A wearable heart-rhythm alert can flag a pattern that may be atrial fibrillation (AFib), and some devices can record a short, user-started ECG. Neither result diagnoses AFib or rules out a heart problem. Treat a finding as a reason to discuss it with a clinician—not as a diagnosis, and not as a reason to change treatment on your own.

Two features, two kinds of evidence

Wearables may offer a pulse-based irregular-rhythm notification, an on-demand ECG recording, or both. They work differently and answer different questions.

Feature What it does What its result means
Pulse-based irregular-rhythm notification Software analyzes pulse signals from the wearable and may flag a pattern suggestive of AFib. On Samsung’s reviewed feature, analysis is opportunistic: it examines pulse-rate data when the wearer is still and enough data are available, rather than continuously monitoring and notifying about every episode. FDA overview; Samsung FDA summary A prompt to seek clinical assessment, not proof that AFib is present.
On-demand ECG The wearer starts a recording that produces a short, single-channel trace similar to Lead I on the Samsung and Withings products described in their FDA summaries. Samsung FDA summary; Withings FDA summary A limited recording that may be classified for rhythms covered by that product’s labeling. It is not a multi-lead clinical ECG or a general cardiac work-up.

A pulse notification is a software interpretation of pulse data; an ECG is a recording of electrical activity. An ECG-capable wearable can provide a trace for review, but a classification still covers only the supported results and the recording captured at that moment.

What to do if your watch flags possible AFib

  1. Read the exact result. Note whether it came from a passive irregular-rhythm notification or an ECG recording, and save any trace or report the app provides.
  2. Contact a healthcare professional for confirmation. The American Heart Association’s September 23, 2025 interview quotes electrophysiologist José A. Joglar advising people whose watch says they have AFib to see a doctor to confirm it. Samsung’s FDA indication likewise says not to take clinical action based on its output without consulting a qualified professional. AHA interview; Samsung FDA summary
  3. Do not self-diagnose or change medication based on the wearable alone. A clinician can decide whether further testing or treatment is appropriate.

Wearable signals can help surface a pattern worth discussing. The FDA says these tools are not meant to provide diagnoses, while noting that notifications may prompt doctors to prescribe tests, drugs, or other treatments when needed. FDA overview

KardiaMobile 1-Lead EKG Monitor, Detects Normal AFib & Arrhythmias, HSA&FSA
  • Simple to Use Without a Subscription: No Bluetooth, Wi-Fi, cords or PC needed. Place the device near your smartphone. Monitor your heart by placing your fingers or thumbs on the silver KardiaMobile EKG sensors. Know in 30 seconds whether your heart rhythm is normal.

Why no alert does not mean your rhythm is normal

A watch can only analyze the data it captures, and its detection windows and thresholds depend on the product. Samsung’s FDA indication says its feature analyzes data opportunistically while the wearer is still, requires enough data, may not notify for every AFib episode, and is not intended to imply that no disease process is present when no notification appears. Samsung FDA summary

So a quiet app is not a clinical all-clear. If you have symptoms or concerns, seek medical advice rather than waiting for the watch to alert you.

What published performance figures do—and don’t—show

An American Heart Association report published November 14, 2021 described a Fitbit-funded US study conducted from May to October 2020. It enrolled more than 455,000 smartwatch or fitness-tracker users who had not self-reported a prior AFib diagnosis. The study’s algorithm operated during inactivity and required at least 30 minutes of sensed irregular rhythm before indicating possible AFib.

  • Among participants who received a notification, 32% had AFib detected on a subsequent ECG patch.
  • Among notified participants older than 65, the algorithm corresponded to concurrent AFib 97% of the time.

These are findings from one study of one algorithm and its participants—not universal accuracy figures for every wearable, user, or setting. The study did not test whether wearable screening reduces strokes. AHA research report, November 14, 2021

Alerts can help, but can also lead to worry and follow-up tests

A notification can prompt timely follow-up, but a false notification may also lead to anxiety, additional tests, or medication. The FDA identifies these as possible consequences; Joglar told the AHA that false alarms can cause anxiety in low-risk people and that he does not recommend wearables across the board for the general population. FDA overview; AHA interview, September 23, 2025

The evidence here does not establish a market-wide false-positive rate, so a particular alert should be interpreted in context by a clinician.
