An Autism-Spectrum Quotient (AQ) score reflects answers to questions about traits associated with autism. It can be a clue in screening or assessment, but it does not diagnose autism—and a low score cannot rule autism out. For adults in the UK pathway covered by NICE, an AQ-10 score of 6 or higher is one reason to offer a comprehensive assessment; clinical suspicion can also justify one.
What does an Autism Spectrum Quotient score mean?
The AQ is a self-report questionnaire. Your result summarizes how you answered questions about autism-associated traits; it is not a definitive yes-or-no test. A higher score may support seeking further assessment, but only a broader clinical evaluation can determine whether you meet diagnostic criteria.
Interpret any threshold in context: the AQ has different versions, and a score’s meaning depends on the version, the population, and the purpose for which it is being used.

AQ-10: a UK adult referral pathway
NICE recommends considering the 10-item AQ (AQ-10) for adults with possible autism who do not have a moderate or severe learning disability. In this pathway, a score of 6 or above is a reason to offer a comprehensive autism assessment. NICE also says to offer assessment when autism is suspected on clinical judgement, taking into account any history provided by an informant. The threshold guides referral; it is not a diagnosis. AQ-50: a different questionnaire
The 50-item AQ is not interchangeable with the AQ-10. NICE’s evidence review examined AQ-50 cutoffs of 32 and 26 in included studies and noted that sensitivity and specificity varied with the cutoff and study population. These are research thresholds, not a universal diagnostic line, and AQ-10’s cutoff of 6 should not be transferred to the 50-item version. NICE full evidence review.
Does a high score mean you are autistic?
No. A high AQ score indicates that your responses warrant consideration alongside other information; it cannot establish an autism diagnosis. NICE’s adult pathway uses AQ-10 as one aid to deciding whether to offer assessment, not as a stand-alone diagnostic test.
Evidence also cautions against treating a questionnaire as a verdict. A 2016 study in an adult diagnostic-service sample found that self-reported AQ scores did not significantly predict who received a clinical diagnosis. That finding applies to the study sample and does not mean questionnaires have no role in assessment. Ashwood et al., 2016.

Can a low AQ score rule out autism?
No. A result below a screening threshold does not independently exclude autism. NICE’s recommendation allows clinical suspicion to lead to an assessment even when the AQ-10 score is below 6.

NICE’s 2021 surveillance review reported that one study found a 64% false-negative rate among adults with suspected autism. That is a finding from a particular study, not a general estimate of AQ-10 accuracy for every person or setting. The review recorded validity concerns, but NICE left its recommendation unchanged because more research was needed. Stakeholders also confirmed they would not use AQ-10 alone to screen for autism. What happens in a comprehensive assessment?
An assessment considers a wider picture than a questionnaire score. NICE’s adult guideline describes attention to developmental history, current functioning, coexisting conditions, sensory features, and direct observation. For more complex diagnostic or assessment situations, clinicians may consider formal assessment tools. NICE adult autism guideline, recommendations 1.2.4–1.2.8.

What should you do after an AQ-10 score of 6 or higher?
- Arrange a conversation with a healthcare professional or local autism assessment service. Share your AQ-10 result and explain the traits or difficulties that led you to complete it.
- Describe your history as well as your current situation. Where possible, include examples from childhood and information from someone who knows your developmental history. NICE’s referral recommendation specifically notes the relevance of informant history.
- Discuss your functioning and support needs. Explain how the traits affect daily life and mention relevant sensory experiences or other health and developmental concerns.
- Follow the local assessment pathway. In the UK adult pathway described by NICE, an AQ-10 score of 6 or above supports offering comprehensive assessment. A clinician’s suspicion may also support an offer regardless of the score.
If you score below 6 but remain concerned, you can still raise those concerns and your history with a professional. The AQ-10 threshold is not a reason by itself to dismiss possible autism.
Who does the NICE AQ-10 recommendation apply to?
This recommendation is UK guidance for adults with possible autism who do not have a moderate or severe learning disability. It is not a universal cutoff for every country, age group, or support need. If you are outside this pathway, ask a local healthcare professional or assessment service what process applies to you; the sources cited here do not establish a single threshold that works across settings.
