergency services for life-threatening symptoms.
- Check hydration and glucose risk. Note ongoing vomiting or diarrhea, urine output, dizziness, and whether insulin or an insulin secretagogue is involved.
- Use the product label. Instructions differ among Wegovy, Mounjaro, Zepbound, and other medicines. Follow the label’s action for suspected pancreatitis, allergic reaction, or other serious effects.
- Call with useful details. Tell the clinician when symptoms started, the last dose and any recent dose change, what you can drink, glucose readings, other medicines, and whether a procedure is planned.
- Do not improvise treatment changes. Do not skip, double, restart, or permanently discontinue doses unless the prescribing or procedural clinician instructs you to do so.
