Wearable health trackers can record movement and, depending on the model, estimate or measure selected body signals. But a watch, ring or band does not measure every health value, and a reading is not automatically a diagnosis. Accuracy varies by metric, device, person and conditions; whether a feature is intended and authorized for medical use matters too.

What can a fitness tracker measure?

Most wearables collect motion data, which they use to report activity such as steps. Some models also use optical sensors to estimate heart rate and related patterns. Specific devices may offer ECG recordings, blood-oxygen estimates, temperature measurements or sleep-related features. These capabilities are not standard across all watches, rings or bands: check the exact model and its supported features.

A useful distinction is whether a device records a sensor signal or estimates a value from signals and software. Step counts and daily scores, for example, are outputs of a device’s measurement and processing—not a direct reading of a medical condition. Use “detects” or “diagnoses” only for a specific feature and intended use that supports those claims.

The FDA maintains a list of medical devices that incorporate sensor-based digital health technology, including qualifying wearables such as watches, rings, patches and bands. The list is not comprehensive, and a device’s regulatory status applies to that device and its stated intended use—not to every feature in its product category. (FDA, “Medical Devices that Incorporate Sensor-based Digital Health Technology.”)

How accurate are smartwatches for health?

There is no single accuracy score for a smartwatch. A living systematic review of Apple Watch measurement studies, published in npj Digital Medicine in 2026, found that performance differed substantially by metric. The review searched nine databases for studies through September 24, 2025; its findings describe the reviewed Apple Watch studies, not every brand or current model.

  • Heart rate: Average bias was small in the reviewed studies, but measurement variability was moderate. A small average difference from a reference does not guarantee that a particular person’s reading is close.
  • Blood oxygen: Average bias was low, but the limits of agreement were wide. Individual estimates may differ meaningfully from a reference measurement.
  • Sleep and steps: The review found moderate accuracy, not perfect agreement.
  • Energy expenditure: Error was often large or inconsistent, so calorie-burn estimates should not be treated as precise measurements.

Fit and skin contact, activity, the person wearing the device and measurement conditions can affect a result. A trend against your own baseline may help you notice a change, but it does not establish its cause or confirm a diagnosis.

What Apple’s newer claims do—and don’t—establish

In a September 2026 announcement, Apple described more frequent heart-rate and heart-rate-variability sampling and a readiness score for Apple Watch Series 12 and Ultra 4. Apple also described a comparison study involving more than 1,000 participants and called the heart-rate sensing the “highest accuracy.” That is a company claim about named models, not an independent, peer-reviewed ranking across wearables.

Can a smartwatch measure blood sugar?

No—not on its own. The FDA warns consumers: “Do not buy or use smartwatches or smart rings that claim to measure blood glucose levels.” In its February 21, 2024 safety communication, the agency said it had not authorized, cleared or approved any smartwatch or smart ring intended to measure or estimate glucose independently.

A watch app that displays readings from a separate, authorized glucose monitor is a different use: the watch is showing data from that monitor, not measuring glucose by itself. If you need glucose monitoring, ask a health care professional about an appropriate authorized device.

Can a smartwatch measure blood oxygen reliably?

A wearable’s oxygen-saturation value is an estimate, and pulse-oximeter readings have known limitations. FDA materials identify skin pigmentation as one potential influence on accuracy. The FDA’s January 2025 draft guidance also discusses factors that can affect performance; it is non-binding and is not a finalized rule.

The FDA’s consumer guidance lists other possible influences, including poor circulation, skin thickness, skin temperature, tobacco use and nail polish. If you are concerned about symptoms or a reading, do not rely on the number alone; consider how you feel and contact a health care provider.

Can an Apple Watch ECG detect a heart attack?

No. Apple says its ECG app, available on Apple Watch Series 4 or later and Apple Watch Ultra models, creates a waveform similar to a single-lead ECG. It can provide heart-rate and rhythm information and classify a recording as atrial fibrillation or sinus rhythm in supported circumstances. Apple Support states: “The ECG app cannot detect a heart attack.”

The ECG app also cannot detect stroke, blood clots, high blood pressure, high cholesterol, congestive heart failure or every form of arrhythmia. A smartwatch ECG is not a full diagnostic workup, and real-world recordings may be inconclusive or unclassifiable.

Apple reports that a clinical trial of approximately 600 subjects found 99.6% specificity for sinus-rhythm classification and 98.3% sensitivity for atrial-fibrillation classification among classifiable results. Those are Apple-reported results for this feature in a controlled trial, not general measures of smartwatch diagnostic accuracy; Apple cautions that real-world recordings may produce more inconclusive results.

  • Metric: Identify whether you care about heart rate, steps, sleep, oxygen saturation, ECG or another output.
  • Evidence: Look for independent validation for that metric, exact model and relevant population. Separate peer-reviewed findings from a manufacturer’s claims.
  • Intended use and regulatory status: Check whether the feature is a wellness estimate or part of a device authorized for a stated medical use. A regulatory status does not extend automatically to other features or products.
  • How the number is produced: Determine whether it is a recorded sensor signal, an estimate derived from signals, or a score generated by an algorithm.
  • Conditions and fit: Consider activity, device contact and relevant skin or environmental factors that may affect the measurement.
  • Model and location: Features and regulatory availability can vary by device generation and geography.
