A new breast or armpit lump, or a breast or nipple change that is unusual for you, should be assessed by a healthcare professional. Breast pain by itself is often not a sign of cancer, but pain that persists should be discussed with a clinician. Seek urgent advice if pain comes with fever or chills, or if the breast becomes red, hot or swollen. Most breast changes are not cancer, but symptoms alone cannot reliably identify their cause.

Which breast changes should you get checked?

Contact a healthcare professional about a new lump in the breast or armpit, unusual thickening or firmness, or another change that is new for you. Other changes that warrant assessment include:

  • Skin dimpling, puckering, redness or scaling.
  • A newly inverted nipple or a nipple rash.
  • Discharge other than breast milk, particularly if it may contain blood.
  • A change in the breast’s size or shape.

These signs are reasons to seek assessment, not proof of cancer. Many breast changes have benign causes, including cysts and fibroadenomas. Do not try to decide whether a lump is safe based on whether it hurts, moves or feels hard. The NHS advises getting a breast or armpit lump or other unusual change checked; the National Cancer Institute describes breast changes that need follow-up.

When is breast pain urgent?

Under NHS guidance for the UK, seek urgent advice if breast pain occurs with a very high temperature or feeling hot, cold or shivery, or if an area of the breast is red, hot or swollen. The NHS also advises urgent contact for pain accompanied by a hard lump that does not move, discharge that may be blood-streaked, a change in breast shape, skin dimpling, or a nipple rash or newly sunken nipple. The NHS route is UK-specific; elsewhere, contact your local healthcare service for the appropriate urgent-care option.

Does breast pain by itself mean cancer?

Usually, no: breast pain alone is not a reliable cancer test and is often related to non-cancer causes. Period-related pain may begin up to two weeks before a period, worsen, then ease when the period ends; it can feel dull, heavy or aching, affect both breasts and spread toward the armpit. Other possible causes include pregnancy, menopause-related hormone changes, some medicines, injury or strain in the neck, shoulder or back, and infection such as mastitis or a breast abscess. The NHS describes these patterns and possible causes.

Pain that persists should be discussed with a clinician. The National Cancer Institute says, “If you have breast pain that persists, it’s important to see a doctor.” Pain can occur with some cancers and alongside other breast changes; do not use the pattern or absence of pain to rule out a problem. The American Cancer Society explains that pain by itself is usually not a sign of most breast cancers.

How soon should you arrange an assessment?

Arrange a routine appointment for a new lump, unusual breast or nipple change, or persistent pain. The NHS also advises routine GP advice when pain is not improving, painkillers are not helping, or you have a family history of breast cancer. Use urgent advice instead when the warning signs described above are present.

A recent normal mammogram does not settle a new symptom. Follow up a new lump or change even if your last mammogram was normal, as the National Cancer Institute advises.

What might happen at a breast assessment?

A clinician may examine your breasts and, if the cause is unclear, refer you to a breast clinic. Assessment may include an examination, mammogram or ultrasound and, when needed, a needle biopsy. The NHS says these tests are often done during the same visit, but this is not guaranteed; biopsy results take longer and may take about a week. The specific tests and timing depend on the assessment.
